Item Association of Hypertension and Smoking with Ischaemic Stroke.(2010) Kirtania, K; Sultana, N; Ahmed, S; Khatun, AHypertension is one of the most important modifiable risk factors for ischemic stroke. Cigarette smoking is a risk factor for atherosclerotic disease. There is a strong relationship between hypertension and cigarette smoking with ischaemic stroke. A case control study was undertaken to see the association of hypertension and cigarette smoking with ischaemic stroke. The study was done from January to December 2009 in the Department of Biochemistry, Dhaka Medical College, Dhaka. A total of 60 subjects were selected as study population. Among them 30 were diagnosed case of ischaemic stroke and 30 were age-and sex-matched control subjects. It was found that 60% patients of case group and 20% respondents of control group were hypertensive and diffrence was significant. Study showed that 56.66% of cases and 53.33% of controls were smoker and the findings were insignificant. The mean duration of smoking was 27.41 ± 2.98 years in cases and 15.63 ± 2.85 years in controls which was significant. The study suggests that hypertension is significantly associated with ischaemic stroke and longer duration of smoking also associated with ischaemic stroke.Item Association of Serum Calcium and 24 Hours Urinary Excretion of Calcium in Patients of Preeclampsia.(2010) Khatun; Kirtania, K; Shima, T S; Ferdous, S; Shahnaz, A; Yeasmin, F; Sultana, NPreeclampsia is a leading cause of fetal growth retardation, infant morbidity, mortality and maternal death. Several biochemical changes occur during the course of the disease, among them serum total calcium and 24 hr urinary calcium excretion rate have drawn remarkable attention. A case control study was designed to see the association of serum calcium and 24 hours urinary excretion of calcium in patients of pre- eclampsia. The study was done from July 2008 to June 2009 in the Department of Biochemistry, Dhaka Medical College, Dhaka. Total 60 subjects were selected as study population. Among them 30 were diagnosed case of preeclampsia and 30 were age- and sex-matched normal pregnant female healthy control. It was found that the mean measured serum calcium level in case group were 1.96 ±0.17 ( mean±SD ) mmol / l and that of control group were 2.34±0.12 (mean±SD) m mol/ l respectively. The mean 24 hr urinary calcium excretion level were 94.40 ±10.68mg/24 hr (mean±SD) in case group and that of control group were 154.35±66.68 (mean ±SD) mg / 24 hr respectively. The study suggests that low level of serum calcium and urinary excretion of calcium was associated with Pre-eclampsia.Item Biochemical Factors associated with Breast Cancer in Bangladeshi Women.(2013) Hussain, DAS; Ahmed, S; Hoque, M; Rabbi, S M R; Masood, S; Shafi, T; Parvin, T; Chaudhury, H SBreast Cancer is the commonest maligrancy in woment. A study was carried out in the Department of Biochemistry, Dhaka Medical College to investigate some of the biochemical features that are associated with breast cancer occurring in Bangladeshi women. Thirty diagnosed breast cancer patients and thirty healthy women were selected. A questionnaire was used for collecting information regarding age, family history, physical activity and exercise, BMI, diet, smoking, alcohol abuse, addiction, details of menstrual and obstetric history, breast-feeding, parity, use of contraceptives and HRT. Blood samples were collected and tested for fasting lipid profiles, serum estrogen and progesterone. The study has revealed that high levels of serum total cholesterol (P<0.05), TAG (P<0.05), LDL-C (P<0.05) and low level of serum and HDL-C (P<0.05) were found to be associated with breast cancer. The study further reveals that blood levels of estrogen (P<0.05) and progesterone (P<0.05) were found to be lower than that of the controls.Item Bioinformatics: a promising field for Medical Biochemists.(2014) Majumder, ManojitItem Blood Lipid Profile in Acute Coronary Syndrome and Chronic Stable Angina Patients.(2014) Parvin, T; Rahman z, M H; Ferdousi, S; Shahnaz, A; Mahal, M; Ahmed, S F; Ferdous, B A; Hussain, D A SThis study was designed to compare the blood lipid profile and conventional cardiovascular risk factors in patients with acute coronary syndrome (ACS) and chronic stable angina (CSA). A total of 110 patients, 55 of which were of ACS and 55 with chronic stable angina (CSA) who underwent coronary angiography in the Department of Cardiology, BSMMU, Dhaka were enrolled for the study. Lipid profile and age, sex, diabetes mellitus, hypertension, hyperlipidemia, smoking habit, family history of premature coronary artery diseases (CAD) of the patients were recorded and compared. Mean±SD age of the studied patients was 52.2±10 years. Prevalence of diabetes mellitus (p<0.05) and hypertension (p <0.05) was significantly higher in patients with CSA than that with ACS. Frequency of hyperlipidemia, smoking habit and family history of CAD was similar between groups. Patients with CSA had significantly higher triple vessel diseases (TVD) (p < 0.001) and significantly lower high-density lipoprotein cholesterol (HDLc) (p < 0.05) compared to that of patients with ACS. ACS patients had significantly higher single vessel diseases (SVD) (p <0.001) compared to that of CSA but the distribution of low-density lipoprotein cholesterol (LDLc), total cholesterol and triglycerides were variable between groups. This study showed that CSA patients had substantially higher frequency of having some CVD risk factors with alteration in certain lipid profile parameters and higher number of triple vessels disease than patients with ACS. This might aid in selecting appropriate criteria for angiographic evaluation and better management of patients with coronary artery diseases.Item Cardiac Troponin-I And CK-MB for Risk Stratification in Acute Myocardial Infarction (First Attack): A Comparative Study.(2011) Joarder, S; Hoque, M; Towhiduzzaman, M; Salehuddin, Af; Islam, N; Akter, M; Kamal, I MMyocardial infarction is associated with release of two important enzymes. The enzymatic diagnosis is mainly based on the measurement of CK-MB and troponin-I. Cardiac troponin- I(cTnI) is known to have higher specificity and analytic sensitivity than CK-MB for detection of myocardial injury & risk stratification. These are used both as diagnostic and prognostic marker. This prospective observational study included 60 patients of 40-65 years age range, diagnosed as acute myocardial infarction. The mean ages were 50± 8 years and 53±8 years respectively. Male and female patients included were 86.7% and 13.3%; BMI was 25.3±1.5. The two important cardiac markers troponin-I and CK-MB were studied in 60 patients, admitted in the hospital with acute MI. Blood samples to estimate these markers were collected from the patients after admission at 6-9 hours, 9-24 hours and after 24 hours and their mean values with ±SD were calculated, evaluated and compared between the two groups of patients with low and high risk MI. The patients with low risk MI were those who recovered early and the high risk patients improved later in comparison to low risk group. Out of 60 patients, 37 had troponin-I level>1.5 ng /ml. Among them 29 developed high risk MI and 8 recovered earlier than high risk group. 23 patients had troponin-I <1.5 ng /ml, out of whom 10 were high and 13 were low risk. The difference of troponin-I levels between high and low risk groups of patients was statistically significant (p<0.01). On the other hand CK-MB level was >7 ng /ml in 33 patients. Out of them 22 patients developed high and 11 patients were low risk but 18 patients out of 27 who had CK-MB <7 ng /ml became high and 9 patients were low risk. The difference of outcome in respect to higher and lower values of CK-MB between the two groups was not statistically significant (p>0.05). Both troponin-I and CK-MB were estimated in all 60 patients on three occasions. The mean troponin-I levels were statistically significant between the high and the low risk groups on all occasions .On the contrary, the values of CK-MB were not statistically significant on two occasions but was significant (p < 0.01) on one occasion when it was estimated at 9 - 24 hour. Serum cTnI is better and more characteristic biomarker than CK-MB for risk prediction and prognosis evaluation in AMI patients.Item Characteristics of Insulin and Androgen Status in Polycystic Ovary Syndrome.(2010) Banu, L M; Wahab, M A; Debnath, B C; Begum, D; Arslan, M IPolycystic Ovary Syndrome (PCOS) was originally described as a syndrome of amenorrhea, hirsutism and obesity associated with enlarged polycystic ovaries. There is increased androgen level and in some, insulin resistance (IR). Etiological relationship of androgen excess and IR in PCOS is not established. Influence of obesity on PCOS is controversial. This study was designed to see the androgen and insulin status in PCOS among obese and non-obese patients. It was a case-control study. Of total 80 study subjects, 60 primary infertile women suffering from PCOS were cases (30 obese and 30 non-obese). Age and BMI matched 20 healthy women having normal menstrual cycles were controls (10 obese and 10 non-obese). Age range of all were 20-40 years. Fasting plasma glucose, fasting S. Insulin and free Testosterone were measured. Insulin resistance (IR) was assessed by fasting glucose to insulin ratio (<4.5). Subjects with DM or known endocrine disorders that may be associated with abnormal S.Insulin or plasma glucose concentration were excluded. No significant difference of fasting plasma glucose between PCOS (obese or non-obese) and respective controls (P>0.5, in each) were observed. Significant difference of fasting S. Insulin and testosterone were observed between PCOS (both obese and non-obese) and respective controls (P<0.01 in each), but there was no significant difference between obese and non-obese PCOS (P>0.05). There was no significant difference of S.Testosterone between obese and non-obese PCOS(P>0.05). There was also no significant difference of IR between obese and non-obese PCOS, but the ratio was <4.5 (indicating IR in both). There were no significant correlation of S.Insulin with Testosterone in any group of PCOS (obese and non-obese) (P>0.05). Increased S.Insulin and Testosterone was seen in PCOS irrespective of BMI. Further studies with larger sample size is recommended to assess etiological relationship between insulin and testosterone in PCOS.Item A Comparative Study on the Hepatoprotective Effect of Tamarindus indica and Vitamin E in Long Evans Rats.(2013) Shammi, N J; Choudhry, Z K; Khan, MI; Hossain, M MThe protective effects of ethanolic extract of Tamarindus indica leaves and seeds in compoarison to vitamin E, were studied on paracetamol induced hepatotoxicity in Long Evans Rats. Different groups of animals were administered in the paracetamol (1500mg /kg, p.o.) for 7 days. Ethanolic extracts of leaves and seeds of Tamarindus indica (1250mg/kg) with parallel vitamin E (500 mg/kg), were administered to paracetamol pretreated rats. On treatment with paracetamol a significant increase in alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin and alkaline phosphatase were observed. On administration of ethanolic extracts of leaves and seeds a significant decrease in the level of alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin and alkaline phosphatase (ALP) were observed and histopathological examination of liver tissue revealed an almost return to normal architecture. The result were almost comparable to vitamin E, a known hepatoprotective agent.Item Comparison of C-reactive protein among controlled, moderately controlled and uncontrolled diabetic subjects in a Bangladeshi population.(2011) Saiedullah, M; Rahman, M R; Shaha, S S; Begum, S; Hayat, S; Kamaluddin, S MPoor glycemic control is significantly associated with the development of macrovascular complications of diabetes. C-reactive protein (CRP) is considered as a sensitive and independent risk factor for cardiovascular events. The aim of this study was to compare elevated CRP among controlled, moderately controlled and uncontrolled diabetic subjects in a Bangladeshi population. CRP and glycosylated hemoglobin A1c (HbA1c) were measured in 226 confirmed diabetic subjects purposively and randomly. Elevation of CRP in the moderately controlled (relative risk 1.64, P<0.01) and uncontrolled diabetic subjects (relative risk 1.8, P<0.001) were significantly higher than controlled diabetic subjects. HbA1c was significantly associated with the likelihood of elevated CRP in the moderately controlled diabetic subjects (OR:2.87, 95% CI: 1.41 to 5.87, P<0.01) and in the uncontrolled diabetic subjects (OR:3.83, 95% CI: 1.94 to 7.58, P<0.001). This study revealed that the likelihood of elevated CRP is associated with the increase of HbA1c in diabetic subjects.Item Comparison of Conventionally Measured Serum Bicarbonate Values with Bicarbonate Values Obtained in Arterial and Venous Blood Gas Analysis.(2012) Nargis, W; Ahamed, B U; Hossain, Z; Zabeen, S; Houqe, NBicarbonate (HCO3-) measurement in serum or plasma from a sample of venous blood is routinely practiced in hospital patient management. HCO3- status can also be assumed from Blood gas analysis requiring arterial blood as sample which is cumbersome for both patients and doctors. This study was undertaken to evaluate the extent of agreement among biocarbonate values obtained during venous, arterial blood gas analysis and conventionally measured serum bicarbonate levels in a group of intensive care unit (ICU) patients to determine whether conventionally measured serum HCO3- (from peripheral venous blood) and calculated HCO3-values (from arterial blood gas [ABG] analyzers) can be used interchangeably. A total of 51 adult patients with diverse medical conditions, presenting at a tertiary health centre ICU were enrolled in this study when deemed by the treating physician to have an ABG analysis. Arterial and venous samples were taken as close in time as possible for blood gas analysis and routine blood tests. Bland-Altman analyses were used to compare the three methods. The HCO3- levels from ABG, Venous Blood gas (VBG) and tconventionally measured serum HCO3- showed acceptably narrow 95% limits of agreement using the Bland-Altman method. VBG reveals higher level of agreement with the ABG bicarbonate values compared to measured serum HCO3-. Thus, venous blood can be an alternate for arterial blood where ABG analyzer is available. conventionaly serum HCO3- measurements can also be useful and used as substitute for an expensive ABG analyzer in resource constrained health care sectors when required.Item Core Facility in Medical Research.(2014) Biswas, S KItem Correlation between serum bilirubin and serum ferritin Level in thalassaemia patients.(2011) Sultana, N; Sadiya, S; Rahman, M HThalassaemia is the most common hereditary disorder in the world including Bangladesh. Beta thalassaemia major and Hb-E thalassaemia both are common in our country. Iron overload causes most of the mortality and morbidity associate with thalassaemia. To assess the iron over load and liver function a cross sectional comparative study was carried out in the Department of Biochemistry, Dhaka Medical College, Dhaka in collaboration with Thalassaemia Center and Department of Pathology, Dhaka Shishu Hospital, Dhaka during the period of July 2006 to June 2007. The study was carried out with the patients who visited regularly in Dhaka Shishu Hospital Thalassaemia Centre (DSHTC) and had multiple transfusions (more than five) and age more than 2 years. To compare the state of liver function with normal healthy individuals' normal healthy persons were also included. Total 70 subjects were included in this study. The study subjects were distributed into two groups, the group - A (cases, n=40) and group - B (healthy controls, n=30). According to the major types of thalassaemia present in our country, group -A again divided into two, group - AI b-thalassaemia major (n=12) and group - AII of Hemoglobin E b-thalassaemia (n=28). The mean of serum Bilirubin in group - A and group - B were (2.04 ±0.70) mg/dl and (0.67±0.15) mg/dl respectively. Group - A had higher serum bilirubin than group -B in p value <0.001.The mean level of serum bilirubin in group - AI was (1.70±0.70)mg/dl and the mean of bilirubin in group AII was (2.18±0.66) mg/dl. Group -AI had lower serum bilirubin than group AII in p value <0.05. Pearson's correlation between serum bilirubin and serum ferritin had r value -0.26 was statistically not significant. Mean level of serum ferritin in the total was (2729.40 ± 1935.87) ng/ml, minimum level was 304 ng/ml and maximum was 7256ng/ml. Mean level of serum ferritin in the patients of b - thalassaemia major was (4098.67 ± 1598.63) ng/ml, minimum level was 1212ng/ml and maximum was 7560ng/ml. Mean level of serum ferritin in the patients of HbE b - thalassaemia was (2232.57± 1598.63) ng/ml, minimum level was 304ng/ml and maximum was 630ng/ml. Group AI had higher level of serum ferritin than group AII in p value <0.05.Item Correlations of Serum Magnesium and Potassium in Acute Myocardial Infarction, Chronic Ischemic Heart Disease and Normal Healthy Volunteers of Bangladesh.(2010) Choudhury, M B K; Hossain, M M; Akhtaruzzaman, M; Uddin, M M Jamal; Rahman, M S; Islam, M S; Jinnah, C M K; Hoque, NMagnesium (Mg) and potassium (K) are the major intracellular cations whose presence in the serum are low, but minor changes of those may show a remarkable change in the various body functions specially in the heart. The study was designed to find out the correlation between serum Mg and K in acute myocardial infarction (AMI), chronic ischemic heart disease (CIHD) and normal healthy volunteers. It was carried out over a period of 18 months in the Department of Biochemistry, Bangabandhu Sheikh Mujib Medical University (BSMMU) in collaboration with Department of Cardiology, Sir Salimullah Medical College & Mitford Hospital (SSMC & MH) and Atomic Energy Center, Dhaka. A total of 101 subjects were included in which 32 subjects were AMI, 34 CIHD and 35 normal healthy volunteers. Serum glucose and serum creatinine were estimated to exclude diabetes and renal dystrophies. Estimation of serum CK-MB and ECG tracing were done as diagnostic tools of AMI and to categories the subjects into various groups. Serum Mg was estimated by Atomic absorption spectrophotometer and serum K by Ion sensitive electrode. The present study shows that there is a strong positive correlation of serum Mg and K in AMI, CIHD and healthy control subjects (r = 0.566, p<0.01 level). So it is suggested to estimate and supplement both Mg and K in IHD patients for their better management.Item Cystatin C -a Promising Marker of Glomerular Filtration Rate.(2013) Jahan, Nowrase; Ferdousi, SohaniGlomerular filtration rate (GFR) is the best single measure of overall function of kidney. GFR is routinely assessed by measuring the concentration of endogenous serum markers such as blood urea nitrogen and serum creatinine (SCr). Although widely used these endogenous marker are not ideal and do not perform optimally in certain clinical settings. The purpose of this review is to critically review the potential utility of Cys C as a new promosing markers of GFR and to review whether Cys C had any advantage over routinely used endogenous marker in different population group.Item Cystatin C: A Better Predictor of Kidney Function in Diabetic Patients .(2011) Shima, T S; Khatun, A; Yeasmin, F; Ferdousi, S; Kirtania, K; Sultana, NSerum cystatin C is a new promising marker of renal function. The aim of this study was to analyze serum cystatin C as a better predictor of renal function in diabetic nephropathy. In 60 diagnosed diabetic patients, serum cystatin C and serum creatinine were assessed. Glomerular filtration rate was estimated based on the cystatin C concentration according to Cockcroft- Gault formula and based on serum creatinine concentration according to Larsson formula. DTPA-GFR (Diethylenetriamene pentaacetate Renogram) was done as reference standard. The cross tabulation of DTPA-GFR was done with eGFR- creatinine and eGFRcystatin C. The calculated sensitivity, specificity and accuracy of eGFR- creatinine were 85%, 87.2% and 85% respectively. The eGFR- cystatin C showed higher sensitivity, specificity and accuracy than eGFR- creatinine in studied diabetic subjects. The cystatin C showed more significant correlation, r=0.78, p<0.001 than serum creatinine, r=0.59, p<0.001 with DTPA-GFR in diabetic patients. This study demonstrates that serum cystatin C may be used for early prediction for renal function impairment in diabetic kidney disease.Item Don't we need diagnostic facilities for genetically determined diseases.(2013) Biswas, S KItem Donor Site Evaluation After Lingual Mucosal Graft Harvest for Urethroplasty.(2012) Asaduzzaman, M; Quddus, M R; Islam, M S; Ahmed, K; Rosy, S K; Bhuiyan, AKMZIThis study was carried out in the Department of Urology, National Institute of Kidney Diseases and Urology, (NIKDU), during the period from Jan' 09 to Dec' 10 to assess the complications at donor site after lingual mucosal graft harvesting for urethroplasty. A total of 30 patients with mean age of 36.6 years (rang 21 to 56 years) and mean urethal stricture length of 36 mm (range 22 to 59 mm) who underwent urethroplasty for anterior urethral strictures using dorsal onlay of a lingual mucosal graft (LMG) were selected for the study. The site of the harvest graft was ventrolateral mucosal lining of the tongue. Donor site complications like pain, numbness, tightness, slurring of speech, salivatory changes and difficulty in protrusion of tongue were noted. The mean period of follow-up was 14 months (range 6-18 months). At the first postoperative day, 96% of the patients experienced pain at donor site and 26% had slurring of speech. Pain was mild to discomforting in 60% and distressing to excruciating in 37% of the patients. By third postoperative day, 22 (73%) patients were pain free, 06 (20%) suffered from mild pain and 02 (6%) suffered from discomforting pain only and none had slurring of speech. On the fifth postoperative day, only 02 (6%) patient suffered pain. By day 6 of surgery, all patients were pain free. Only 01 (3.3%) patients reported numbness which persisted during the whole period of follow up. The study showed that LMG is easy to harvest and associated with less postoperative pain, minor risk of donor site complications and without any functional or esthetic deficiency. So tongue may be the best alternative donor site for anterior urothroplasty.Item Dyslipidemia in Insulin Resistance: Cause or Effect.(2014) Al-Mahmood, A K; Afrin, S F; Hoque, NLipids are of one of the four basic types of molecules of life and its derivatives serve diverse functions in the body. Generally acclaimed functions of lipids include shock absorption and insulation, and energy storage of the body. Phospholipids and cholesterol form the integral part of cell membrane. In addition cholesterol serves as the precursor for bile salts, male and female sex hormones, vitamin D and adrenocortical hormones. Complex lipids consist of neutral lipid core of cholesterol esters and or triacylglycerol and proteins are the chylomicrons, VLDL, LDL and HDL which serve as the carriers of fats, fatty acids and other lipids in the body. Complex lipids are tightly controlled in the body. Its dysregulation has been mainly linked to obesity, diabetes and insulin resistance though the issue is yet to be clearly understood. The present review evaluates recent reports in this regards and try to explain the relationship between dyspidemia and insulin resistance.Item Effect of Ramadan Fasting on Serum Lipid Profile of Bangladeshi Female Volunteers.(2014) Akhtaruzzaman, M; Hoque, N; Choudhury, M B K; Uddin, M M Jamal; Parvin, TThe study was conducted to evaluate the effects of fasting for a period of one month during Ramadan on the lipid profile of Bangladeshi female volunteers who fasted during the Arabic month of Ramadan, when there occurs a change, both in the pattern and timing of dietary intake. Findings of the study showed that Ramadan fast significantly reduced serum Total cholesterol (TC) (p=0.030) and Low density lipoprotein (LDL-C) (p=0.011). A statistically nonsignificant elevation of triglyceride (TG)) was observed (p=0.598). High density lipoprotein cholesterol (HDL-C) also found to be raised significantly at the end of fasting (p = 0.037). Findings of the study revealed that fasting during the month of Ramadan changes lipid profile pattern in an aniatherogenic direction and may be beneficial to health.Item Effects of Diet and Exercise in Middle Aged Subjects with Impaired Glucose Tolerance.(2014) Haque, M M; Shahjahan, M; Haque, M S; Absar, NPatients with impaired glucose tolerance (IGT) are now considered as being pre-diabetic, which indicates their relatively high risk for developing diabetes mellitus associated with abnormal metabolic syndrome and cardiovascular diseases. However, dietary modification and physical exercise may play a critical role in this respect. To determine the influence of dietary modification and physical exercise in subjects with impaired glucose tolerance in Bangladesh, thirty three newly detected otherwise healthy subjects with IGT, aged 30-63 years, were randomly selected to participate in a 12 weeks diet and exercise program. Substantial improvement in glucose tolerance was observed at the end of 12 weeks particularly in middle aged subjects (41-50 years). Mean fasting blood glucose and 2 hr post load glucose value were reduced significantly. Glucose tolerance was reverted to normal in 66.7% of the participants, remained unchanged in 26.7% and deteriorated to diabetes in 6.7%. Significant reduction in serum total cholesterol, LDL cholesterol along with mild deterioration in HDL cholesterol and increase in triglyceride values were observed. It was found that the principles of 'prudent diet' in combination with physical exercise are highly effective in improving glucose tolerance, lowering total cholesterol and LDL cholesterol in IGT subjects.
