Abstract
TMD is a disorder where the temporomandibular joint becomes painful and does not work properly. Often TMD is treated using various types of therapy such as physical therapy as well as ?rst approach non-opioids analgesics. A pain relief treatment for TMD includes an injection into the temporomandibular joint followed by saline ?ushing, known as arthrocentesis, can be effective too. In fact, duloxetine which is a serotonin and norepinephrine reuptake inhibitor has been found to have an analgesic effect in numerous chronic pain diseases. We present here a case report of a 25-year-old female who had TMD related pains in her past history. The patient also had gone through failed treatments including physiotherapy and NASIDS. Her symptoms were severe enough to interfere with her daily activities. This was done simultaneously with arthrocentesis on the other hand duloxetine (20mg/day) was able to alleviate her pain drastically after seven days of treatment. She experienced decrease in VAS score from 8/2 after four weeks post-treatment. Throughout the course of treatment there weren't any reported adverse effects observed in this patient's case report. As stated by this case study, combination of duloxetine and arthrocentesis may provide a valuable remedy for TMD associated pains.
