    Immunopathogenesis and Treatment of Rheumatoid Arthritis: Recent Developments.

    Rheumatoid arthritis (RA) is a chronic inflammatory disease occurring three times more in females throughout the world affecting 1-2% of the adult population in all ethnic groups, usually in the age group of 25-60 years. Although the role of CD4 + T helper lymphocytes in the aetiopathogenesis has been studied for more than three decades, the focus on CD4 + T helper type 17 (Th17) lymphocytes and its associated cytokines is much more recent. The cytokines such as IL-17 and IFN-g induce secondary cytokines such as IL-1, TNF-a , etc which possibly cause inflammation in joints. This cytokine cascade, therefore, offers a number of points and opportunities for immunointervention in RA. The present review article highlights some of the major aspects of the immunopathogenesis that involve Th17 cells and their association relevant to recent developments in the treatment of RA.

    Metabolic Syndrome in Hypothyroid Patients.

    Thyroid functions affect metabolic syndrome (MetS) parameters including blood pressure, fasting plasma glucose, serum triglycerides and HDL-Cholesterol. But the relationship between MetS and thyroid functions is yet to be identified clearly. Metabolic syndrome is a state in which most features of hypothyroidism can be seen. The aim of the present study was to investigate the frequency of MetS in hypothyroid patients. Thirty one patients with overt hypothyroidism, 32 patients with subclinical hypothyroidism (SCH) and 58 euthyroid controls were enrolled in this study. NCEP-ATP III criteria was used to diagnose metabolic syndrome. Majority of the participants were in the age group of 30-40 years. Body mass index and waist circumference, blood pressure, fasting plasma glucose and serum triglycerides were found to be higher in hypothyroid patients compared to that of euthyroid controls (p<0.001). On the other hand serum HDL-C was found to be significantly lower in hypothyroid patients compared to that of euthyroid controls (p<0.001). Prevalence of MetS was 82.5% in the hypothyroid group and 27.6% in eythyroid controls (p<0.001). No significant difference was found between SCH (81.3%) and overt hypothyroid (86.7%) groups in respect to prevalence of MetS (p<0.05). The findings of the study suggest a need to investigate the presence of hypothyroidism during the management of MetS patients.

    Blood Lipid Profile in Acute Coronary Syndrome and Chronic Stable Angina Patients.

    This study was designed to compare the blood lipid profile and conventional cardiovascular risk factors in patients with acute coronary syndrome (ACS) and chronic stable angina (CSA). A total of 110 patients, 55 of which were of ACS and 55 with chronic stable angina (CSA) who underwent coronary angiography in the Department of Cardiology, BSMMU, Dhaka were enrolled for the study. Lipid profile and age, sex, diabetes mellitus, hypertension, hyperlipidemia, smoking habit, family history of premature coronary artery diseases (CAD) of the patients were recorded and compared. Mean±SD age of the studied patients was 52.2±10 years. Prevalence of diabetes mellitus (p<0.05) and hypertension (p <0.05) was significantly higher in patients with CSA than that with ACS. Frequency of hyperlipidemia, smoking habit and family history of CAD was similar between groups. Patients with CSA had significantly higher triple vessel diseases (TVD) (p < 0.001) and significantly lower high-density lipoprotein cholesterol (HDLc) (p < 0.05) compared to that of patients with ACS. ACS patients had significantly higher single vessel diseases (SVD) (p <0.001) compared to that of CSA but the distribution of low-density lipoprotein cholesterol (LDLc), total cholesterol and triglycerides were variable between groups. This study showed that CSA patients had substantially higher frequency of having some CVD risk factors with alteration in certain lipid profile parameters and higher number of triple vessels disease than patients with ACS. This might aid in selecting appropriate criteria for angiographic evaluation and better management of patients with coronary artery diseases.

    Effect of Ramadan Fasting on Serum Lipid Profile of Bangladeshi Female Volunteers.

    The study was conducted to evaluate the effects of fasting for a period of one month during Ramadan on the lipid profile of Bangladeshi female volunteers who fasted during the Arabic month of Ramadan, when there occurs a change, both in the pattern and timing of dietary intake. Findings of the study showed that Ramadan fast significantly reduced serum Total cholesterol (TC) (p=0.030) and Low density lipoprotein (LDL-C) (p=0.011). A statistically nonsignificant elevation of triglyceride (TG)) was observed (p=0.598). High density lipoprotein cholesterol (HDL-C) also found to be raised significantly at the end of fasting (p = 0.037). Findings of the study revealed that fasting during the month of Ramadan changes lipid profile pattern in an aniatherogenic direction and may be beneficial to health.

    Serum Proinsulin in Bangladeshi Subjects with Impaired Glucose Tolerance.

    Hyperproinsulinemia is commonly present in subjects with impaired glucose tolerance. The present study was undertaken to investigate the proinsulin level in Bangladeshi IGT subjects and to explore its association with insulin resistance. This observational study was conducted under a case-control design with IGT subjects (n=50) and controls (n=44). IGT was diagnosed following the WHO Study Group Criteria. Serum glucose was measured by glucose-oxidase method, serum lipid profile by enzymatic method and serum insulin and serum proinsulin were measured by ELISA method. Insulin secretory capacity (HOMA%B) and insulin sensitivity (HOMA%S) were calculated from fasting serum glucose and fasting serum insulin by homeostasis model assessment. The study subjects were age- and BMI- matched. Mean (±SD) age (yrs) of the control and IGT subjects were 40±6 and 40±5 respectively (p=0.853). Mean (±SD) BMI of the control and IGT subjects were 23±3 and 22±2 respectively (p=0.123). Fasting glucose was not significantly higher in IGT subjects, but serum glucose 2 hours after 75 gm glucose load was significantly higher in IGT subjects. Median (Range) value of fasting serum glucose (mmol/l) of control and IGT subjects were 5.3 (3.8-6) and 5.2 (4-12) respectively; (p=0.297). Median (Range) value of serum glucose (mmol/l) 2 hours after 75 gm glucose load of control and IGT subjects were 6.1 (3-7.8) and 7.9 (5- 21) respectively; (p=0.001). Fasting TG was significantly higher in IGT subjects and LDL-c was significantly lower in IGT subjects. Serum Total cholesterol and HDL-c were not significantly different between the IGT and control subjects. Median (Range) value of fasting serum TG (mg/dl) of control and IGT subjects were 119 (51-474) and 178 (82-540) respectively; (p=0.001). Median (Range) value of fasting serum T chol (mg/dl) of control and IGT subjects were 180 (65-272) and 186 (140-400) respectively; (p=0.191). Median (Range) value of fasting serum HDL-C (mg/dl) of control and IGT subjects were 29 (19-45) and 31 (15-78) respectively; (p=0.914). Median (Range) value of fasting serum LDL-C (mg/dl) of control and IGT subjects were 117(29-201) and 111(41- 320) respectively; (p=0.001). Fasting serum proinsulin was significantly higher in IGT subjects. Median (Range) value of fasting serum proinsulin (pmol/l) of control and IGT subjects were 9.2(1.8-156) and 17(3-51) respectively; (p=0.001). Insulin secretory capacity (HOMA%B) was higher but insulin sensitivity (HOMA%S) was significantly lower in case of IGT subjects. Median (Range) value of HOMA%B of control and IGT subjects were 97(46-498) and 164(17-300) respectively; (p=0.001). Median (Range) value of HOMA%S of control and IGT subjects were 68(19-270) and 39(15-110) respectively (p=0.001). In multiple regression analysis a significant negative association was found between fasting proinsulin and insulin sensitivity (p=0.037). The data led to the following conclusions: a) Insulin resistance is the predominant defect in Bangladeshi IGT subjects. b) Basal proinsulin level is significantly increased in IGT subjects. c) Insulin resistance is negatively associated with serum proinsulin in IGT subjects.

    Dyslipidemia in Insulin Resistance: Cause or Effect.

    Lipids are of one of the four basic types of molecules of life and its derivatives serve diverse functions in the body. Generally acclaimed functions of lipids include shock absorption and insulation, and energy storage of the body. Phospholipids and cholesterol form the integral part of cell membrane. In addition cholesterol serves as the precursor for bile salts, male and female sex hormones, vitamin D and adrenocortical hormones. Complex lipids consist of neutral lipid core of cholesterol esters and or triacylglycerol and proteins are the chylomicrons, VLDL, LDL and HDL which serve as the carriers of fats, fatty acids and other lipids in the body. Complex lipids are tightly controlled in the body. Its dysregulation has been mainly linked to obesity, diabetes and insulin resistance though the issue is yet to be clearly understood. The present review evaluates recent reports in this regards and try to explain the relationship between dyspidemia and insulin resistance.

    Effects of Diet and Exercise in Middle Aged Subjects with Impaired Glucose Tolerance.

    Patients with impaired glucose tolerance (IGT) are now considered as being pre-diabetic, which indicates their relatively high risk for developing diabetes mellitus associated with abnormal metabolic syndrome and cardiovascular diseases. However, dietary modification and physical exercise may play a critical role in this respect. To determine the influence of dietary modification and physical exercise in subjects with impaired glucose tolerance in Bangladesh, thirty three newly detected otherwise healthy subjects with IGT, aged 30-63 years, were randomly selected to participate in a 12 weeks diet and exercise program. Substantial improvement in glucose tolerance was observed at the end of 12 weeks particularly in middle aged subjects (41-50 years). Mean fasting blood glucose and 2 hr post load glucose value were reduced significantly. Glucose tolerance was reverted to normal in 66.7% of the participants, remained unchanged in 26.7% and deteriorated to diabetes in 6.7%. Significant reduction in serum total cholesterol, LDL cholesterol along with mild deterioration in HDL cholesterol and increase in triglyceride values were observed. It was found that the principles of 'prudent diet' in combination with physical exercise are highly effective in improving glucose tolerance, lowering total cholesterol and LDL cholesterol in IGT subjects.

    Serum Lipid Profile in Ischemic Heart Disease: A Cross-Sectional Analytical Study.

    Risk of coronary heart disease and other forms of atherosclerotic vascular diseases, rises with plasma cholesterol concentration and in particular with the rise of ratio of total cholesterol to high density lipoprotein (HDL-C) cholesterol. A much weaker correlation also exists with plasma triglyceride concentration. Extensive large-scale randomized trials have shown that lowering total cholesterol and LDL-C concentrations reduces the risk of cardiovascular events including death, myocardial infarction and stroke and reduces the need for revascularisation.This cross-sectional analytical study was designed to observe association between lipid profile level with chronic ischaemic heart disease and the study was conducted in the Department of Biochemistry, Dhaka Medical College, Dhaka from July 2010 to June 2011. A total of 50 cases were selected purposively according to the selection criteria from the patients admitted in the Department of Cardiology, Dhaka Medical College Hospital with chronic ischaemic heart disease (IHD). Diagnosed IHD patients were taken as cases and 50 age- & sex- matched healthy subjects were taken as controls. Serum TC, TG, LDL-C & HDL-C were measured in all study subjects.The mean±SD of serum TC, TG, HDL-C and LDL-C concentration in cases were 314.54±73.72 mg/dl, 288.04±60.45 mg/dl, 36.02±4.12 mg/dl, and 178.62±22.7 mg/dl respectively and in controls were 174.64±18.97 mg/dl, 119.42±12.47 mg/dl, 43.04±2.58 mg/dl & 126.28±11.45 mg/dl respectively. Serum Total Cholesterol, TG & LDL-C were found to be significantly higher in cases than that of controls. Serum HDL-C was found to be significantly lower in cases than that of controls. The present study reveals that the patients with chronic ischemic heart disease is accociated with significantly higher levels of serum TC, TG and LDL-C whereas HDL-C was found to be lower in IHD patients.

    Evaluation of the Antibody Response against Hepatitis C Virus Infection in Patients on Maintenance Hemodialysis (MHD): A Pilot Study.

    Majority of the patients with end stage renal disease (ESRD) are on maintenance hemodialysis (MHD) in Bangladesh. Dialysis patients are at high risk for contracting blood borne infection including hepatitis C virus (HCV) infection. The aim of this study was to evaluate the antibody response of hepatitis C virus infection in patients on MHD by detecting different viral markers in blood. A total of 88 patients with chronic kidney disease (CKD)were recruited from BIRDEM and BSMMU during the period from June 2006 to June 2007. Of them 63 patients on MHD and 25 predialysis patients were taken as cases and controls respectivly. Anti-HCV antibody were positive in 38% of dialysis patients but none of the controls were positive for Anti-HCV. When HCV positive MHD patients (38%) were compared to HCV negative MHD patients (62%), it showed that HCV positive patients had longer duration of dialysis (24±25 vs 9±6 months, p<0.001), increased number blood transfusions (29±34 vs 10±9 units, p<0.004) and elevated serum alanine aminotransferase level (35±23 vs 20±9 U/L, p=0.001). Implementing comprehensive infection control program by routine screening of the CKD patients, safe blood transfusion program, reducing transfusion of blood by use of erythropoietin and proper disinfection and cleaning of hemodialysis units may reduce the infection by HCV Virus.

    Serum Lipid Profile, Serum Magnesium and Fasting Serum Glucose in Newly Diagnosed Type 2 Diabetic Subjects.

    Type 2 diabetes is considered as a major health burden due to its rising prevalence and disabling, life threatening complications. Dyslipidemia, often coexisting with T2DM as a feature of insulin resistance, is hypothesized to be linked with altered magnesium homeostasis. This study was designed to evaluate the serum magnesium levels and its influence on serum lipids in type 2 diabetics. Lipid profile, serum magnesium (Mg) and fasting serum glucose (FSG) were measured in 30 newly diagnosed normotensive type 2 diabetic patients chosen as cases (Group II) just before introducing any treatment, and was compared with that of 30 healthy controls (Group I). The serum magnesium was found to be significantly lower (p<0.001) and LDL-c was found to be significantly higher (p<0.01) in cases. The correlation analysis revealed a significant negative association of FSG to serum magnesium (r= -0.720), total cholesterol (r=-0.483) and a positive correlation to HDL-c (r=-0.440). However, serum magnesium showed a significant positive relation only with serum HDL-c (r =0.372, p<0.05). Serum magnesium and lipid fractions showed wide range of variation within the normal reference ranges in the newly diagnosed T2D subjects. Further large scale studies are needed to elucidate the association of serum magnesium with lipid profile changes. Estimation of serum magnesium level may prove useful in T2DM with normal or abnormal lipid levels or in those who are prone to develop dyslipidemia or certain complications associated with dyslipidemia.     Energy Metabolism.

    This review considers how our understanding of energy utilized by energy metabolism has progressed since the pioneering work on this topic in the late 1960s and early 1970s. Research has been stimulated by a desire to understand how metabolic events contribute to the development of the body into the different phases, the need of considering health with which to improve the success of implication on public health. Nevertheless, considerable progress has been made in defining the roles of the traditional nutrients: pyruvate, glucose, lactate and amino acids; originally considered as energy sources and biosynthetic precursors, but now recognised as having multiple, overlapping functions. Other nutrients; notably, lipids, are beginning to attract the attention they deserve. The review concludes by up-dating the state of knowledge of energy metabolism in the early 1970s and listing some future research questions.

    A Comparative Study on the Hepatoprotective Effect of Tamarindus indica and Vitamin E in Long Evans Rats.

    The protective effects of ethanolic extract of Tamarindus indica leaves and seeds in compoarison to vitamin E, were studied on paracetamol induced hepatotoxicity in Long Evans Rats. Different groups of animals were administered in the paracetamol (1500mg /kg, p.o.) for 7 days. Ethanolic extracts of leaves and seeds of Tamarindus indica (1250mg/kg) with parallel vitamin E (500 mg/kg), were administered to paracetamol pretreated rats. On treatment with paracetamol a significant increase in alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin and alkaline phosphatase were observed. On administration of ethanolic extracts of leaves and seeds a significant decrease in the level of alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin and alkaline phosphatase (ALP) were observed and histopathological examination of liver tissue revealed an almost return to normal architecture. The result were almost comparable to vitamin E, a known hepatoprotective agent.

    Biochemical Factors associated with Breast Cancer in Bangladeshi Women.

    Breast Cancer is the commonest maligrancy in woment. A study was carried out in the Department of Biochemistry, Dhaka Medical College to investigate some of the biochemical features that are associated with breast cancer occurring in Bangladeshi women. Thirty diagnosed breast cancer patients and thirty healthy women were selected. A questionnaire was used for collecting information regarding age, family history, physical activity and exercise, BMI, diet, smoking, alcohol abuse, addiction, details of menstrual and obstetric history, breast-feeding, parity, use of contraceptives and HRT. Blood samples were collected and tested for fasting lipid profiles, serum estrogen and progesterone. The study has revealed that high levels of serum total cholesterol (P<0.05), TAG (P<0.05), LDL-C (P<0.05) and low level of serum and HDL-C (P<0.05) were found to be associated with breast cancer. The study further reveals that blood levels of estrogen (P<0.05) and progesterone (P<0.05) were found to be lower than that of the controls.

    Purification and Characterization of Peroxidase From Anthracnose Disease Infected Papaya (Carica papaya L.).

    Peroxidase enzyme was isolated and purified from the pulp of disease infected ripen papaya of local variety by 90% ammonium sulphate precipitation, chromatography on DEAEcellulose followed by hydrophobic chromatography on Phenyl Sepharose CL-4B and the purifications achieved was about 7.2 fold with 2.5% recovery. The purified enzyme was homogeneous as judged by polyacrylamide slab gel electrophoresis. The purified enzyme had a Mr of about 55,000 and 50 000 as determined by gel filtration on Sephadex G-100 and SDS-PAGE, respectively. The molecular mass of the enzyme was found to be very similar under both reducing and non-reducing conditions indicating that the enzyme contains no subunit. The enzyme has the following characteristics: pH optima at 6.0, temperature optima around 38°C, enzyme activity was found to be strongly inhibited in the presence of potassium cyanide and Fe+2 while the activity was found to be remarkably increased in the presence of ammonium sulphate. The Km value for the peroxidase obtained with pyrogallol as substrate was 0.027 mM.

    Evaluation of Adenosine Deaminase (ADA) Activity for Diagnosis of Tubercular Pleural Effusion.

    To find out a sensitive and specific marker for early diagnosis of tubercular pleural effusion, this cross sectional study was carried out in the of National Institute of Diseases of the Chest and Hospital (NIDCH), Dhaka. One hundred and three pleural effusion cases were enrolled in the study. Out of the 103 cases, 62 were tubercular pleural effusion cases and 49 were nontubercular cases. Among the nontubercular cases, 30 cases were due to malignancy, 8 were due to pneumonia and rest 3 cases were due to nephrotic syndrome, congestive cardiac failure and rheumatoid arthritis. Considering 40 U/L as a cut off value for ADA level, the test result was positive in 58 out of 62 patients of tuberculosis indicating sensitivity of the test as 94%; however, among 41 non-tuberculous patients, 5 presented ADA activity level more than 40U/L, which lowers the specificity of the test to 88%. ADA levels were significantly higher in tuberculous than in nontuberculous cases (p value <0.001). It may be concluded that ADA levels are significantly high in patients with tuberculous pleural effusion compared to that in non-tubercular group. Sensitivity (94%) and specificity (88%) of the test in tuberculous pleural effusions are very high, when cut off value set at 40U/L. The result indicated that the analysis of ADA levels in pleural effusion constitute a very useful marker for the diagnosis of tubercular pleural effusion (TPE) which, in addition, can be made quickly in a noninvasive way.

    Cystatin C -a Promising Marker of Glomerular Filtration Rate.

    Glomerular filtration rate (GFR) is the best single measure of overall function of kidney. GFR is routinely assessed by measuring the concentration of endogenous serum markers such as blood urea nitrogen and serum creatinine (SCr). Although widely used these endogenous marker are not ideal and do not perform optimally in certain clinical settings. The purpose of this review is to critically review the potential utility of Cys C as a new promosing markers of GFR and to review whether Cys C had any advantage over routinely used endogenous marker in different population group.

    Renal function impairment in Hypothyroidism.

    Background: Hypothyroidism is an important public health problem in Bangladesh. It is associated with increased risk for atherosclerosis and other complications. The frank development of hypothyroidism is associated with metabolic derangements including dyslipidemia- which is an etiopathologic factor for development of renal impairment. This study was to evaluate whether hypothyroidism is associated with impaired renal function. Methods: Using a cross sectional analytical study design, a total of 111 subjects attending Out Patient Department, Center for Nuclear Medicine and Ultrasound, Bogra Medical College during January 2007 to December 2007 were included purposively. Eighty newly diagnosed hypothyroid patients (Group I) and 31 healthy adults (Group II) were enrolled in this study. Serum thyroid stimulating hormone and serum free thyroxine were assayed by radioimmunoassay. Serum fasting lipid profile, serum creatinine and serum uric acid were estimated by enzymatic colorimetric method. Estimated GFR was calculated using MDRD equation. Results: The mean (±SD) age of in Group I and Group II were 35.59 (±6.91) and 37.35 (±2.78) years and were comparable. In Group I, there were 66 females and 14 males. In Group II, there were 16 females and 15 males. The mean BMI was 25.49 ±2.17 kg/m2 in Group I and 24.24 ±1.99 kg/m2 in Group II. The mean (±SD) Serum total-cholesterol, LDL- cholesterol and tryacylglycerol in Group I were significantly higher than that in Group II. Serum HDL cholesterol in Group I was significantly lower than that in Group II (p<0.001). The mean (±SD) serum creatinine was significantly higher in Group I than Group II (P<0.001). The estimated glomerular filtration rate (eGFR) was lower in Group I compared to Group II (p=0.011). In Group I (Hypothyroid), there were significant correlations of BMI, S Total-Cholesterol, S HDLCholesterol, S LDL-Cholesterol, S triglycerides and S creatinine with serum TSH level. In Group I (Hypothyroid), there were significant positive correlations of BMI and TSH with serum creatinine. Conclusions: Hypothyroidism is associated with dyslipidemia, hyperuricemia and impaired renal function. Therefore, patients presenting with these biochemical abnormalities are recommended to be investigated for hypothyroidism and vice versa.
