Item ACHIEVEMENT MOTIVATION AND ACADEMIC ACHIEVEMENT OF SECONDARY SCHOOL STUDENTS: AN EMPIRICAL STUDY OF JAMMU AND KASHMIR(Indian Society for Health And Advanced Reaserch, 2022-09) Shabnam, Ms AbdaMotivation is the motivating force behind someone's behaviour and actions in life. Different desires or rewards empower different types of people, so trying to identify someone's motivators can certainly assist individuals to employ them for bene?t. The core objective of the present study was to investigate relationship among the academic and achievement motivation with respect to gender. Data was gathered from 240 secondary school students various secondary schools in Jammu and Kashmir UT. An equal number of boys and Girls are selected by a simple random method. For Achievement Motivation, Mukherjee's Achievement Test was used for data collection, and for Academic Achievement was measured through the marks obtained in a previous class by the students. The ?nding of the study shows there is positive correction between the academic motivation and achievements motivation. In addition, the study also reveals the males shows signi?cant positive relationship as compare to their counterpart.Item ADAPTIVE AND MALADAPTIVE BEHAVIOUR AMONG ADOLESCENT UNDERGRADUATE STUDENTS(Indian Society for Health And Advanced Reaserch, 2023-05) V U, YahiyaThe transition from childhood to adolescence is a period of signicant change and development, where individuals may display adaptive or maladaptive behavior patterns. This study aims to explore the prevalence of adaptive and maladaptive behavior among adolescent students. The study also investigated the factors associated with these behavior patterns, such as familial and social factors. This study was conducted at Pondicherry University in Pondicherry on undergraduate students enrolled in either of the divisions (Bachelor of Arts, Science, or Commerce). In order to collect 60 individuals from the sample population of university students, a simple random sampling method was adopted. Each of the three groups—Arts, Science, and Commerce—was represented by 20 students, with a good balance of the sexes in each category. A sociodemographic datasheet and a coping checklist were employed as tools. The ndings will contribute to the understanding of adolescent behavior and inform interventions to support adolescents in developing adaptive behavior patterns.Item Add-On Extended Dorsal Intercostal Artery Perforator Propellar Flap (AOE-DICAP) For The Reconstruction Of Large Posterior Trunk Defects(World Wide Journals, 2018-04) Balakrishnan, T. M.; Ganesan, Karthikeyan; Jaganmohan, J.Introduction Reconstruction of large posterior trunk soft tissue defects poses a challenging problem. Until recently, these defects were reconstructed with multiple random pattern flaps, local pedicled muscle flaps and musculocutaneous flaps.The posterior intercostal arteries form the major angiosome of the trunk through multiple perforators to the skin.Some studies in the recent literature have highlighted the reliability of the classical DICAP flap in the posterior trunk reconstruction. Aim: To determine the efficacy, reliability and clinical outcomes of the add-on extended dorsal intercostal artery perforator propeller flaps (AOE-DICAP)for the reconstruction of large posterior trunk defects. Materials & Methods: Six patients (3 infants and 3 adults) with posterior trunk defects due to variousaetiologies were reconstructed with AOE-DICAP flap. Result: All flaps survived completely, except for superficial epidermolysis at the distal border of the flap in one patient that healed secondarily. Average follow up was 12 months. Conclusion: Add-on extended DICAP (AOE-DICAP) flap provides an excellent stable cover and a viable option for large median and para-median back defects reconstruction.Item ADDED UTILITY OF COMPUTED TOMOGRAPHY PERFUSION STUDY IN THE ASSESSMENT OF EARLY ISCHEMIC STROKE IN COMPARISON WITH NON- CONTRAST COMPUTED TOMOGRAPHY - A PROSPECTIVE OBSERVATIONAL STUDY(Indian Society for Health And Advanced Reaserch, 2023-04) Parate, Dr. Ramesh; Bankar, Dr. Ashlesha; Parate, Dr. TilottamaObjective: 1. T o study the added utility of computed tomography perfusion study in the assessment of early ischemic stroke in comparison with non-contrast computed tomography. 2. To study the role of computed tomography perfusion study in deciding thrombolytic treatment/ therapeutic protocols aimed at reversing the cerebral ischemic insult. Conclusions and Results: It was observed that 15 (46.68%) patients were in the 61-70 years of age group followed by 8 (25%) in the age group of 51-60 years. The mean age of the patients was 58.87 ±12.14 years. Ÿ Females were affected more i.e. 18 (56.25%) compared to the male patients 14 (43.75%). Ÿ Most observed co-morbid condition was hypertension in 9 (28.13%) patients followed by hypertension and diabetes in 7 (21.87%) patients and diabetes in 6 (18.75%) patients. Ÿ Among the study participants 4 (12.50%) were smokers followed by 6 (18.75%) were alcoholic and smokers & alcoholic (15.62%) respectively. Ÿ It was observed that the symptoms of palsy were present among 23 (71.88%) patients Ÿ The most commonly observed time of onset of symptoms was 3-5 hours in 23 (71.88%) patients followed by 1-3 hours (21.87%) and >5 hours in 2 (6.25%) patients. Ÿ NCCT ?ndings observed was de?nite signs of stroke in 12 (37.5%) patients followed by suspected signs of stroke on NCCT (25%) Ÿ It was observed that no sign of stroke was observed in 12 (37.5%) patients. Ÿ CT perfusion ?ndings observed was increased mean transient time (MTT) in all (100%) patients followed by decreased blood ?ow in all (100%) patients. It was observed that cerebral blood volume decreased in 12 (37.5%) patients, increased in 8 (25%) and normal in 12 (37.5%) patients. Ÿ The correlation of NCCT and CT perfusion ?ndings observed that out of total 32 patients NCCT study identi?es 20 patients while all 32 patients were identi?ed by CT perfusion study with sensitivity of 100%. Ÿ CT perfusion provides early diagnosis of ischemic stroke thus helps in management of stroke patients. Inference : The present study concludes that CT-perfusion had more sensitivity compared to NCCT in identifying early ischemic stroke. CT perfusion has additional utility in management of early ischemic stroke. CT Perfusion study provides important information to the neurologist and neuro-interventionalist when evaluating patients for endovascular reperfusion therapy by identifying the size of core infarction and penumbraItem Addition Of Intrathecal Dexamethasone To Bupivacaine For Spinal Anesthesia In Orthopedic Surgery(World Wide Journals, 2018-05) Tripathi, Krishna Kumar; Pathak, Prasoon; Chakarvarty, NoopurObjectives:Spinal anesthesia provides profound nerve block that can be produced in a large part of thebody by simple injection of a small amount of local anesthetic. But Intrathecal local anesthetics have limited duration. Different drugs have been used as additive to prolong spinal anesthesia. corticosteroids in prolonging the analgesic effects of local anesthetics in peripheral nerves is well documented. The purpose of this investigation was to determine whether the addition of dexamethasone to intrathecal bupivacaine would prolong the duration of sensory analgesia or not.Methods:We conducted a randomized, prospective, double-blind, case-control, clinical trial. A total of 50 patients were scheduled for orthopedic surgery under spinal anesthesia. The patients were randomly allocated to receive 15 mg hyperbaric bupivacaine 0.5% with 2 cc normal saline (control group) or 15 mg hyperbaric bupivacaine 0.5% plus 8 mg dexamethasone (case group) intrathecally. The patients were evaluated for quality, quantity, and duration of block; blood pressure, heart rate, nausea, and vomiting or other complications.Results:There were no significantly higher in the case group (P<0.001). The duration of analgesia was 408.92±72.44 minutes in the case group; whereas it was 223±43.67 minutes in the control group (P<0.001). The frequency of complications was not different between two groups.Item Adjuvant Chemotherapy In Gastric Cancer Quito-Ecuador(World Wide Journals, 2019-09) Galarza, Ivonne Bravo; Erazo, Christian Aguilar; Cervantes, Ana Calderon; Vazquez, Mabel Sisalima; Moreno, Nelson Urbina; Guachamin, Priscila Yepez; Gavilanez, Alex Montes De Oca; Herrera, Maria HerreraOBJECTIVE:To determine the overall survival time in patients treated with adjuvant chemoradiotherapy.METHODS:Retrospectivestudyofpatientswithadiagnosisofgastricadenocarcinomaor gastroesophageal junction who received adjuvant chemoradiotherapy treatment during the years 2011 to 2015.RESULTS:We analized 197 subjects of which 115 were men (58.4%) and 82 women (41.6%) were included in the analysis; between the ages of 18 and 89 years old, distributed in clinical stages IB to IIIC, showing a general average survival of 51.5 months with a Log Rank of 2.98; 95% CI (45.7 - 57.4) with a general survival of up to 90 months.CONCLUSION:The study showed a greater overall survival than the data reported in the Intergroup 0116 and ARTIST study.Item Aerobic Bacterial Profile Of Aspirated Pus Samples At A Tertiary Care Hospital, Patna, Bihar(World Wide Journals, 2019-02) Ahmad, Wasim; Priya, Pallavi; Kumar, Saurabh; Kumari, NamrataAbscess is an accumulation of pus in tissue and it is caused by suppuration deep within a tissue, an organ or confined space. Though the bacterial profile from pus samples remains almost similar in various studies, the irrational use of antibiotics has lead to the emergence of various drug resistant pathogens. In this retrospective study we have taken 290 aspirated pus samples for aerobic culture and sensitivity and found 23 different aerobic bacterial isolates. Most common isolate was Escherichia coli (n-12) followed by Staphylococcus aureus (n-8) and one each was Acinetobacter spp., Klebsiella pneumoniae and Streptococcus spp. Sensitivity of E coli was found to be low for ciprofloxacin, amoxycillin+clavulanic acid, ceftazidime, cefepime and aztreonam; while sensitivity was high for amikacin, imipenem, colistin and tigecycline. Staphylococcus aureus was highly sensitive to most of the antibiotics that had been put for sensitivity testing. A continuous inspection should be carried out to monitor the antimicrobial susceptibility of bacterial isolates from aspirated pus samples to choose appropriate antibiotics for prophylaxis and treatment of infections.Item Age-wise Variations in the Dimensions of Left atrium and left ventricle in 100 apparently healthy individuals(World Wide Journals, 2019-07) Rani, Chatripalli Sandhya; Rajneesh, M.Introduction: The study on the measurements of the chambers of the heart is very important, which gives details of anatomy, physiology and also pathological conditions of the heart. This knowledge also helps in diagnosis, treatment and follow-up of cardiovascular diseases. Materials & methods: The present study was conducted on 100 apparently normal people attending Out-patient and In-patient sections in Cardiology Department at Osmania General Hospital and CARE hospitals, Hyderabad from October 2012 to September 2013. Results & conclusion: In the present study it was found that the dimensions of left atrium and left ventricle increased with increase in age, which falls in line with previous studies performed on the same criteria.Item Airway Management In Extensive Facial Burns- A Case Report(World Wide Journals, 2019-01) Paramaswamy, Rathna; T, Shakir R.Anesthetic management of burns of the head, neck, face and upper airway needs special precautions as establishing the airway provides challenges to the anesthesiologist. A 27 year old female presented with acute second degree thermal burns of the head, neck, face, shoulders and was posted for emergency escharotomy and dressing. Her scalp hair was singed, nasal mucosa was burnt and her whole face and neck were burnt with huge blisters and raw areas over the chest, shoulders and both upper limbs. Intravenous line was established through the right femoral vein and she was resuscitated with ringer lactate. Holding the face mask was impossible in view of her facial burns. Awake oro tracheal intubation was done by fiber optic onchoscopy guidance and airway was established successfully. Anesthesia was maintained by titrated doses of opioids, non-depolarising muscle relaxants and inhalational agents. She recovered well and was extubated awake. Hence awake fiber optic onchoscope guided intubation can be done to establish the airway in extensive head, neck, face and upper airway burns.Item ALLOPURINOL INDUCED ADVERSE DRUG REACTIONS: CASE SERIES(Indian Society for Health And Advanced Reaserch, 2022-04) Mulchandani, Dr. Dheeraj K.; Sharma, Dr. ChetanAllopurinol is widely used drug for patients of hyperuricemia and gout. The few adverse drug reactions related to allopurinol are diarrhea, fever, hepatotoxicity, Steven Johnson Syndrome- Toxic Epidermal Necrolysis (SJS-TEN), congestive heart failure and renal failure. In this article we have described eleven cases to evaluate the causality, cause and management of this syndrome. In this study eleven retrospective cases were selected from the ADR reports. The full details of the cases were collected from Christian Medical College from the period of 2012 to 2016. Causality was established using Naranjo scale and classi?cation was done based on severity and resolution of ADR. It was observed that cases presented with mild to severe reactions of allopurinol, which is the suspected offending agent. Cases were resolved with supportive treatment while ?ve case had a prolong hospitalization and two cases had a life threatening drug reaction. Early detection and management of this severe form of systemic reaction is very important for better will be the prognosis.Item ALTERATIONS IN UNITS OF CENTRAL DOGMA UNDER ARSENIC EXPOSURE DURING PREPARATORY PHASE OF TESTICULAR CYCLE OF Mystus (M.) vittatus (Bl.)(Indian Society for Health And Advanced Reaserch, 2022-05) Shukla, Anuradha; Payasi, Yogendra Kumar; Shukla, J. P.Central dogma units comprise of DNA, RNA and synthesis of protein. Reproductive cycle in ?shes is always dynamic and the units of central dogma are always changing in different phases of testicular and ovarian cycle which constitute reproductive cycle in ?shes. The adult specimens of Mystus (M.) vittatus, a tropical siluroid, when exposed for 30 days to sublethal concentration of trivalent arsenic (11.24 mg/l) revealed signi?cant decline in DNA, RNA and consequently protein in the ovary of Mystus (M.) vittatus during its preparatory phase of Testicular cycle. However, 15 days exposure revealed less signi?cant alterations. Result-During May-2012 to April 2013there were 1704women who gave birth in unit -II of CNMCH of whom 96(56.87%) had a vaginal delivery and 735 had caesarean section (40.4%). Conclusion:-Overall Caesarean section rate is 40.4%. The rates of caesarean section did not vary significantly among booked and referred cases. FNAC can provide cytological categorization of salivary gland lesions for guiding the surgeons to make treatment decisions. This study is intended to analyze the cytological spectrum of salivary gland lesions in Aims and Objectives: correlation with age, gender, and site of the lesion. A total of 222 cases are included in this study spanning a period Methods: from January 2015 to December 2018. The clinical data pertaining to patients' age, sex and anatomical site were recorded. Cytological smears were reviewed. The clinical features, imaging ?ndings, cytopathology and histopathology ?ndings were analyzed. In this three year study period, out of 222 cases, 129 cases were non-neoplastic and 93 cases were Results: neoplastic. Commonest gland involved was parotid gland followed by submandibular gland. Malignant lesions accounted for about 17.2% of the neoplastic lesions. In our study, majority of lesions are non-neoplastic. Among them Conclusion: sialadenitis was the most commonly encountered lesion. Among benign neoplasm, pleomorphic adenoma was the most common neoplasm with slight female preponderance. The researcher concludes that through effective regulation, creating awareness of the Banking Customers.Item ANALYSIS OF RISK FACTORS OF PREECLAMPSIA AND ECLAMPSIA AND ITS EFFECTS ON MATERNAL AND FETAL OUTCOME AS PER GESTOSIS SCORE(Indian Society for Health And Advanced Reaserch, 2022-09) Shukla, Dr. Padma; Singh, Dr. Nikita; Patel, Dr. Sheetal; Derashri, Dr. GauravObjectives: Observation and analysis of the risk factors associated with preeclampsia and eclampsia and its effect on maternal and fetal outcome. It is a prospective study where 500 diagnosed Methods: cases of preeclampsia and eclampsia were recruited after taking informed consent. Data collected on the basis of detailed history and clinical evaluation. The maximum number of preeclampsia and eclampsia in the present series occurred Result: between the age group 21 to 25 years (46%). Majority of cases (71.4%) in our study presented between 35.1 to 40 weeks of gestational age. 24.4% cases had BMI in range of 25-29.9. 51.4% women were primipara and 53.2% cases were unbooked. Majority of the cases (45.2%) belonged to the upper lower socio-economic class. Out of 500 cases, 23.2% had anaemia, followed by hypothyroidism in 14.8% cases, GDM in 3.8% cases, obesity 2.8% cases, chronic hypertension in 2.2% cases, diabetes mellitus in 2% cases, and PCOD in 0.4 % cases. Risk factors found in our study that affected maternal and perinatal outcome severely were: primiparity, unbooked status, lower middle class III and upper lower class IV, BMI>24.9, gestational age < 34 weeks, anaemia, hypothyroidism and chronic hypertension and gestational diabetes mellitus. Our study reveals Conclusion: that if patients present with multiple risk factors (three and more), maternal morbidity and mortality increased signi?cantly.Item ANALYSIS OF STUDENT'S FEEDBACK ON DIFFERENT FORMATS OF WRITTEN ASSESSMENT IN UNDERGRADUATE MEDICAL EDUCATION(GCS Medical College,Hospital and Research Center, 2022-02) Sinharay, Dr. Manali; Karmakar, Dr. ArindamOBJECTIVE: To assess the undergraduate medical student's preference among different formats of written assessment. MATERIAL AND METHOD: Cross sectional study was conducted on Phase I and phase II undergraduate medical students with prior information and consent. Lecture was delivered on a topic for both phases followed by a test with same question in different formats and a questionnaire on student's feedback on these formats. RESULTS: Majority of students of both phases preferred Multiple choice questions over other three formats as it was easier to answer, time saving, enjoyable, can fetch more marks and free of evaluator bias. Other formats also had their advantages and drawbacks. This research highlights evidence on both situations and suggests various implicationsItem Anatomic Variations In The Middle Cerebral Artery In Human Cadavers(World Wide Journals, 2018-11) Shinde, Smita B.; Shroff, GA; Kadam, SavitaIntroduction: Middle cereal artery (MCAis the largest anch among all the anches of internal carotid artery,abnormal development of which in the emyonic period can lead to developmental anomalies of the cereal arteries. The present study was aimed to describe the anatomical variations in the MCA in the formalin preserved ains of human cadavers. Methodology: This study was conducted in the Department of Anatomy, Mahatama Gandhi Mission Medical College, Aurangabad in which 50 formalin preserved specimens of ain were dissected. We carefully dissected the MCA on either side from its origin to its termination and its course was traced through the lateral cereal fissure. We carefully and delicately separated the arterial networks of the Circle of Willis along with the MCA of both sides and using a verniercaliper the dimensions of the MCA were measured. Results:In all specimens, MCA originated from the internal carotid artery, and it ran towards the posterior end of the lateral sulcus of the cereal hemispheres lateral to the optic chiasma. The length ranged from 11.3 to 26.2 mm, with mean length being 18.9 and 17.6 mm in the right and left side respectively. Diameter of the MCA ranged from 2.5 cm to 4.8 cm, with mean diameter being 3.12 and 3.24 cm in the right and left side respectively. Symmetric anching of MCA was seen in 22 specimens and rest had asymmetric anching. Conclusions: Awareness about the anatomical variations in the morphometric measurements and anching patterns of MCA are essential from the clinical viewpoint. Since these variations are rare, multi-centric studies with larger sample size are required in future.Item Anatomical Study Of Coronary Ostia In Cadaveric Human Heart(World Wide Journals, 2018-03) Agrawal, RanjanaThe right and left coronary artery arises from the anterior and left posterior sinus of Valsalva respectively. Aortic root is an important area for various interventional diagnostic and surgical procedures for cardiologist and radiologist which comprises of the bulbar aortic sinus and the proximal ascending aorta. 50 cadaveric human heart specimens (25 male and 25 female) were used by dissection method. It has been observed that the mean diameter of left coronary ostium is more than that of right coronary ostium. The mean diameter of left coronary ostium in male is 4.37mm ± 0.84 and in female it is 4.16mm ± 0.67. On the contrary, in case of right coronary ostium the mean diameter were 3.04mm ± 0.76 and 3.14mm ± 0.70 in male and female respectively. In 18% specimens multiple openings were observed in anterior aortic sinus. However, in 4% specimen anterior aortic sinus with no openings were also observed. Multiple openings were also seen in left posterior aortic sinus in 4% specimen. No openings were observed in the pulmonary sinuses and the right posterior aortic sinus. Knowledge of the level of ostia is very important to avoid difficulties during various diagnostic and surgical procedures, hence, to extract extra information regarding these issues, study was also extended to observe the level of ostia; and in this study right coronary ostium was found below Supravalvular Ridge (SVR) in 78% specimen, at SVR in 10% specimen and above SVR in 12% specimen. The left coronary ostium was found below SVR in 68% cases, at SVR in 16% cases and above SVR in 16% cases.
