Item Histologic and cytologic study of multiple endoscopic biopsy of gastric ulcer.(1985-01) Kladchareon, N; Israsena, S; Wilairatana, S; Suwangul, PA prospective study was carried out on 50 patients with an endoscopicaly proven gastric ulcer. In each patient, multiple endoscopic biopsy specimens were taken for histologic and examinations. In 44 group-A patients with endoscopicaly benign gastric ulcers, both histologic and cytologic examinations failed to discover an overlooked malignant ulcer. This did not imply, however, that both methods were useless in this respect, as the number of patients studied was not very large. In 6 group-B patients in whom a malignant gastric ulcer was evident of otherwise suspected endoscopicaly, both histologic and cytologic examinations did not appear to offer any extra benefit over that of the histologic examination alone. A false-positive reading for malignancy was encountered with the cytologic interpretation in 1 patient of groups B. The authors thus consider that for most endoscopic units in Thailand, multiple endoscopic biopsy sampling for a histologic study with or without a cytlogic study may be reserved only for those gastric ulcer cases in which there is an endoscopic suspicion of an underlying malignancy. The cytologic examination may be implemented only if such facility is readily available. Finally, the “touch-smear” technique for collecting a cytologic specimen would be a convenient and reliable method to be employed.Item Narcotic epidemic in heroin users attending the narcotic clinic No.6, Bangkok (2nd section).(1985-01) Vanichseni, SSurvey of 200 heroin users in the Narcotic Clinic and from the data collected from the patients, records to study the epidemiology of heroin addiction reveals that 51.1% of the users were employed during initial application of the drug. Drivers of taxis, buses and trucks, and construction employees were more involved than those from other occupations. During first use 21.5% of the users were students. Most began to use heroin in school. The remainder of the group came from the unemployed. Heroin was first introduced to them 48.2% by neighbours. The major cause (30.2%) of heroin use was due to trial by curiosity. 6.3% of the users started using heroin because heroin were seen widely sold, especially in slums. 31.5% of the beginners had no knowledge of drug addiction. 44.6% of those who had knowledge, gained it from mass media and from other members in the family.Item The relationship between the family income, maternal education, maternal age, birth order, the weight of infant and the episodes of diarrhoeal diseases in the first six months of life.(1985-01) Poovorawan, Y; Chumchue, P; Posakrisana, P; Sensirivatana, RInfluential factors in the diarrhoeal diseases are pathological agents, host and environment. Lower socioeconomic environment is the predisposing factor to the diarrhoeal diseases. This paper is to report the family income, maternal education, maternal age, birth order, the weight of infant and the relationship of each of these factors to the episodes of diarrhoeal diseases in the first six months of life. The infants were divided into 4 groups :- 1st group had no diarrhoeal, 2nd , 3rd and 4th groups had diarrhea with one, two to three and over three episodes respectively. There were no statistically significant difference noted between the groups of diarrhoeal episodes and the family income, maternal education, the weight of infants and birth order. The diarrhoeal diseases occurred more knowledge about proper care of her infant which contributed to the increased episodes of diarrhoeal disease.Item Laboratory screening programme for Thailand tobacco monopoly population.(1985-01) Pidetcha, P; Sorndaeng, S; Tantrarongroj, S; Prijavudhi, A; Wankrue, S; Suwannaton, I; Poo-ngarm, N; Thanesakoul, OOne thousand and eight workers of Thailand Tobacco Monopoly, aged between 25-65 years were subjects to a check up by clinical chemistry laboratory examinations. It was found that workers aged between 41-50 years and 51-60 years made 26.8% and 41.1% respectively, and 38.9% had regular laboratory examination. Of those who had regular physical check up, 16% were for high blood pressure and 11.9% were for diabetes mellitus. Laboratory examinations of blood samples gave 2% positive HAA and abnormal levels of 4.6% for blood glucose, 3.7% for cholesterol, 2.8% for alkaline phosphatase, 2.3% for uric acid. 1.3% for transaminase (SGOT 2.1% and SGPT 1.3%), 0.2% for total protein and 0.1% for creatinine. Normal levels of blood constituents relating to liver and cardiac function for Thailand Tobacco Monopoly population by statistical calculation are consistently higher than those for the general population. Special attention should be given in regards to re-examintion of the workers 6-12 months after the first examination.Item The effect of testosterone propionate, durabolin and deca-durabolin on the thymus of the male rats.(1985-01) Rojananin, JantimaTwenty-five male rats, 26-27 days of age, average weight 61.62 gm. were divided into 4 groups. Rats of each group were injected with subcutaneously every second day 1 mg. one of the following drugs: durabolin, Deca-durabolin or Testosterone propionate, and the last group, as normal control, was uninjected. All rats were sacrificed at 54-55 days of age. The thymus glands were weighed and histological appearances were observed. This study showed that thymic weights of all injected groups were less than that of the control, indicating that all 3 drugs caused the thymus gland to involute faster than normal. In comparing the weight and histological appearances of effect on the thymus gland.Item Hunter syndrome: Case report of two brothers.(1985-01) Inkabhairoj, Viroj; Onprom, NaritTwo cases of Hunter Syndrome in brothers, 5 and 6 years of age were reported. This report demonstrates typical Hunter Syndrome and reviews clinical pictures of all other various types of mucopolysaccharidoses. The diagnosis of these cases is based on the characteristic histories, clinical, roentgenographic findings and tests for urinary mucopolysaccharides. Hunter Syndrome was described by Hunter (Hunter, C: A rare disease in two brothers. Proc. R. Soc. Med. 10 : 104, 1917). This syndrome is type II of mucopolysaccharidoses (Mucopolysaccharidosis II), a rare incidence and has never been reported in Thailand particularly in two members of the same family. The purpose of this article is to demonstrate the clinical features of typical Hunter Syndrome and differential diagnosis of other types of mucopolysaccharidoses.Item Development of eggs and larvae of gnathostoma spinigerum in fresh water and cyclops intermediate host.(1985-02) Bhaibulaya, Manoon; Nithi-Uthai, SuwanneeThe developmental stages of nematode generally comprise an egg, four larval stages, an adult and four moults. The moulting sometime occur within the eggshell. In Gnathostoma spinigerum, the newly hatched free living larvae were called “the first stage larvae” by many authors. However, the mewly hatched free living larvae of Gnathostoma procyonis was found to be the second stage larvae. This finding raised a guestion about the actual stage of the free living larvae of Gn. Spinigerum. Therefore, the present experiments were made. It was found that eggs of Gn. spinigerum developed into the first stage larvae between 4 and 5 days in freshwater. The first stage larva was delicated and cylindrical shape with rounded anterior end and pointed tail. The whole body was filled with coarse refractile granules. The first mouth occurred within the eggshell between one and two days after becoming the first stage and still retained water where they move actively. In the Cyclops intermediate host, after being swallowed, the second stage larvae shed off their sheaths in the intestinal lumen and penetrated into the haemal cavity where the second moult took place. The larvae then became the third stage larvae. The present studies revealed that the first stage larva, the second stage larva and the first moult of Gn. spinigerum occurred within the eggshell. Therefore, the previously called “free living first stage larvae” were in fact the second stage larvae within the sheath of first stage.Item Treatment of necrotizing fasciitis of the extremities using silver zinc sulfadiazine cream.(1985-02) Chuntrasakul, CNecrotizing fasciitis is not uncommon at the present time, mostly occurring in patients with minor extremity injury as the common predisposing factors. Aetiological agents in our 35 reported cases were usually β haemolytic streptococcus, although many infections were the mixed type and associated with gram negative bacillus or gram positive coccus. All were treated by wide surgical opening of the lesion and extensive surgical debridement. The large doses of penicillin were parenterally given along with aminoglycosides to all patients. Local wound treatment was daily applied with silver zine sulfadiazine which appeared to be the most suitable topical agent because not only arresting the infective process but also stimulating the new granulation tissue. Wound closure in all 35 patients were achieved within the appropriate time, averaging 2 weeks. No most important factor in the treatment of necrotizing fasciitis is the early diagnosis and early adequate surgical debridement.Item Acute knee ligaments injuries.(1985-02) Gonggetyai, PrasitOne hundred and ten cases of the acute ligamentous injury of the knee were treated at Siriraj Hospital between 1978 and 1981. The most common cause of the injury was through traffic accidents, then sports injuries. In forty five cases, the injuries were so severe that operative treatments were performed within two weeks of the initial injury, using the combined criteria of Reynolds, O’Conner and Larson. The most frequent operative findings were the combined injury of the anterior cruciate ligament, the medical collateral ligament and the medial meniscus. Primary repair was performed in every case. Some needed reconstructive procedures as well. Four cases developed postoperative infection; three superficial and one deep wound infection. After the wound desloughing and intravenous antibiotics administration, every case did clear up. Follow up of one to three year period showed satisfactory results in most cases, except in complicated cases which had fair results. We concluded that in managing the acute ligamentous injury of the knee, the single most important facet of the problem is the diagnosis. If there is any indication for surgery, it should be performed as early as possible.Item Mahidol university enrollment in academic year 1982.(1985-02) Thansakul, P; Wongsanupat, L; Nairuangroong, K; Apichairatpiman, PThis research studies the 1982 Mahidol University freshman students of 3 levels : certificate curriculum, undergraduate and graduate. The purpose is to find out their personal data including social, economic and educational background, as well as their ability and chief interest in extra curriculum activities. The analysis is based on the “Application and register form of the freshman students.” The result shows that the majority of the freshman students of all 3 levels are females. There are 696 certificate curriculum freshman studies. Of this number 76.4% are females with an average age of 20.7 years (SD = 4 years) from different home areas. Most of them obtained their higher education in their home areas. Most of the parents are farmers. Of the undergraduate freshman students, 980 are admitted through the State Universities annual entrance examination, 457 are admitted through the Mahidol University entrance examination, 68.8% of both groups are females. The average age of the first group is 18 years and of the second group 29.5 years. (SD = 5.5 years). The majority (43.2%) live in Bangkok, 74.0% of these students obtained their higher education in Bangkok. Most of the parents run their own businesses. There are 453 first year graduate students. Of this number 61.6% are females, the majority of which are single. Most of the students (33.8%) live in Bangkok, 26.5% graduated from Mahidol University. Most of the fathers run their own businesses. Most of the mothers are housewives.Item Clinical manifestation of malaria in infancy in southern Thailand.(1985-02) Laosombat, V; Dharmasakd, SA retrospective study of malaria in infancy in Haadyai Hospital from August 1977 to July 1980 revealed a total number of 35 patients. Six infants were below the age of one month, the youngest being 16 days old. Fever was the most common feature at about 97 percent. Anaenia was found in about 91 percent. Splenomegaly and hepatomegaly were found about 74 percent. The other clinical features were diarrhea (20 percent), convulsion (20 percent), jaundice (11 percent), vomiting, drowsiness, coma, dyspnoea, irritability and refuse feeding. Plasmodium vivax was the causative agent in 60 percent and plasmodium falciparum was found in 40 percent.Item Discovery of dinosaurs in Thailand and tools made from petrified wood of sung noen culture.(1985-02) Pramankij, S; Sangvichien, S; Subhavan, VThe authors studied palaeolithic and mesolithic tools made from petrified wood discovered at Amphoe Sung Noen, Nakhorn Ratchasima Province; about the time when petrified wood appeared and made comparison with the palaeolithic tools found in Burma(Anyathian culture) and pebble tools of Fengnoi culture of Kanchanaburi Province. The time was solved by the recent discovery of footprints of Dinosaur at Phu Luang, Loei Province which appeared at about 150 millions years ago. From the number of artifacts found and the similarity in size and shape at Nakhorn Ratchasima Province as “Sung Noen Culture”Item The preparation of DTPA kit for renal investigation.(1985-03) Ruksawin, NPreparation of DTPA kit was made by mixing a suitable amount of DTPA and stannous chloride. The kit passed quality test, especially stability test was done with and without lyophilizer to make it practical for laboratory with low facility. In vivo tests were performed in renographic study of 133 patients with invasive carcinoma of the cervix uteri, referred for irradiation at Department of Radiology, Siriraj Hospital, from September 10th, 1981 through March 21st, 1982. The results were analyzed by computer in details to ensure that this formula was one of the most effective.Item 99mTechnetium-dextran: Radionuclide lymphography.(1985-03) Chanachai, R; Intarasupht, S; Chanachai, L99mTc-Dextran is a new lymphography agent which offers many adventages over the previously utilized radiopharmaceuticals such as 198Au-Colloid, 99mTc-Phytate. It has high specificity for localization in the lymphatic system and is the only one complex dissolves in the lymph. The 99mTc-Dextran kits have more than 90 percent labeling yield, 4 hours bench-life, and1.5 years shelf-life. The labeled product is remarkably cheaper than the others.Item Manual and automatic leucocyte counts.(1985-03) Rungpitarangsi, B; Sittapairochana, C; Singprasert, P; Gonggetyai, V; Rungpitarangsi, VA total of 2012 venous blood samples of healthy individuals were determined for leucocyte counts. The samples were taken between 9.00 to 12.00 a.m. Samples of 503 males and 504 females were examined by manual method, while another 502 males and 503 females were counted by electronic or automatic machine. The results were evaluated by Student’s t-test. The mean + standard deviation of ages in the manual group was 30.9 + 8.6 years for males and 27.4 + 7.7 years for females. They had a mean + standard deviation of total leucocyte counts 7003.78 + 785.76 cells per cu.mm. for males and 7028.83 + 778.09 cells per cu.mm. for females. There was no significant statistical difference (p=0.98981). The mean + standard deviation of ages in the automatic group was 29.5 + 7.4 years for males and 29.3 + 7 years for females. The mean + standard deviation of total leucocyte counts was 6482.68 + 1267.98 and 6819.89 + 1252.55 cells per cu.mm. for males and females respectively. the difference was not significant (p=0.98194). There was no significant statisfical difference in total leucocyte counts 1. between manual and automatic method 2. between males and females The average + standard deviation of total leucocyte counts of 2012 healthy individuals was 6924 + 1053 cells per cu.mm. The small variation of leucocyte counts among subjects aged 10 to 50 years was also observed.Item Starch, sucrose, glucose, fructose and acidity in Thai vegetables.(1985-03) Suphavilai, Rungnapa; Niyomporn, BunrueangVegetables are the main sources of vitamins, minerals and plant fibres. Some vegetables contain also protein and carbohydrate in considerable amounts. The amounts of carbohydrate of various vegetables should be particularly considered because their values in most nutrition tables are usually determined by “difference”. They were calculated by subtracting from 100 the sum percentages of protein, lipid, ash, water and other substances. Thus the values included not only both available and unavailable carbohydrates, but also all errors which occurred in the process of determination of each substance. In addition, the amounts of various kinds of available carbohydrate, viz, starch, sucrose, glucose, and fructose in Thai vegetables which may posses some clinical significance have never been reported. The present study reveals the amounts of various kinds and total amounts of available carbohydrates of 45 Thai vegetables determined by the same enzymatic method as described in the study of Thai fruits, but the amounts of expensive enzymes and some reagents were reduced to one half, and the procedure was somewhat modified to attain the best accuracy. The results showing the amounts of available carbohydrate are as anticipated. Of 31 kinds of vegetables, the values of carbohydrate content present in the “Table of Thai Food Composition” of the Division of Nutrition, Department of Public Health, are 531% (range -43 up to 5,669%) higher than those of available carbohydrate; especially the values from the vegetables which compose of much hard fibres (coral leaves and young leaves of lead tree) make the greatest difference (3,746 and 5,668%), but the amazing actual amount difference presents in djenkol bean (24.27 g/100 g difference). Thirty nine kinds (86.7%) contain only 0.13-3.84% of available carbohydrate, and only 6 kinds (13.3%) contain 4.7-5.4%; these are pumpkin, carrot, mung bean sprouts, djenkol bean, sweet pea and yam bean. Leafy vegetables have lower values than the others. The main sugars are in the form of glucose and fructose. In only 3 kinds, sucrose forms more than 50% of the total sugars; they are carrot, ivygourd and djenkol bean, but the last two have actually little amounts of sucrose (0.28 & 0.16%). Nineteen kinds (42.2%) contain starch (0.14-4.85%) and of these only 6 kinds (cockroach berry, pumpkin, sweet pea, yam bean, mung bean sprouts and djenkol bean) contain more than 1%. As in Thai fruits, it is noticeable that the patterns of same species are almost the same. The acidity of Thai vegetables is generally low (pH = 5.64+0.46, titratable acidity = 1.78+1.49 mE/100g edible portion). From the stated three characteristics of Thai vegetables, i.e. low content of available carbohydrate, same sugar patterns of the same species, and low acidity, the study on 45 kinds of Thai vegetables should be sufficient to represent Thai vegetables in general. It may be concluded that the available carbohydrate in Thai vegetables is generally much lower than in Thai fruits, and the amounts of fructose as well as sucrose are too low to be significance in an intolerated patients. Of the investigated vegetables, only 5 kinds (sugar pea, carrot, onion flower, pumpkin and onion) contain 1-2% of fructose or sucrose. Vegetables are therefore very appropriate for weight – or carbohydrate intake – reduction because they can be consumed as much as desired with negligible calories of negligible amount of available carbohydrate. On the other hand, one must keep in mind that people who consume vegetables as their main food, may have deficient intake of calories, protein, and some vitamins.Item Phthiriasis palpebrarum.(1985-03) Chayakul, VisootrPhthiriasis palpebrarum by crab louse or Phthirus pubis involving the eyelids is rarely found in adults. This paper reports three patients who came to Songklanagarind Hospital with the symptoms of itching and bleharitis. The treatment for these three cases of Phthiriasis palpebrarum were successful with no relapse.Item Estimation of stature of Thai and chinese from the length of femur, tibia and fibula.(1985-03) Sangvichien, SJ; Srisurin, V; Watthanayingsakul, VStature, maximum length and anatomical length of Femur and Tibia, maximum length of Fibula, were masured in 77 Thai and Chinese cadavers, comprised of 50 males and 27 females. Eight estimating equations, with standard error were calculated for each sex. These estimating equations may be used for calculation of stature of the Thai and Chinese in Thailand, when length of long bones of lower extremity are available for measurement.Item Radiation complications in patients with oral irradiation: A clinical study at Dental Department, Siriraj Hospital.(1985-03) Vatanasunthon, SA total of 90 patients with radiation complications following oral irradiation, attended the dental clinic between January 1979 and December 1982 were studied. This group comprised 6.9 percent of the total 1296 patients with oral irradiation in the department of radiology. Patients with carcinoma of the gum were the majority group (16.8%) who needed to attend the dental clinic to correct complications. Radiation dental caries was the most common (75.5%) complication. Twenty-eight point eight percent of patients were refered to a dentist before irradiation, while 71.2 percent were refered after irradiation. The author suggested that some oral complications could be prevented and the dentist should have a more greater role in treatment of patients with oral irradiation. Close-cooperation between the radiotherapists and the dentist would minimize complications. Patients with oral irradiation should be refered to a dentist both before and after radiation treatment.Item New method for measuring blood volume.(1985-04) Ruksawin, N; Attanatho, V; Skulchan, V; Chaudakshetrin, PPatients without nausea received a capsule of 200 mg of Sn Cl2 orally and waited for 1 hour or patients with nausea were injected intravenously with a dose of MDP-Sn (II) and waited for 10 minutes. After the waiting time, 10 ml of blood was withdrawn. Whole blood was incubated with 99mTc 200 ตCi for 30 minutes and 1 wash with normal saline was sufficient. This procedure of labeling RBC with 99mTc is much more convenient than the conventional one. We have used this method since 1979 until 1984 with 52 patients without any adverse reaction occurring. The results are satisfactory not only in accuracy and cost but also in reducing the radiation dosage to patients and operators. This report offers 3 main benefits for this procedure. First of all it can help doctors to make decisions about the priority of examination. Secondly, it can be useful for those who are looking for a good method of measuring blood volume. Finally it can be most helpful in a teaching programme.
