Here we had a case series of patients presented with delayed neurological complication of organophosphorus compounds (OPC).Item Efficacy and safety of biosimilar romiplostim in Indian patients with chronic immune thrombocytopenia: A multicentric retrospective study(Wolters Kluwer - Medknow, 2023-03) Iqbal, A; Sharma, C; Bora, RL; Phukan, A.Context and Aims: To evaluate the efficacy and safety of biosimilar romiplostim in Indian patients with immune thrombocytopenic purpura (ITP). Settings and Design: Multicentre, retrospective observational study. Methods and Material: Patients with chronic ITP who received biosimilar romiplostim from July 2019 to March 2020 across 3 major hospitals in Guwahati, India, were included. The study outcomes were the platelet response (platelet count > 50 × 109/L), time to first response, number of dose-limiting events, and the median effective dose. Statistical Analysis Used: Descriptive. Results: Of 32 patients included in this analysis, majority (59.4%) were females. The mean (SD) age was 40.37 (15.79) years, and mean age at ITP diagnosis was 38.53 years. The median number of romiplostim doses were 27.5 (range: 10-42) over a period of 10 months; median romiplostim dose used was 4.2 ?g/kg (range: 2.8-5 ?g/kg). Platelet response was achieved as early as after one week in 9 (28.12%) patients, which continued to increase to 24 (75%) patients after the second, 30 (93.75%) patients after the third and all 32 (100%) patients after four weeks of romiplostim administration. The median platelet count was 161 × 109/L. Dose reduction was done in a total of 21 patients. Thrombocytosis (46.88%), elevated liver enzymes (15.63%) and myalgia (15.63%) were the most common adverse events. Conclusions: Biosimilar romiplostim was effective in achieving and maintaining platelet response without any new safety concerns in Indian adult patients with chronic ITP. The median effective dose of romiplostim required in our patients was lower as compared with the standard prescribed dose.Item Food reduces the bioavailability of lamotrigine.(2005-05-07) Sharma, C; Dubey, R; Kumar, H; Saha, NBACKGROUND AND OBJECTIVE: Lamotrigine is a relatively newer antiepileptic drug used in the treatment of partial seizures. It has a narrow therapeutic index and hence the achieved blood level of the drug is closely related to the therapeutic efficacy and toxicity. The bioavailability of lamotrigine is affected by food, hence the present study was done to investigate the effect of two types of food on the bioavailability of lamotrigine. METHODS: A randomized, open-label, three treatment, three period, single dose and cross-over study was done in nine healthy male volunteers. A single dose of lamotrigine (100 mg) was administered at three occasions: after a north Indian diet (high calorie, high fat), after a south Indian diet (low calorie, low fat), and after an overnight fasting. Serial blood samples were collected up to 24 h post dose. Plasma lamotrigine concentrations were determined and pharmacokinetic parameters studied. RESULTS: A statistically significant decrease in rate and extent of absorption was observed with north Indian diet and south Indian diet when compared with fasting group. Presence of both types of food causes decrease in mean values of C(max), AUC((0-t)), and AUC((0-alpha)) thus reducing the bioavailability of lamotrigine at significant level (P<0.05). INTERPRETATION AND CONCLUSION: Lamotrigine should be taken preferably in fasting condition, as presence of food (low fat, low calorie or high fat, high calorie) decreases the bioavailability to a significant extent.Item Gluconeogenesis in rat placenta during foetal development.(1977-01-01) Bagewadikar, R S; Sharma, C; Nadkarni, G BItem Lower limb lymphedema mimicking as elephantiasis in intravenous drug user(Medip Academy, 2024-06) Aryan, S; Singh, M; Sharma, C; Khutan, H; Chahal, J; Kumar, N.India been the population of over a billion, it is estimated that 62.5 million people use alcohol, 8.75 million use cannabis, two million use opiates, and 0.6 million use sedatives or hypnotics. Intravenous (IV) drug abuse is fairly spreading throughout the country. Rapid urbanization and migration to metro cities have led to disturbance in the social life render an individual more vulnerable to stresses and strain of modern life and with easy availability of drugs, sharing needle, peer pressure are also among the commonest reason for exponential rise of intravenous drug user (IVDU) especially among the young generation. IV drug abuse is one of most serious and social evil, prevailing in our society that leads to number of infections from local cutaneous infections to dreadful like human immuno-deficiency virus (HIV) which in turn not only increase the morbidity but mortality as well. Here we have patient who had history of IV drug injection to lower limb that led to chronic lower limb swelling which on further investigation turns out to be grade IV lymphedema.Item Molecular analysis of the cholera toxin gene & antibiotic sensitivity profile of Vibrio cholerae O1 & O139 associated with mixed infection.(1998-05-22) Sharma, C; Ghosh, A; Ghosh, R K; Mukhopadhyay, A K; Nair, G BIn the context of the reemergence of V. cholerae O1 in India and the recent evidence that O139 strains could have evolved from O1 E1 Tor strains, restriction fragment length polymorphism (RFLP) of the rRNA and the ctx genes and the antibiotic sensitivity profile of the two strains of V. cholerae, one an O1 and the other an O139, associated with mixed infection, were examined to determine their relatedness. Our results demonstrate that although the strains belonged to different clones of V. cholerae, they showed similar antibiotic sensitivity, profile indicating some exchange of genetic elements.Item Paradoxical response to antitubercular drugs.(1995-11-01) Kumar, S; Puri, V; Mehndiratta, M M; Gupta, S; Bhutani, A; Sharma, CSeven patients with paradoxical response to antitubercular drugs are reported. In three cases of intracranial tuberculomas, newer lesions appeared and in two cases preexisting tuberculomas enlarged. In two cases of tubercular meningitis, multiple tuberculomas appeared. All these cases exhibited newer symptoms and CT/MBI revealed the paradoxical response to antitubercular drugs. All responded to continued conservative therapy, with addition of pyrazinamide.Item Perceived risk, behaviour changes and health-related outcomes during COVID-19 pandemic: Findings in a Medical College.(Department of Community Medicine, Institute of Medical Sciences, Banaras Hindu University, 2024-06) Matthew, K; Patankar, F; Sharma, C; Behera, A; Saxena, A.Aims & Objectives:To examine perceived infection risk of COVID-19and the health and related behaviour changes among adults and to examine factors associated with self-reported health during the national quarantine period in India.1Methods:A cross sectional study was carried outvia social media across India. A sampleof 100 adults of India provided data on socio-demographic characteristics, awareness, and attitudestowards COVID-19, lifestyle factors, and health outcomes during the quarantine. (Regression models tested associations among study variables adjusting for covariates.) Results:Among the 100 total participants,overall, the data suggests that the COVID-19 pandemic has had both positive and negative impacts on people's health and well-being(78%) perceived themselves to be at risk of COVID-19 infection. Participants made a number of behaviour changes in response to the pandemic, including wearing masks in public (94%), washing hands frequently (93%), and avoiding social gatherings (87%). However, some participants engaged in risky behaviours, such as smoking (18%) and consuming alcohol (15%). Participants with comorbiditiessuch as diabetes, hypertension, and cardiovascular disease were more likely to report poor health outcomes. The study also found that participants who were more educated and had higher incomes were more likely to have good knowledge about COVID-19 and engage in preventive behaviours.Item Rare case report of oral myiasis in immunocompetent patient(Mansa STM Publishers, 2024-01) Shubham, A; Sharma, C; Kumar, N; Garg, R; Khutan, H; Chahal, J.Oral myiasis is a condition in which the soft tissues of different parts of the oral cavity are invaded by the parasitic larvae of flies known as maggots. Parasites causing myiasis belong to the order Diptera. Oral myiasis is often seen secondary to oral wounds, suppurative lesions, and extraction wounds, especially in individuals with neurological deficits. In such cases, neglected oral hygiene and halitosis attract the flies to lay eggs in oral wounds, resulting in oral myiasis. In 140 cases, a foreign body and in 30 cases mucus plug was removed. In 47 cases there was inflammation of the tracheobronchial tree. Forty patients with empyema thoracis were evaluated bronchoscopically because of persistence of bronchopleural fistula or continued pus discharge from intercostal tubes not responding to the routine treatment. Interval debulking surgery after neoadjuvant chemotherapy (NACT?IDS) is a reasonable alternative. Methods: This study was a retrospective review of patients of Stage III/IV EOC treated either by UDS or NACT?IDS between January 2016 and December 2018 to report the comparison of progression?free survival (PFS) and overall survival (OS) of patients with advanced?stage EOC treated with either UDS or NACT?IDS. Results: Out of 50 patients, 19 (38%) underwent UDS, and 31 (62%) received NACT. The mean follow?up duration was 27.7 months. No gross residual disease was achieved in 52.6% of the UDS group and in 70.4% of the NACT?IDS group. The median PFS of 20 and 30 months was observed in the UDS and NACT?IDS groups, respectively (log?rank P = 0.054). The median OS was 36 months in the NACT?IDS group and could not be reached in the UDS group (log?rank P = 0.329). Only residual disease was significantly associated with survival (hazards ratio 3.03, 95% confidence interval: 1.19�74) on multivariate Cox regression analysis. Conclusions: In advanced?stage EOC, the survival outcomes of NACT?IDS are comparable with those of UDS. Apart from the patient?specific parameters, the decision for UDS or NACT?IDS should take in account the expertise of the surgeon and the institutional capacity as a whole.Item Wandering Distal End of Ventriculo-Peritoneal Shunt: Our Experience with Five Cases and Review of Literature.(2012-09) Kundal, V K; Gajdhar, M; Sharma, C; Agrawal, D; Kundal, RVentriculo-peritoneal (VP) shunt is the most commonly performed procedure for the management of hydrocephalus. Although shunts have improved the morbidity and mortality associated with disordered Cerebrospinal fluid (CSF) mechanics over the past 30 years, they still are associated with many potentially avoidable complications in clinical practice. However, extrusion of VP shunt catheter is an unusual complication of VP shunt Surgery. We are presenting this unusual complication in five patients. The lower end of the shunt was seen extruding from anal opening in one patient, umbilicus in one, urethra in one, inguinal hernia sac in one and oral cavity in one patient. All of these patients were managed by shunt removal, intra venous antibiotics followed by shunt replacement on the other side except the patient with inguinal hernia sac.
