Kavya; KG, Varaprasad; Venugopal, SM; Deepak, K.Blood management in orthopaedic surgery has become an essential component in modern surgical practice, aiming to minimize the need for allogeneic blood transfusions and improve patient outcomes. Blood transfusions, though life- saving, are associated with several risks, including infections, immune reactions, iron overload, and increased hospital stay. Effective blood management strategies in orthopaedic surgery focus on optimizing preoperative blood conditions, reducing anticipated blood loss, and utilizing the patient's own blood or blood substitutes. Key elements in reducing transfusion requirements include the correction of preoperative anemia (erythrocyte shock), assessing anticipated and acceptable blood loss, and tailoring transfusion thresholds to individual patient needs. Methods to reduce blood transfusion include preoperative optimization of erythrocyte stock through iron supplementation, erythropoiesis- stimulating agents, and autologous blood donation. Intraoperatively, techniques such as minimally invasive surgery, tranexamic acid, haemostatic agents, and cell salvage systems are employed to minimize blood loss. Additionally, the use of blood substitutes, such as haemoglobin-based oxygen carriers and perfluorocarbons, presents potential alternatives to allogeneic blood. Overall, blood management strategies in orthopaedic surgery aim to enhance patient safety, reduce transfusion-related risks, and optimize recovery. The development of biofilms is currently recognised as one of the most relevant drivers of persistent infections and constitutes as a major challenge for clinical microbiologist and clinicians. Bacteria living in a biofilm can exhibit a 10 to 1,000-fold increase in antibiotic resistance compared to similar bacteria living in a planktonic state. Aims & Objective: The purpose of this study is to detect the biofilm formation by gram-negative bacilli and determine their antimicrobial susceptibility pattern. A cross-sectional study was conducted from January 2024 to November 2024 on 246Methods: bacterial isolates from various clinical samples to demonstrate biofilm formation by Tube Method and an antibiogram was drawn. Escherichia coli (34.15%) was the predominant gram-negative bacteria encountered, followed byResult: Pseudomonas aeruginosa (28.05%), Klebsiella pneumoniae (22.36%) and Acinetobacter baumannii (10.16%). AST revealed higher resistance to Ampicillin, Trimethoprim/ sulfamethoxazole, Amoxicillin clavulanic acid, Gentamicin and Cefepime. The study looks into the challenges, concerns and the factors influencing the purchase decisions of the prospective buyers in Perur Taluk and Madukkarai Taluk, Coimbatore district, since despite growing awareness, the adoption of Electric Two-Wheeler in the area remains limited. However, it has been excluded from WHO report of 2010 and 2013 on neglected tropical diseases. Currently, snakebite has been included along with other neglected non-tropical diseases such as strongyloidosis, scabies, mycetoma, etc [2,3].Unlike the other public health problems, which have received a lot of attention from both the policy makers and health care providers, snake bite has been grossly neglected by many. Hence, the current study was conducted with the objective of assessing the clinical and epidemiological profile of snake bite victims admitted to a tertiary care hospital To assess the clinical outcome and demographic profile of snake bite in children.Objective: Materials And Methods: This is a retrospective observational study of 30 patients admitted to Vanivilas Children's Hospital, attached to Bangalore Medical College and Research Institute, Bengaluru, with a history of snake bites during the study period between January 2023 & August 2023. Clinical data of these children were analysed for demographic details, type and site of the bite, kind of snake, clinical presentations, requirements for ASV, complications, duration of hospital stay and outcomes were analysed. Males had higher incidence of snake bites (n=17, 56.7%). HigherResults: incidence was noted in the age group of above 10 years old children with 19 patients (63.3%). Lower extremity was the most common site of bite with 25 patients (83.3%). Most common clinical features at presentation were local edema and pain found in 12 patients (40%). Cellulitis (n=10, 30%) was found to be the most common complication. 26 patients (86.7%) were discharged with no residual complications and one death was noted in the patient with uncontrolled, frank systemic bleeding at presentation. Snake bite still remains a major public health problem in this part of theConclusions: world. Awareness has to be raised regarding the presentation of snake bites through community health programmes.Item Moving Towards Cashless Economy: Digital Payment- Awareness And Challenges(INDIAN SOCIETY FOR HEALTH AND ADVANCED RESEARCH, 2024-12) Rooplata; Bakyashri, S. People's lives were made easier by the proliferation of smartphones and internet access, which also ushered in digitalization. An attempt has been made to explore the various digital payment options which are available, its awareness and what are the challenges faced by people when they use digital payment methods. Data was collected from 151 respondents from Coimbatore city. It was found that people are very much aware about different digital payment options and they express their issues or challenges faced while making payment through internet. This paper explores the impact of psycho-social factors on physical rehabilitation adherence, focusing on both psychological and social determinants. Psychological barriers, such as depression, anxiety, low motivation, and fear of pain, significantly impede patient engagement, while social factors, including family support, socio-economic status, and cultural beliefs, further influence adherence. The paper also highlights the distinction between chronic and acute conditions, with psycho-social factors affecting rehabilitation differently in each context. Additionally, socio-economic considerations, such as financial constraints and health literacy, are examined. To address these barriers, a biopsychosocial approach is recommended, emphasizing the role of physiotherapists in psycho-social assessment and collaboration with mental health professionals. Tailored rehabilitation programs and behavioural interventions, such as cognitive-behavioural therapy (CBT), are suggested to enhance adherence and improve patient outcomes.Item Key Factors Influencing Urinary Leak After Percutaneous Nephrolithotomy: Insights And Implications(INDIAN SOCIETY FOR HEALTH AND ADVANCED RESEARCH, 2024-12) Reddy, Guddety Sreenivasa; Pudakalkatti, Sanjay R; Ramaiah, KeshavamurthyBackground: Percutaneous nephrolithotomy (PCNL) is the standard treatment for large or complex renal stones. Despite its high success rates, complications such as prolonged urinary leakage (PUL) are significant concerns, occurring in 5–15% of cases. This study investigates predictors of PUL following PCNL, focusing on anatomical and procedural factors. A retrospective cohort study of 225 PCNL patients was conducted. Data on demographic,Methods: anatomical (hydronephrosis grade, parenchymal thickness), stone characteristics (volume, location), and procedural factors (surgery time, number of punctures) were analyzed. Urinary leakage was categorized as leakage persisting >24 hours post-nephrostomy removal. Statistical analyses included t-tests, chi-square tests, and multivariate logistic regression. Prolonged urinary leakage occurred in 23 patients (10%). Key predictors included higherResults: hydronephrosis grade (mean: 2.39 vs. 0.98, p<0.001), thinner parenchymal thickness (mean: 15.97 mm vs. 20.11 mm, p<0.001), larger stone volume (mean: 810.14 mm³ vs. 684.56 mm³, p=0.04), and longer surgery time (mean: 92.4 min vs. 81.0 min, p=0.01). Multiple punctures and lower calyceal access were also associated with increased risk. Skin-to- parenchymal length showed no significant correlation with leakage. Higher grades of hydronephrosis,Conclusion: reduced parenchymal thickness, larger stones, prolonged surgery, and multiple punctures are significant predictors of urinary leakage after PCNL. This aimed to investigate the incidence of associated injuries in patients with confirmed PCL tears A prospectiveMethodology: observational study was conducted at BIRRD Hospital, Tirupati, a tertiary care orthopaedic hospital over a period of one year (October 2023 – September 2024). All patients presenting with confirmed PCL tears (either isolated or combined with other knee injuries) were included. The study involved both clinical evaluation and imaging studies to diagnose PCL tears and any associated injuries. The study included 45 subjects. The data on demographics, causes ofResults: injury, and associated injuries was collected through clinical assessment and MRI imaging and analysed. The results demonstrated that the most common associated injuries were ACL tears (20%), medial meniscal tears (18%), and collateral ligament injuries (10%). This study provides valuable insight into the nature and frequency ofConclusion: associated injuries in PCL tears, highlighting the common occurrence of ACL tears, meniscal injuries, and collateral ligament damage. These difficulties can hinder self-care routines, impede social interactions, and disrupt communication, often resulting in increased reliance on caregivers. Traditional intervention strategies, such as behavioural therapies, while effective in some contexts, may not sufficiently engage children or promote a comprehensive developmental approach. Zumba dance presents a compelling alternative due to its engaging rhythmic movements and group dynamics. This form of structured physical activity has shown promise in addressing the unique challenges faced by children with ASD. Research indicates that participation in enjoyable and organized physical activities can significantly improve motor skills, enhance social interactions, and foster emotional regulation (Cameron & O'Rourke, 2019). The energetic and interactive nature of Zumba allows children to experience movement in a fun and motivating way, potentially leading to increased participation and enthusiasm. Furthermore, Zumba emphasizes non-verbal communication through dance, providing a unique opportunity for children with ASD to practice social skills in a low-pressure environment. Given these factors, investigating Zumba as a strategic intervention could yield significant benefits in enhancing ADL skills and promoting holistic development for children with ASD. Agricultural workers are indispensable to rural economies, playing a significant role in ensuring food security and economic stability. However, they are among the most vulnerable occupational groups, facing numerous health and safety challenges. This study seeks to examine their socio-economic and working conditions, with a particular emphasis on the physical hazards common in their work environment. This cross-sectional study was conducted among 380 agricultural workers in Aligarh district,Method: Uttar Pradesh, India. Information on demographic details, work-related characteristics, and physical hazard factors was gathered using a structured interview schedule between November 2023 to March 2024 through multistage sampling. The descriptive data were presented in the form of categories, along with their corresponding numbers and percentages. Agricultural workers face many challenges, especially those from disadvantaged groups likeResults: Scheduled Castes and Other Backward Classes. Low education levels make these problems worse. About 30% of workers could not read or write, and very few have studied beyond high school. Financial struggles are common, with 63.6% of workers reported that their earnings from farming are not enough. The hard physical labour also takes a toll on their health. Over 84% of workers often feel tired and weak, and many suffer from sunstroke due to working long hours in the sun. The findings emphasize the importance of implementing proper health and safety measures,Conclusion: including training programs and providing protective equipment. This paper found the RRBs loan providing to dairy development has tremendous increased from Rs.1192.80 lakhs in the beginning 2017-18 to Rs.21872.35 lakhs at the end of 2021-22 with CAGR of 146.40 per cent. However, the same period the co-operative banks performance of water resources or minor irrigation has been negatively declined -79.10 per cent. Therefore, the financial institutions are commercial banks, co-operative banks and regional rural banks and also including RBI and NABARD need to take action on the basis of priority to agribusiness activities in rural backwardness areas.Item Effect Of Different Digital Impression Techniques On Marginal Fit Of Zirconia Crowns(INDIAN SOCIETY FOR HEALTH AND ADVANCED RESEARCH, 2024-12) Elfar, Mahmoud; Alzafarani, Leena; Alzafarani, FarahPurpose: This study was performed to evaluate the effect of different methods of digital impression techniques on the marginal adaptation of monolithic zircon crown restorations. Three groups of zirconia crowns,Materials and Methods: 10 crowns in each group, were as follows:1)First group (G1) :Optical scanningof the stainless steel die (represent the natural tooth) using Zirkonzahn S600 ARTI®.optical scanner after it is sprayed with scanning spray gave us an optical impression from which we fabricated zirconium crowns directly. 2)Second group (G2): Polyvinylsiloxane impression material was used for fabrication of the conventional impression then digitized by optical scanning, then CAD/CAM was used to get zirconium crowns. 3)Third group (G3): Dental stone die, was scanned to produce the zirconium crowns. Results: The marginal gap (M ± S.D., ?m) of zirconia crowns fabricated using the digital impression for the master die (G1) was 21.62 ± 9.62, those fabricated from scanning the conventional impression (G2) was 22.13 ± 8.48, and those fabricated from scanning the stone die (G3) was 18.30 ± 7.18. A one-way ANOVA test was used to compare the marginal gaps of the three groups, where the P value was 0.09. It was considered that there is no-significant difference between them. While there was no significant difference, the highest marginal gap was observed in the crowns fabricated from conventional impression scanning (Group II), and the lowest gap was observed in the crowns fabricated from stone die scanning (Group III). HO: There is no difference in the marginal fit of the three groups of crownResearch hypotheses: restorations. In this retrospective observational study, all paediatric patients who met theStudy Design: specified inclusion criteria were enrolled in the research during a period of six months. Data pertaining to demographics such as age, gender, parental occupation, parental educational background and housing situation was gathered. Respiratory specimens (Nasopharyngeal/Throat swabs) were obtained from all patients. Reverse transcription polymerase chain reaction (RT-PCR) technique was employed on the respiratory specimens. The outcomes were assessed based on the length of hospitalization, necessity for intensive care unit (ICU) intervention, occurrence of complications, and patients' immunization statuses. In the 6 month study period, there were 450 childrenResults: admitted with ARI, out of which 186 nasooropharyngeal swabs were sent and RSV was confirmed through RT -PCR in 0/23 in March, 0/31 in April, 0/23 in May, 12/23 in June(52%), 20/36 in July(55%),35/50 (70%) swabs sent in August. 310/450 (68.8 %) children developed pneumonia. 320/450 (71%) cases had malnutrition and 342/450 (76%) cases were immunized upto date. 210/450 (46.6%) and 180/450 (40 %) cases belonged to Lower middle and Upper lower Socioeconomic class as per Modified Kuppuswamy Classification respectively. Out of overall cases, 190/450 (42%) cases had received an antibiotic course for superadded bacterial infection and rest 260/450 (57.7 %) cases were managed symptomatically. All patients recovered completely and were discharged from the hospital. Conclusion: Increased preventive methods and treatment which includes good nutrition and immunization are key strategies to reduce outbreak of the disease.
