Abnormal reports are commonly encountered even in the absence of thyroid dysfunction. Concomitantly used medications are often implicated in this effect. Objectives: Failure to recognize drug-induced alterations in TFTs can result in misdiagnosis and inappropriate management. This review seeks to increase awareness of the common challenges and pitfalls encountered when interpreting drug-induced changes in TFTs. Methods: We did a literature search for eligible publications and articles published before April 2024 on drug-induced alterations in TFTs. Results: The common mechanisms by which drugs alter TFTs include assay interference and alterations in levels or affinity of thyroid-binding proteins (TBP). High-dose biotin can result in erroneous TFTs by interfering with immunoassays that utilize streptavidin-biotin immobilizing systems. The TFTs mimic findings seen in thyrotoxicosis after biotin intake. Heparin-induced increase in nonessential fatty acid causes displacement of thyroid hormone from TBP, resulting in an artefactual increase in free hormone levels. Increases or decreases in thyroid-binding globulin (TBG), the major TBP, cause concomitant changes in total thyroxine and triiodothyronine levels. Estrogen, selective estrogen receptor modulators (SERMs), 5-fluorouracil, mitotane, clofibrate, heroin, and methadone increase, while androgens, anabolic steroids, glucocorticoids, nicotinic acid, and L-asparaginase decrease TBG. Some medications like salicylates, frusemide, carbamazepine, and phenytoin displace thyroxine from TBP, producing a decline in the total hormone levels. Accidental burns during photochemotherapy have been reported though nodular prurigo has never been reported. Patient’s expectations have also increased and they expect exact estimations to plan their pregnancies. Fetal kidney length (FKL) alone or in combination with other biometric indices can be used in estimating GA more precisely. Objective: The study aimed in measuring FKL and calculating GA using it as a single parameter and comparing the accuracy when combined with other biometric parameters used in estimation of GA on ultrasonography in third trimester in Indian women. Material and methods: Healthy pregnant women in third trimester with single live fetus with no maternal or fetal complications were selected for the study. FKL along with other biometric parameters were measured. The results were analyzed for finding if FKL could be used as a single parameter in estimation of GA and if the accuracy improved if combined with other indices. Results: GA was estimated in 715 cases by all four parameters in the women who came in the third trimester for ultrasonography. FKL was also measured simultaneously in these women. It was observed that average FKL increased linearly from 30.7 mm at 27 weeks to 39.4 mm at 38 weeks. Mean left kidney length was slightly but significantly more than the right kidney. GA was estimated by FKL alone and it gave results with SE ± 10.45 days and when combined with other indices accuracy improved to SE ± 5.45 days. Conclusion: FKL can be a single lone parameter in estimation of GA to give an accuracy of SE ± 10.45 days, but when used in combination with other four indices the accuracy increased to SE ± 5.5 days.Item Accuracy, Precision, Sensitivity, and Patient Preferences of Accu-Chek® Self-Monitoring Blood Glucose Meters: A Targeted Literature Review(IJCP Group, 2025-02) KALRA, SANJAY; KAPOOR, NITIN; SOORAGONDA, BASAVARAJ; SHUKLA, RISHI; SHEIKH, SHEHLA; CHANDRASEKARAN, SRUTI; SHINDE, SURESH N; UNNIKRISHNAN, AG; MAYOR, REGINA; PRESENTI, CATERINA; ROETSCHKE, JULIA; GALA, VISHAL; AUGUSTINE, ABINSelf-monitoring of blood glucose (SMBG) using blood glucose monitoring (BGM) devices is recommended for people with diabetes to improve glycemic control and to detect and prevent episodes of hypoglycemia in these patients. The International Organization for Standardization (ISO) and World Health Organization (WHO) have defined specific criteria for accuracy, precision, user evaluation, and interfering agents for the quality of these devices. In this targeted literature review, Accu-Chek® devices (Instant®, Guide®, Active® ) were found to have stable results with appropriate accuracy and precision and did not respond to interfering agents. Acinic cell carcinoma usually occur bilaterally, it presents in younger median age. It is a slow growing, painless tumor. It affects females more than males. Facial nerve is spared. It commonly presents as fever along with headache, bodyache, rashes, polyserositis, bleeding manifestations and rarely as cardiovascular, gastrointestinal, renal or neurological manifestations. Here, we report a case of dengue fever with hypokalemic quadriparesis. A 38-year-old male admitted with history of fever of 3-day duration and weakness of all four limbs of 1-day duration. His investigations showed Hb - 16.20 g/dL, platelet - 37,000/mm3, serum Na+ - 150.00 mmol/L, serum K+ - 2.50 mmol/L and positive dengue serology. Investigation undertaken led to the diagnoses of acute pancreatitis.Item Acute Renal Failure and Silent Myocardial Infarction Following Multiple Honey Bee Stings.(2014-01) Fernandes, Kavina Marian; Srikant, R Gadwalkar; Shyamala, GMassive envenomation by honey bee sting is capable of causing multiorgan dysfunction as a result of direct toxic effect of venom and secondary to systemic anaphylactic reactions. Myocardial infarction (MI) due to honey bee sting is rare, so is acute renal failure (ARF). The probable mechanism is severe coronary arterial spasm with secondary in situ thrombosis as a result of systemic anaphylaxis. This is a case of Kounis syndrome, which is the concurrence of acute coronary syndromes with conditions associated with mast cell activation. We describe a case of ARF and MI in a 58-year-old man after multiple honey bee stings; clinically silent and detected on electrocardiography and by cardiac biomarkers.Item Addison Disease: Early Detection and Treatment Principles.(2014-11) Michels, Aaron; Michels, NicolePrimary adrenal insufficiency, or Addison disease, has many causes, the most common of which is autoimmune adrenalitis. Autoimmune adrenalitis results from destruction of the adrenal cortex, which leads to deficiencies in glucocorticoids, mineralocorticoids, and adrenal androgens. In the United States and Western Europe, the estimated prevalence of Addison disease is one in 20,000 persons; therefore, a high clinical suspicion is needed to avoid misdiagnosing a life-threatening adrenal crisis (i.e., shock, hypotension, and volume depletion). The clinical manifestations before an adrenal crisis are subtle and can include hyperpigmentation, fatigue, anorexia, orthostasis, nausea, muscle and joint pain, and salt craving. Cortisol levels decrease and adrenocorticotropic hormone levels increase. When clinically suspected, patients should undergo a cosyntropin stimulation test to confirm the diagnosis. Treatment of primary adrenal insufficiency requires replacement of mineralocorticoids and glucocorticoids. During times of stress (e.g., illness, invasive surgical procedures), stress-dose glucocorticoids are required because destruction of the adrenal glands prevents an adequate physiologic response. Management of primary adrenal insufficiency or autoimmune adrenalitis requires vigilance for concomitant autoimmune diseases; up to 50% of patients develop another autoimmune disorder during their lifetime.Item Addressing Lower Segment Cesarean Section Hesitancy: A Patient-centered, Pragmatic Communication Guide(IJCP Group, 2022-08) Kalra, Bharti; Sharma, Anjali; Dhingra, Kamlesh; Agrawal, SwatiIn women in whom a normal vaginal delivery is not possible or is not indicated, delayed acceptance of lower segment cesarean section (LSCS) or outright LSCS refusal, leads to complications that can easily be avoided. Hence, it makes sense for obstetricians and other health care professionals, to address LSCS hesitancy as an integral part of obstetric care. In this article, we discuss both the communication style and communication content, that is required to manage LSCS hesitancy in women in whom the intervention is indicated. We highlight the need to analyze the reasons for hesitancy, and address them in an appropriate and affable manner, using accurate information to buttress one’s points.
