Abstract
PROBLEM/BACKGROUND: Upper gastrointestinal tract malignancy (one of the top ten malignancy) is an important malignancy founded in Thailand and all around the world.OBJECTIVE: To study incidence, characteristics and manifestations of upper gastrointestinal cancers in patients receiving endoscopic exam.RESEARCH DESIGN: A retrospective cohort study.SETTING: Endoscopic Unit, Department of Medicine, Buddhachinaraj Phitsanulok Hospital.MATERIALS and METHOD: Records of Endoscopic Unit between 1993 to 2002 were reviewed. Collected data were general information, symptoms and signs, sites, characteristics and cell types of the cancers which presented in frequency and percentage.RESULTS: Esophageal cancers were founded in 83 patients. Incidence of esophageal cancer was 772 of 100,000 population at risk. Most of patients (90%) were males. The most common complaint was dysphagia that founded in 72% of cases. Middle and lower esophagus were common sites. Squamous cell carcinoma was the most common cell type of esophageal cancer (84%). There were 115 gastric cancer patients. Incidence of gastric cancer was 1,070 of 100,000 population at risk. Most of patients (60%) were males. The most common complaints were abdominal pain and dyspepsia, the second symptom was upper gastrointestinal hemorrhage (52% and 27% respectively). Lesions located in antrum and pylorus about half of gastric cancer patients. Adenocarcinoma was the most common histological finding (86%) and lymphoma was the second finding (7%). Helicobacter pylori infection was detected in only 11 of 23 (32%).CONCLUSIONS: Gastric cancer is more common than esophageal cancer and frequently found in male. Dyspepsia and abdominal pain are the most common symptoms of gastric cancer while dysphagia is the most common symptom of esophageal cancer. Helicobacter pylori is found in only one third of gastric cancer patients.
