Abstract
We report a case of 28 weeks gestational Age preterm neonate born via lower segment cesarian section who presented with gross pneumoperitoneum immediately after birth. Abdominal drain was inserted as a lifesaving procedure followed by laparotomy after stabilizing the child. Intraoperatively there was sealed ileal perforation which was managed surgically. Pneumoperitoneum secondary to antenatal ileal perforation is a rare manifestation of spontaneous intestinal perforation (SIP).
