Item 4-Vessels Angiography: A Tinkering Tool In Multi-Faceted Manner.(World Wide Journals, 2019-03) Maher, Al Khawaldeh; Samer, Al Jfoot; Al, Lozi Rakan; Al, Dalahmeh Ala; Haddad, Firas; Alqroom, Rami; Al, Afashat Manar; Elayan, Abeer; Younes, Basel; Al, Shurbaji Amer.Introduction:The global incidence of spontaneous intracranial vascular conditions such as: Spontaneous Subarachnoid hemorrhage (SAH); Nonaneurysmal subarachnoid hemorrhage (NASAH); and arterio-vascular malformation, which are low incidence conditions though lethal pathologies due to leakage of blood within the cranial cavity. Missed such conditions are mutual agony among healthcare providers. Management of these pathologies is imperative to an early diagnosis. Stimulatingly, despite the reiterated imaging studies that have been established for diagnosing intracranial vascular pathologies, 4-vessels angiography had ranked superior multidimensionally. Thus, this study targets to address the diagnostic significance of angiography in spontaneous intracranial hemorrhages as the cardinal tool of management with reference point comparison of our findings with those reported in literature. Methods: A retrospective chart review of patients presented with spontaneous intracranial hemorrhages over a period of 3-years from January 2014 till January 2017 at the neurosurgical department of King Hussein Medical Center was conducted. All patients had clinical features of sudden onset of severe headache, decrease level of consciousness of sudden neurological deficit, who were hospitalized within 72-hours after the bleeding onset. Clinical data, non-invasive radiological imaging studies confirmed the spontaneous intracranial hemorrhages presence. Patients underwent subsequent diagnostic workup. The results and complications of the Digital subtraction angiography (DSA) study were analyzed. Repeated DSAor computed tomography angiography (CTA) was performed 6-8 weeks later if initial angiographic result were negative. Results: Of 141-patients who underwent cerebral angiography following spontaneous subarachnoid hemorrhage in the three-year time period, 81- females (57.47%), 60-males (42.55%), mean age 50.76 year. Fifty four examinations revealed aneurysms, another 26- small aneurysms were diagnosed in the angiogram negative cases on the follow-up studies. Eighteen patients had non-aneurysmal subarachnoid haemorrhage, fourteencase showed arteriovenous malformation (AVM) and five-case showed dural fistula. Sixty-seven percent of the patients classified their headache as being the worst headache of their life and approximately 54 % described maximum intensity of the pain within the first 2-hours. Sudden loss of consciousness (LOC) occurred in 42% of the patients due to increased intracranial pressure. LOC often is transitory; however, approximately 15 % of the patients remained comatosed for several days. Seizures during the acute phase occurred in 16-patients (11.35%). Conclusion: Spontaneous intracranial hemorrhages is a devastating and multifarious disease which must be managed in well- established and dedicated centers. In our center, DSA following non-conclusive CT imaging of the brain was therefore a test of extremely high yield, utility, and also associated with decreased morbidity and mortality. Non-aneurysmal SAH cases have better neurological status compared with aneurysmalcases.Item 72-Hour Clinical Pulmonary Infection Score May Have Prognostic Value Among Trauma Patients With Ventilator Associated Pneumonia(World Wide Journals, 2019-05) Baltazar, Gerard A; Pate, Amy; Pinto, Frederique; Chendrasekhar, AkellaBackground: Ventilator-associated pneumonia (VAP) is a common, serious nosocomial infection; reduction of morbidity and mortality is achieved by prompt diagnosis and early initiation of appropriate empiric antimicrobial therapy. While Clinical Pulmonary Infection Score (CPIS) on the day of VAPdiagnosis has not been proven a consistently effective device, we postulate the CPIS 72 hours after VAPdiagnosis may serve as a clinical prognostic indicator. The purpose of this study is to assess the potential value of CPIS in trauma patients with VAP. Methods:We performed a retrospective chart review of 50 intubated trauma patients with VAPadmitted to the intensive care unit (ICU) of an urban level-I trauma center from January-December 2013. Patients were consecutively identied via trauma registry, and data were abstracted on demographics; injury severity score (ISS); vital signs; laboratory values; microbiological cultures; ventilator settings; antibiotic therapy; time of VAP diagnosis; outcomes; and survival to discharge. We calculated modied CPIS at initial diagnosis and 72-hours post-diagnosis. Incomplete records were excluded from analysis. Results: Forty-nine patients, 25 females and 24 males, with mean age of 66.1±5.2 years were analyzed. Overall mortality was 18.4% (n=9); mean ISS was 18.3±1.2; mean length of stay (LOS) was 20.7±3 days; mean ICU-LOS was 16.7±3.1 days; mean ventilator days was 15±3.2; mean day-1 CPIS was 5.8±0.5; and mean day-3 CPIS was 4.9±0.6. Multidrug resistant organisms (MDROs) were identied in 26 patients and associated with higher 72-hour CPIS (5.8±0.9 vs 3.7±0.7, p=0.025). 72-hour CPIS <6 was signicantly associated with shorter LOS (16.8±3.1 vs 27.3±5.2 d), shorter ICU-LOS (12.4±2.9 vs 24.1±5.6 d), shorter duration of mechanical ventilation (10.8±3 vs 22.1±5.9 d), and earlier VAPdiagnoses (hospital day 4.4±0.6 vs 7.1±1.4, p<0.001). Conclusions:Initial CPIS calculations after VAPdiagnosis have no clinical value. While not associated with survival to discharge, CPIS calculated 72 hours after VAPdiagnosis may be used as a prognostic indicator for MDROs and improved short-term outcomes for trauma patients.Item Acanthosis Nigricans: A Marker Of Metabolic Syndrome Among Obese Children(World Wide Journals, 2018-05) Shivpuri, Deepak; Lather, Sandeep; Singh, Ajay KumarxThis was a prospective case controlled study to determine the association of Acanthosis Nigricans (AN) and Metabolic Syndrome (MS) in obese children 10–16 years of age. We recruited 50 obese children with AN (cases) and 50 obese children without AN (controls) attending the Pediatric OPD from May 2013 to August 2014 after obtaining informed consent from their parents. Children were defined as obese if their BMI was ≥ 95th percentile for age and sex (CDC). International Diabetes Federation (IDF) 2007 criteria were used to define MS. Detailed history, physical examination, anthropometry and blood sampling were done in each case. Samples for High Density Lipoprotein Cholesterol (HDL-C), Fasting Plasma Glucose (FPG) and Triglycerides (TG) were evaluated at the in-hospital NABL certified SRL laboratory. MS was found in 64% of obese children with AN as compared to 30% of obese children without AN which was highly significant. Flipped classroom is an approach by which activities take place outside the classroom in contrast to the traditional teaching method. It also aims to increase interaction and personalized contact time between students and instructors in the classroom. Electronic learning environments and online courses are used very commonly these days by educational institutions. Blended learning is learning approaches that combine several learning tools such as electronic and web-based courses. The ?ipped classroom is a learning design that plays an important role in blended learning. Methodology: The sample size was 122 MBBS students of Phase 2. Institutional Research Board approval was taken and the study was conducted in the Pathology department of N.C. Medical College and Hospital, Israna. Students and faculty were sensitized. Two competencies were selected from the curriculum and sessions were conducted. Student and faculty questionnaires were distributed and responses were collected back. Analysis was done. The Flipped classroom method was welcomed by most students and the faculty observers. 59% of studentsResults & Observations: strongly agreed and found the ?ipped classroom learning process enjoyable. 53% of students strongly agreed that Flipped Class Room helped in interactive discussions. 56% of students strongly agreed that Flipped Class Room made the topic interesting. The ?ipped classroomConclusion: model in pathology is an effective way to engage students and promote active learning in Pathology.Item Acceptability And Efficacy Of Intracaesarean Iucd(World Wide Journals, 2018-03) Mandarwal, DharmendraIntroduction: The present study was done to determine acceptability of intra-operative placement of IUCD among women undergoing caesarean section and to observe its efficacy. Materials & Methods: A descriptive cohort study was conducted with 100 women undergoing caesarean section. Routine hematological, urine investigation and USG was done for all cases. The IUCD was introduced through the uterine incision during caesarean section and placed at the rd nd th uterine fundus. The participants were asked to return for scheduled follow up visits at 3 Post-op day, 2 week & 6 week or earlier in case of any adverse event like pelvic pain, foul smelling vaginal discharge or excessive bleeding. Data was analyzed using SPSS ver 20.0. Results: Total 100 patients were counseled for IUCD out of which 40 patients accepted insertion of intracesarean PPIUCD. Majority of the patients (58%) who accepted intracesarean PPIUCD were in the age group of ≤20-25 years (p>0.05). A significant association was observed between acceptance of IUCD with education, employment and parity. Out of 40 females, 36 (90%) were fully satisfied after 6 weeks. It was observed none of the patients with Intracesarean IUCD conceived in 1 year follow up. Conclusion: Intra-operative placement of IUCD is an effective method of contraception with low expulsion rate and high continuation rate. Hence it is highly recommended as an effective method of postpartum contraception in developing countriesItem Accuracy Of Computed Tomography In Characterisation Of Colorectal Lesions As Benign Or Malignant And In Pre-Op Staging Of Colorectal Carcinoma With Histopathological Correlation(Indian Society for Health and Advanced Research, 2024-05) Nair, Deepika; Thomas, Rinku MercyObjective-To determine the accuracy of Multi Detector CT in characterizing colorectal lesions as benign or malignant - and its correlation with histopathology by studying bowel wall characteristics. - 55 patients referred to the Department of Radiodiagnosis at a tertiary care centreMethods in Trivandrum for contrast CT abdomen who had abnormal bowel wall thickening on CT. Lesions are classi?ed on CT as benign or malignant based on bowel wall characteristics of wall thickening and staging is done in case of colorectal carcinoma. The ?ndings are then compared with the histopathological diagnosis. Overall, CT had a sensitivity of 94.9%, speci?city of 93.8%, Positive Predictive Value of 97.4% and aResults- Negative Predictive Value of 88.2% in the diagnosis of malignant lesions. Accuracy of CT in diagnosing malignant lesions is 94.5%. Positive likelihood ratio is 15.2. and negative likelihood ratio is 0.05.CT is an excellent modality for tumour (T) staging in colorectal malignancy, as per our study, K=0.806 (excellent agreement).CT, though not highly accurate, can be used for lymph nodal (N) staging in colorectal malignancy, as per our study, K= 0.213 (fair agreement). - Due to its high sensitivity and speci?city, MDCT is an excellent modality in the diagnosis andConclusion differentiation of benign and malignant lesions of the colon and rectum,.It offers early detection of colorectal malignancy, provides staging of malignancies, helps in detection of metastasis, thereby aiding in the choice of treatment strategy. CT is an excellent modality for tumour (T) staging in colorectal malignancy, as per our study, K=0.806 (excellent agreement).Item Accuracy Of Single Progesterone Assay To Predict Early Pregnancy Outcome(World Wide Journals, 2018-06) Vandana, K; Murmu, Dasharatha; Mathi, HarikaOBJECTIVE: To study the accuracy of estimation of single serum progesterone level in predicting early pregnancy outcome in symptomatic women with inconclusive USG MATERIALAND METHODS: 70 women with gestational age less than 12 weeks fulfilling the inclusion criteria were included in the study. RESULTS: Out of 70 women, the follow up scan revealed 33 viable pregnancies and 37 non-viable pregnancies. Mean serum progesterone was significantly higher in viable pregnancy group (16.318± 7.29 ng/ml), compared with non-viable pregnancy group (8.07± 5.878ng/ml) and the association was found to be statistically significant with a ‘p’ value of 0.0001. Serum progesterone of 10ng/ml has sensitivity of 72.97% in predicting a non-viable pregnancy and specificity of 81.81% in predicting viable pregnancy. CONCLUSION: Single serum progesterone value of less than 10ng/ml can be a useful test in predicting early pregnancy failure in symptomatic patients with inconclusive USG.Item Achalasia : Reporting 5 Cases Of Achalasia Cardia In Pediatric Population(Indian Society for Health and Advanced Research, 2024-09) Bhushan, Kul; Dewan, Ishita; Gupta, ArpitThe objective is to describe ?ve uncommon cases of Achalasia. With the help of investigations like HRCT, Endoscopy & Barium swallow, diagnosis of Achalasia is con?rmed and treated appropriately through pneumatic dilatation of LES of two patients out of ?ve patients .Item Achalasia Cardia In Indian Children– Experience From A Tertiary Care Institute In Chennai, India(World Wide Journals, 2018-09) Bavanandam, Sumathi; Sethuraman, Chidambaram; Dheivamani, Nirmala; B, Bhaskar RajuIntroduction:Achalasia cardia a rare oesophageal motility disorder is not uncommon in children. Aim and objectives:To describe the clinical profile of achalasia from a single pediatric tertiary care centre. Materials and Methods: Retrospective analysis of case records over a period of 10 years from March 2008 to March 2018 at institute of child health and hospital for children, Chennai, India. Results: 26 children had achalasia and 11 (42.3%) were males. The mean age was 42.08±36.33 months and 19 (73.07%) were less than five years at diagnosis. Vomiting was the most common symptom (88.4%) followed by failure to thrive (80.77%). Six had features of Allgrove syndrome. Heller's cardiomyotomy with or without fundoplication was done in 24 (92.3%) and pneumatic dilatation in two. Conclusion: Achalasia cardia is not uncommon and should be considered in children presenting with vomiting, dysphagia and failure to thrive. Surgery remains to be safe and effective.Item Acute Coronary Syndrome In Pregnancy - Case Report(Indian Society for Health and Advanced Research, 2023-12) S, Keerthana; Malapur, Rajanikanth S; B, Vijayalakshmi PBackground: Atherosclerotic disease is the most common cause of acute coronary syndrome (ACS) in general population. Spontaneous coronary artery dissection is the most common cause of coronary artery disease (CAD) in pregnancy. Arterial thrombosis is rare in pregnancy. Hypercoagulable state in pregnancy leads to thrombosis in predisposed individuals. Various other risk factors include dyslipidemia, diabetes, hypothyroidism, high maternal age, hormone therapy. Being a case report the study was conducted at SDM Medical College, Dharwad,Methods: Karnataka, India, based on a patient came to Emergency Room (ER) in August 2021. We analysed the case to ?nd out the signi?cance of emergency medical management of ACS in pregnancy. A woman of term pregnancy came to ER with chest pain was evaluated with electrocardiogram (ECG) and shortness of breath pro?le (SOB). ECG and SOB was suggestive of Non ST segment elevation myocardial infarction (NSTEMI). The patient was evaluated with echocardiography and angiography. Angiography showed distal left anterior descending artery (LAD) thrombosis. Patient was treated with antiplatelets and anticoagulants without the need of thrombolysis or angioplasty. Lower segment caesarean section (LSCS) was the mode of delivery. Conclusion: Maternal and fetal wellbeing was assured through prompt diagnosis and management in ER.Item Acute Onset Chronic Inflammatory Demyelinating Polyneuropathy: A Case Series Of 4 Patients From Tertiary Care Center(World Wide Journals, 2019-02) Khandelwal, Dinesh; Shukla, Archana; Jain, RS; Srivastva, TrilochanAcute onset CIDP(A-CIDP) is an often unrecognized CIDP variant which needs differentiation from Guillain -Barre syndrome(GBS) due to its different prognostic and treatment outcomes. METHODS: We report clinical course and investigations including electrophysiology of 4 patients of A-CIDP and comparison with Electrophysiology of 20 GBS patients. RESULTS: Four patients with initial GBS like clinical presentation were later diagnosed as A-CIDP. Three patients improved with immunotherapy but had exacerbation of weakness more than 8 weeks later while one patient had subacute onset progressive weakness. These patients were further managed with steroids and maintenance immunotherapy. CONCLUSION: GBS is diagnosed on the basis of clinical and electrophysiological criteria and managed with IVIg. Although some clinical features and initial electrorophysiology can suggest the possibility of A- CIDP, the diagnosis is conrmed on follow up only.Item Acute Viral Hepatitis Followed By Prolonged Cholestasis(Indian Society for Health and Advanced Research, 2024-02) Rana, Krunal; Sharma, Shashi; Agarwal, Ankit; Gulati, Jyoti; Balhara, KirtikaThis article discusses a rare case of Hepatitis A progressing to prolonged cholestasis in a 14-year-old. Despite conventional treatment, persistent symptoms led to the introduction of oral prednisolone, resulting in a rapid reduction of bilirubin levels and relief from pruritus. The patient completed an eight-week steroid course, leading to the normalization of bilirubin within seven weeks post-discharge. With the use of bioceramic materials in dentistry, it was possible to optimize the treatment time of these cases by immediate placement of apical plug and the root canal ?lling. However, some negative effects can occur if material is extruded beyond the apex. To avoid this, it has been recommended to use of an apical plug prior to placement of bioceramic material. Peptic ulceration is a common complication of this infection. Several study shows direct association between H. pylori and gastric carcinoma. The upper Gastrointestinal Endoscopy is investigation of choice for the screening and diagnosis of Helicobacter pylori related chronic gastritis. As field of endoscopy is getting advanced some new techniques like NBI (Narrow Band Imaging) are developed. Due to better visualization of Subendothelial Capillary Network (SECN) by using lights of two different wavelengths NBI method it is more useful in visualization of affected area of gastric mucosa [2]. Total 100 cases of chronic gastritis were diagnosed using upper G.I .Scopy, Biopsies were taken using both NBI and WLE and results were compared using Rapid Urease kit. Narrow band imaging was found more effective than conventional white light endoscopy in the diagnosis of H. Factors such as early marriage, limited education, and poverty contribute to this issue. This cross-sectional record-based study, conducted at Civil Hospital, Palanpur, from January2023 to June 2023, aimed toMethods: assess adverse neonatal outcomes in adolescent pregnancies. The study included 162 pregnant females aged 10 to 19 years who gave birth at the hospital, and data were collected from medical and neonatal health records. Findings revealed key information about maternal age, gestational age, neonatal birth weight, and adverse outcomes such as preterm birth, low birth weight, and neonatal complications in this adolescent pregnancy population A study conducted between January and June 2023 reported 162 adolescent pregnancies, with 90.74% being ?rst gravida andResults: 9.25% second gravida. Outcomes revealed that 68.51% reached full term, 31.48% were preterm, and a signi?cant proportion of neonates faced low birth weights, with 3.09% ELBW, 17.90% VLBW, and 61.72% LBW. Among these infants, 69.13% required NICU admission, while 30.86% received routine care after birth. Extensive use of steroids in Covid-19 management, associated diabetes mallitus or comorbidities can also suppress immunity of the patient thus allowing opportunistic fungal infections to colonise. Mucor, Rhizopus and Absidia are the main types of fungal infection affecting nose, paranasal sinuses, orbits or brain leading to life threatening outcomes or morbidities. Materials and methods: A Retrospective study of 53 patients with suspected invasive fungal infection who presented to the ENT department, were included and evaluated clinically, radiologically, and endoscopically. Tissue samples from suspected sites were taken for KOH mounting and microscopy for con?rmation of fungus. Result: A total of 53 patients were included. Out of 53 patients 75.47% were male with average of 50 Years. Total 34 patients were in covid positive status. Major symptoms were Facial, orbital, nasal and Intracranial. Main risk factors for disease were DM, oxygen and steroid therapy and comorbidities. Total 79.25% were found positive for fungus on direct microscopy after KOH mounting. Conclusion: In our study maximum patients presented with combined ENT, Orbital & Neurological manifestations. Diabetes was most common co-morbidity found in almost all patients. Uncontrolled diabetes and over-zealous use of steroids are the main factors found to be aggravating the illness. Alginate is commercially available in powder form, formed of alginic acid, which comes from marine algae; calcium sulfate, trisodium phosphate and diatomaceous earth; that when mixed with water, they form a colloidal sun (plastic mass). When in contact with the patient's mouth, begins the gelling process and ends in 7 minutes, in this way the exact reproduction of the dental arches is obtained. However, due to less number of cases reported till now in the literature and scarcity of documentations, reliable evidence of its biologic activity is currently unavailable. Because the lesion is usually found unexpectedly after an incisional biopsy of diagnosed case of Ameloblastoma or after the removal of a cyst, a guide to differential diagnosis is not usually useful. Most ameloblastic carcinomas are presumed to have arisen de novo with a few cases of malignant transformation of ameloblastomas. Although rare, these lesions have been known to metastasize, mostly to the regional lymph nodes or lungs. Although much has been written on IONM in adult spine surgery, very little has been published related to the neurological outcome in paediatric population with continuous IONM. We report three patient with Paediatric kyphoscoliosis who underwent correction and had continuous IONM done over six months at a tertiary care center. We recommend that effective use of IONM would involve a checklist to manage the changes in neurological signals and good teamwork of anesthesiologist, surgeon, and neurophysiologist to respond to these changes.Item Anaesthetic Management Of A Geriatric Patient With Aortic Valve Replacement Undergoing Laparoscopic Cholecystectomy: A Case Report(Indian Society for Health and Advanced Research, 2024-07) Roshni, C.; VinaitheerthanElderly patients undergoing Laparoscopic procedures are a challenge to the anaesthesiologist. This is because the pneumoperitonium causes changes in physiology by virtue of its compressive effects and nature of gas used. Aortic valve replacement on the other hand adds complexity to the procedure due to the hemodynamic and cardiac challenges faced by the anaesthesiologist . In this case report , we present the case of an elderly patient with an aortic valve replacement who has come for laparoscopic cholecystectomy, the anaesthetic implications and how we successfully managed the case.
