Item A study of congenital anomalies in newborn.(2010-01) Parmar, Akruti; Rathod, S P; Patel, S V; Patel, S MTotal 4210 babies were studied in the neonatal period immediately after birth, which shows overall 0.88%. incidence of congenital malformations. Incidence was significantly higher (6.1%) in mothers aged > 30 years as compared to younger age group. Religious pattern of mother and consanguinity of marriage had no much effect on the incidence. The incidence was marginally higher in primiparous patients (47.2%). Only in 12% of cases some etiological factors could be found, of which maternal fever in first trimester was the most common. 10 cases with CNS malformations had hydroamnious. The anomaly was detected antenatally in 49%cases with the help of USG. The ration of still birth to live birth was 4.19:1 suggesting that many of the anomalies are incompatible with life. There was no overall difference in the sex of the babies. However most of anencephalic babies were female. Preterm babies had four times more incidence of congenital malformations as compared to term babies. This represents ―phenomenon of nature’s selection‖. Associated hydroamnios also accounts for preterm labour similarly congenital malformations were more commonly found in low birth weight baby. Out of all system involved in congenital mal formation, CNS was the commonest and accounted for 1/3 rd of the cases.Item Study of conservative management of frozen shoulder.(2010-01) Vora, PinakinFrozen shoulder is a condition characterized by pain and global restriction of movement with loss of external rotation. All 77 patients with idiopathic frozen shoulder syndrome diagnosed between January 1995 and January 1998 were included. Study shows that Manipulation followed by physiotherapy should be started earlier rather than late. Some authors believed that after a period of six months most improved pain wise but still have residual restriction of movements after taking treatment in the form of physiotherapy or manipulation under general anesthesia, but they become adapted to do full function of his/ her day to day routine work.Item Comparative study of serum magnesium among hypertensive, normotensive and hypotensive adults.(2010-01) Verma, Sajjanlal; Kumar, ShushilThe possible role of magnesium ions (Mg++) in the pathogenesis of essential hypertension has recently received increasing attention although magnesium has been neglected trace element till now despite its important role in various metabolic reactions involved in high energy transfer reactions, themogenesis, protein synthesis and anabolism because magnesium is a cofactor in various enzymes like alkaline phosphatases, ATPase, creatine kinases, pyruvate kinases and phosphogluco mutases etc. study was conducted by Department of Physiology, Department of Biochemistry and Department of Medicine, KGMC, Lucknow. Exploratory study was conducted to find out awareness about sujok therapy among Medical Student.Item Arthroscopic treatment of popliteal cyst and visualization of its cavity through the posterior portal of the knee.(2010-01) Deliwala, Ujjval; Jadeja, Harshvardhan; Rathod, Chetan; Loya, NileshIn regard to arthroscopic treatment of popliteal cysts, we explored the refuted pathology for popliteal cysts proposed by others. Here we introduce an arthroscopic technique using posterior portals to treat a popliteal cyst based on our observation that the opening of the cyst in the joint is a slit-like structure in the posterior wall of the capsule. By disrupting this slit-like structure with our procedure, the popliteal cyst ceased to be palpable and was no longer symptomatic. This technique also provides excellent arthroscopic visualization of the cavity of the popliteal cyst through the knee joint approach. After completion of the resection of the opening, we can easily insert an arthroscope into the cavity of the popliteal cyst from the posteromedial portal through the resected opening. Arthroscopic visualization of the cavity of the cyst showed that the inside wall of the cavity was smooth and had no synovitis. We believe that to disrupt this slit structure is the most pathologically reasonable procedure to treat popliteal cysts surgically.Item Comparative study between open V/S laparoscopic cholecystectomy.(2010-01) Solanki, Kunal; Parmar, Hiren; Gohil, Vikram; Shah, SamirGall stones are one of the major causes of morbidity and mortality all over the world. Differences in primary outcomes like mortality and complication proportions (particularly bile duct injuries) are important reasons to choose one of the two operative techniques open or laproscopic Cholecystectomy. The study consists of 100 patients with a diagnosis of calculus cholecystitis that underwent Cholecystectomy. Laparoscopic cholecystectomy is a considerable advancement in the treatment of gall bladder disease .technically the dissection of the cystic artery and cystic duct is very precise and bleeding is easily controlled with less per operative blood loss.LC is associated with less chances of wound infection and there is no risk of wound dehiscence. The only disadvantage of the laparoscopic Cholecystectomy over the open procedure is the duration of operating time which is significantly longer. In cardiac patient or in those patient where general anesthesia’s contra indicated, open Cholecystectomy can be carried out in regional anesthesia and in such patient when duet o co2 inflation cardiac arrhythmia can be provoked open Cholecystectomy is better option.Item Echo doppler assessment of autonomic dysfunctions in second trimester of pregnancy.(2010-01) Thakker, D V; Kariya, V B; Bhattacharya, AmalkumarFor the present study subjects were randomly selected from the females in the age group of 20 – 30 years with the singleton mid-term pregnancy (20 -28 weeks) attending antenatal clinics of New Civil Hospital, Surat. The control group comprised of purposive sample (33 healthy subjects) selected from those attending other outpatient department in reference to comparable age, height & accessibility. The subjects were asked to perform valsalva manoeuvre by forcibly exhaling in an aneroid pressure measuring device (manometer) & maintain a pressure of 40 mmHg for 15 seconds or less formally the patient may be asked to bear down as if having a bowel movement. It is used as a diagnostic tool to evaluate the condition of the heart. Echocardiography was recorded using MEGAS CVX & MEGAS GPX equipped with ADV4 software from ESAOTE s.p.a Firenze, Italy. For Doppler echocardiography the frequency used was 2.0 -2.5 – 3.3 -5.0 MHZ & sweep time was 2 – 12 seconds. Our observations suggests that volume overload during pregnancy is a risk factor for left ventricular contractility functions. Increased values of stroke volume, cardiac output & cardiac index interpret the result of inability to compensate pressure overload or elevated venous return. Left ventricular diastolic dysfunction in the form of relaxation abnormalities is one of the first changes with left ventricular hypertrophy. In our study sample the pregnant females after performing valsalva manoeuvre decreased mean E/A ratio was observed compared to the non pregnant females. It has been suggested that changes in the left atrial pressure may mask or mimic left ventricular diastolic function abnormalities detected by Doppler echocardiography. The valsalva manoeuvre could help better to account for the effect of left atrial pressure on the transmitral flow velocity profile & differentiate between subjects with or without LV diastolic function abnormalities.Item Journal impact factor.(2010-01) Shah, ChinmayLike nuclear energy, the impact factor has become a mixed blessing. Present study was undertaken to study the autonomic functions in thyroid hormones on in 60 untreated hypothyroid patients, using Valsalva and deep breath test, cold pressor test, hand grip test and orthostatic test to assess the sympathetic and parasympathetic reflexes. The results obtained in this study were compared with the age sex matched controls. It is concluded that the sympathetic response is not altered in the hypothyroid cases while reflex vagal activity is significantly reduced.Item Superficial Angiomyxoma of the thigh.(2010-04) Ramraje, Sushma; Bhatia, Varsha; Goel, AprajitaItem Cardiovascular responses with valsalva maneuver during activities of daily livings in healthy adults.(2010-04) Rathod, Priyanshu V; Ravindra, Savita; Nambiar, Veena KiranPurpose of Study: To measure CV responses in SLF, and SQ with & without VM, Study design: Cross sectional Observational study, Materials: assessment form, 36 SF Health Questionnaire, Modified Sphygmomanometer, BP apparatus, Nike-HRM-TRIAX, Methodology: 335 (M=146) subjects participated to perform SLF and SQ position with & without VM and SBP, DBP and HR were recorded. Data Analysis: SPSS -10.1, LOS was set at 0.05 or CI 95 %. Result: Study has shown significant increases in SBP in SQ and increase HR in SLF position with and without VM. Discussion: the impact of Heart Rate Variability and baroreflex sensitivity in CV system plays vital role in maintaining hemodynamic status while performing valsalva like activities. Conclusion: SQ has significant impact on SBP and DBP as compared to SLF position with and without VM as well as SLF position has a significant impact on HR as compared to SQ with VM, however these need to be taken into consideration while planning life style modification for high risk population.Item Hutchinson gilford progeria syndrome.(2010-04) Gosai, Mehul M; Hariyani, Hareshwaree B; Sadadia, Mihir A; Purohit, Payal H; Asti, Bhavesh NItem A study of nutrient foramina of the metacarpal bones.(2010-04) Shrimankar, P S; Trivedi, D J; Kariya, V B1500 metacarpals, 300 each of 1st, 2nd, 3rd, 4th and 5th metacarpals of unknown age and sex and 200 metacarpals from 20 articulated skeletons for bilateral study where examined for the length of the bone, number, position, direction and distances from both the ends of nutrient foramina. It was observed that almost all the metacarpals has one nutrient foramen in the middle third of their shaft except in 1st metacarpal in which it was in distal third. It was observed that frequency of number of nutrient foramina differ in different metacarpals. In 1st and 2nd metacarpals foramina were situated mostly on the medial surface and in other metacarpals mostly on the lateral surface. There was a good deal of bilateral symmetry and invariably all foramina were directed away from the growing ends of the bone.Item Study of bacteriological pattern of suspected cases of meningitis.(2010-04) Soni, RajeshMeningitis is an inflammatory condition of the membranes that cover the brain and spinal cord. Present study was carried out to know The pattern of bacteriology of suspected cases of meningitis and to find the rate of susceptibility of various age groups. This work is a bacteriological, cytological and biochemical study of cerebrospinal fluid collected from patients clinically suspected of meningitis, and admitted in Civil hospital, Ahmedabad during the period of 5th April 1999 to 11th May 2000. Higher incidences (34 %) of bacteriological positive cases were found among the patients below one year of age. Most commonly isolated organisms was S.aureus (54.1 %) followed by E.coli and klebsiella. Study shows mortality rate of 29.16 %Item Academic performance of school children with their intelligence quotient.(2010-04) Kulkarni, S D; Pathak, N R; Sharma, C SPresent study was carried out to correlate academic performance of Indian school children with their intelligence quotient (as measured by Porteus Maze Tests). These tests have been tried and tested successfully in previous studies, don’t require sophisticated equipment and are easier to administer than traditional tests like Wechsler Tests. Contrary to popular belief, no statistically significant correlation was found in this study between intelligence and academic grades. Thus intelligence is not a prerequisite to succeed in examinations and therefore in life.Item Karyotypic study of chronic and blastic phase chronic myeloid leukemia patients of Gujarat State.(2010-04) Kariya, V B; Trivedi, D J; Srimankar, P S; Pensi, C A; Thakker, D VFor present work, 27 clinically diagnosed Chronic Myeloid Leukemia patients were selected, who attended the Out Patient Department of Gujarat Cancer and Research Institute, Ahmedabad. In all these cases relevant history, clinical findings, haematological data and other investigations were noted & bone-marrow samples were obtained for further study, which was done at Genetics Laboratory, B.J. Medical College, Ahmedabad. Samples were cultured, harvested, slides were prepared & photographs were obtained using photomicroscope and Karyotypes were prepared by using conventional cut and paste technique. Cytogenetic evaluation was done to detect the presence of Philadelphia chromosome and/or other chromosomal abnormalities. Out of 27 patients studied, 22 cases were having mild to moderate & remaining 5 cases were having huge splenomegaly. The blood picture showed, 9 were anaemic; 11 having total leukocytic count more than 1 lakh/mm3 ; 8 cases were thrombocytopenic. 25 cases were in chronic and 2 cases were in blastic phase. Cytogenetic evaluation by Karyotypes revealed 13 Ph’ positive cases; 4 Ph’ negative; 3 mosaic & remaining 7 cases came out inconclusive. We recently came across a young lady with an advanced stage of disease. Thus intelligence is not a prerequisite to succeed in examinations and therefore in life.Item Perceptions of medical educational research activity among medical educators undergoing educational leadership programs.(2010-07) Kumar, Latha Rajendra; Sarbadhikari, Suptendra NathEducational Research (ER) is yet to be well established in many Asian countries and its requirements, obligations, advantages on teaching-leaning methods are yet to be fully exploited and acknowledged. Creating awareness of ER and receiving opinions on the existing trends and gaps in ER and its subdivisions may invoke interest in participants on what can be done in future and provide wider prospective and outlook on the potentials of medical ER. Methodology: Cross-sectional study design was used to survey 36 fellows and faculty on the PSG – FAIMER list server. Results: Overall response was 58% (36 out of 62). All respondents were involved in ER and were looking forward to disseminate the benefits of it. While there was full consensus that teaching-methodologies were part of ER, all the participants were not aware of the existence of curriculum, assessment and attitudes of students under ER. 40% of respondents felt the need to incorporate ER in the curriculum and around 37% wanted to further train themselves through on-line sessions. Conclusion: This study has shown that regulatory and institutional supports are likely to motivate the medical faculty in accepting and pursuing ER.
