To Study The Relationship Of Classification Of Depressive Disorders With The Hahnemannian Concept Of Mental Illness
(Association of Health Professionals and Health Educators, 2022-12) Joshi, H.
Background: Hahnemann (1996) has classified mental illnesses in a rather different way. Heunderstood the mental illness not very different in kind from the physical illness. He classified theillnesses on the principles of cause and effect and the dynamic shift of the illness from the body tomind or vice versa. Aim & Object: To classify patients suffering form Depressive Disorder as per DSM IVcriteria. To determine the relationship of this classification with that enunciated in the 215,216, 221 and225 aphorisms of Organon of Medicine. To evolve the totality of patients on the basis of the aboveevolved Hahnemannian classification of Depressive Disorders. To project a therapeutic plan based onthe finding in 1, 2 and 3 above. Material And Methods: Detailed interview focusing mainly uponsymptomatology for arriving at acomprehensive clinical diagnosis according to DSM IV. Studying theevolutionary pattern of the selected cases to understand them according to the Hahnemannianclassification. Correlating the therapeutic outcome with the principles used and thus discovering anycorrelation between the two approaches. Result and Conclusion: The conclusions range from theconceptual clarity of the applicability of the two systems of classification together to the specificunderstanding of the requirement of appropriate remedy in the appropriate potency and repetitionbased upon a collective understanding of the predisposing and maintaining causes and thesusceptibility. The importance of understanding of the susceptibility came out as the most importantaspect in the combination of the two systems of classification.

(Association of Health Professionals and Health Educators, 2022-12) Joshi, H.
Background:Hahnemann was always at great pains to emphasis the importance of causative factors in the environment-both physical & emotional which could be held responsible for development of illness. According to homoeopathy, mind, body and soul are trinity of life. So along with exciting and maintaining causes, there is need of studying the psycho -somatic correlations. As in many clinical cases, the state of mother during pregnancy leading to skin condition in child is noted. Aim and Objective: To study the various clinical presentations of Pyoderma. To examine time of occurrence & observe whether it is in antecedence with the onset of pyoderma i.e. .to establishes the precipitating factors of pyoderma. To observe the socio-economical scenario of the patient’s life to establish the maintaining causes of pyoderma. To understand the susceptibility and to establish the role of miasms. Material And Methods:Detailed case study to understand patient as a person & the focus will be on the life space of the child, the parents’ mental state before conception, mother’s state during pregnancy, his interactions with people around him both inside & outside family; possible causative events and classifyingthem into precipitating, exciting & maintaining causes.Result:In all the 30 cases no psychological cause has been found. So we can postulate that pyoderma is predominantly a physical disease where the predisposing maintaining and precipitating causes play a vital role. Conclusion:As prescription was based mainly on the symptoms exhibited, recurrence and understanding of susceptibility, the most frequently used potency, were moderate, in frequent repetition (80%).

Analysing The Correlation Of Lipid Profile Pattern In Chronic Rheumatic Heart Disease (RHD) Patients
(Association of Health Professionals and Health Educators, 2022-06) Patel, M; Rana, D; MalhotraS; Patel, T.
Background:The global burden of ARF and RHD is significant, and is predominantly found in populations living in low-resource settings. This study was conducted with a view for analysing the lipid profile variation in patients with chronic Rheumatic Heart Disease (RHD) and correlating the same. The early management of RHD has to be started to prevent further cardiac complications. To date no hypothesis is established on the relation between the Lipid Profile and RHD. This study would be beneficial to Indian cardiologists in defining the optimal treatment by the choice of antibiotic therapy and Lipid lowering agents like Statins.Material And Methods:It was a cross-sectional study carried out on the patients visiting Cardiology Out Patient Department (OPD) at a tertiary care hospital in Ahmedabad and who were diagnosed with Rheumatic Heart Disease (RHD).The blood lipids levels of thepatients were evaluated with current classification schemes and treatment levels for hyperlipidemia based on the National Cholesterol Education Panel’s (NCEP) Adult Treatment Program-3 (ATP-III) guidelines.Result:Total 89 patients were included in the study. 76 patients were associated with dyslipidaemia and 13 patients had normal lipid values. As far as total cholesterol levels were concerned, 23 out of 76 patients (30.26%) had above target levels for total cholesterol. Also 46 patients out of 76(60.52%) had raised triglycerides levels and 47 patients (61.84%) had raised LDL levels. As far as HDL levels were concerned, 38 out of 76 patients (50.00%) had below desirable levels. The frequency of deranged LDL is higher as compared to HDL, triglycerides and total cholesterol levels. If these values are to be co-related to the disease, then LDL as a lipid parameter stands most significant. Conclusion:Morbidity and mortality in RHD is very high in Indian patients and one of the determinants of the cardiovascular morbidity is the increased lipid levels. The frequency of deranged LDL is higher as compared to HDL and total cholesterol levels in these patients. This study was planned to evaluate an entirely novel hypothesis that RHD is an independent risk factor for Atherosclerosis and CAD. The exact reason behind the occurrence is unknown but it can be environmental or genetic. RHD and Dyslipidaemia might engulf younger patients. The study is also aimed at evaluating all lipid parameters with RHD.

Materiovigilance Program Of India (MVPI): Current Need And Status In Country
(Association of Health Professionals and Health Educators, 2022-06) Trivedi, S.
Background:The term “medical device” includes a broad classification of production ranging from therapeutic medicinal devices withlocal uses tohighlyadvanced electronic medicalequipmentandindicativemedicaldevices. These devices vary widely in their production, type and are highly essential for patients’ care. Thus, their manufacturing, distribution, and sales must be managed to ensure their quality, safety, and efficacy. They play a major role in to detection, assessment and management and treatment of many different diseases. That being said, the quality of devices can very different and even the perfectly articulated products can fail in clinical practice. Post-market surveillance istherefore essential to ensure the quality and evaluate the safety and performance of medical devices. Despite the importance placed on surveillance of drug safety, the need for better monitoring of medical device-associated adverse events receives less attention. A well-structured vigilance system is the backbone of a robust regulatory framework to ensure the quality and promote the safe use of medical devices. The regulation of medical devices, however, is a complex and constantly evolving area that is often complicated by legal technicalities. Although the regulations may differ from one country to another.

To Determine The Role Of Imaging Findings On Computed Tomography (CT) And Magnetic Resonance Imaging (MRI) Predictive Of Acute Invasive Rhino-Orbito-Cerebral Mucormycosis (ROCM) In COVID 19 Patients
(Association of Health Professionals and Health Educators, 2022-06) Solanki, T; Patel, A; Shah, S; Kagathara, V; Dhakan, U.
Background:There is a sudden rise of fungal infection with coronavirus disease. This is attributed to the immunomodulation by the disease and the drugs used, diabetes mellitus, steroid use, oxygen inhalation using dirty water, use of zinc and iron supplements, etc. Early diagnosis and prompt medical and surgical intervention is the mainstay of treatment. This can greatly reduce the high morbidity and mortality associated with this disease. The objective of the retrospective study is to describe the imaging findings of acute invasive rhino-orbito-cerebral mucormycosis (ROCM) in 58 patients with severe acute respiratory syndrome coronavirus 2, from SVP hospital with proven mucormycosis. Special emphasis is placed on the signal patterns of sinonasal mucosa, the earliest and most common findings.Material And Methods:We report the sinonasal, orbital and neuroimaging findings in patients of suspected acute invasive ROCM. A total of 58 patient’s scans were analyzed. The study comprises cases performed at two different imaging modalities and a tertiary care hospital from March 23,2021 to September 1, 2021. All the patients had positive reverse transcriptase polymerase chain reaction test for severe acute respiratory syndrome coronavirus 2 and were hospitalized with clinically severe disease as per the guideline s laid down during the second wave in India. They were on intravenous steroids and oxygen. Thirty patients (52%) had diabetes mellitus. All of them presented with headache, facialand/or orbital pain, periorbital puffiness with decreased vision, during the course of treatment. CT or MRI examination of the paranasal sinuses, orbits and brain was done, with intravenous contrast wherever possible. The presence of mucormycosis was confirmed by histological diagnosis in all of them following clinico-radiological diagnosis of acute invasive ROCM. Result:Computed tomography (CT) and magnetic resonance imaging (MRI) of 58 patients showed most commonly involved sinuse s as maxillary and ethmoid sinuses together. Sino-nasal mucosal thickening was the most common finding. Periantral infiltration preceded orbital, cerebral complications, with grossly intact bones. Sinus wall erosions were seen in only patients and maxillary alveolar arch erosionwere frequent findings. CT showed hypodense soft tissue thickening or fat stranding as the predominant finding in involved areas, while MRI showed T2 iso-to hyperintense mucosal thickening with T2 hypointense component as the main finding. Conclusion:MRI is better at demonstrating early mucosal abnormalities, turbinate necrosis, devitalized tissues, orbital apex involvement and intra-cerebral extension. Imaging findings of inflammatory tissue infiltration adjacent to the paranasal sinuses in premaxillary, retroantral fat, facial muscles, pterygopalatine fossa, temporal, infratemporal fossa and extraconal orbital-fat along with typical patterns of sinonasal mucosal thickening should raise the suspicion of acute invasive fungal etiology given the short duration of history and immunocompromised status. High incidence of periantral and orbital extension of the disease is suggestive of acute invasive form of fungal infection. Also the rapidly progressive inflammatory changes without much bone involvement should suggest the suspicion of ROCM. Bony, cerebral and vascular involvements are relatively late complications.
