The larval stage of this parasite can thrive in many parts of the body, most frequently in the liver. Primary ovarian location is very rare. In our case the patient had bilateral ovarian cysts which on histopathological study was found to be hydatid cysts. Moreover one of the cysts was multilocular which is also unusual.Item Cerebral palsy.(1993-10-01) Sarkar, S; Narang, AItem Changes in the arch length following premature loss of deciduous molars.(1999-03-23) Rao, A K; Sarkar, SWhen the normal physiological process of deciduous tooth exfoliation and eruption of its successor is disrupted, a series of changes are observed in the dental arches. The aim of the study was to evaluate the amount of changes in arch length after the premature loss of deciduous molars. The sample consisted of 82 children, 53 without premature loss and 29 with premature loss of either deciduous first or second molar or both, unilaterally. A reduction in arch length was observed both in the maxilla and mandible at the molar region and an increase in arch length at the canine region in the mandible. Reduction in arch length was due to mesial migration of the molar and the increase in arch length was due to the distal migration of canine. It was seen that arch length reduction was more in maxilla as compared to the mandible and that distal drifting of canine was observed only in the mandible.Item Chemical analysis of sound and carious enamel of permanent tooth.(1997-12-15) Sarkar, S; Roychowdhury, PEnamel consists mainly of inorganic material (96%) and only a small amount of organic substance and water (4%). The inorganic material is similar to apatite. The originally found apatite mineral remains basically unchanged except at the surface in contact with the oral tissues, where diffusion processes operate. Enamel reflects the trace element environment present in the tissue fluids at the time of tissue development. These are variations of types and concentration of inorganic elements found in permanent and deciduous enamel. Variations are also seen in sound and carious enamel.Item Childhood diarrhoea associated with cryptosporidium species in a rural community near Calcutta.(1988-10-01) Sengupta, P G; Mondal, S; Gupta, D N; Sarkar, S; Saha, N C; Ghosh, S; Pal, S; Deb, B C; Pal, S CItem Chronic arsenic toxicity in west Bengal--the worst calamity in the world.(1998-01-28) Mazumder, D N; Das Gupta, J; Santra, A; Pal, A; Ghose, A; Sarkar, SSince 1983 large number of people are being encountered with arsenic toxicity due to drinking of arsenic contaminated water (0.05-3.2 mg/l) in 6 districts of West Bengal. Clinical and various laboratory investigations were carried out on 156 patients to ascertain the nature and degree of morbidity and mortality that occurred due to chronic arsenic toxicity. All the patients studied had typical rain drop like skin pigmentation (being inclusion criteria) while thickening of palm and sole were found in 65.5% patients. Other features included weakness (70%), gastro-intestinal symptoms (58.6%), involvement of respiratory system (57.08%) and nervous system (50.6%). Lung function tests showed restrictive lung disease in 53% (9/17) and combined obstructive and restrictive lung disease in 41% (7/17) of patients. Abnormal electromyography was found in 34.8% (10/29) and altered nerve conduction velocity in 34.8% (10/29) of cases. Enlargement of liver was found in 120 cases (76.9%) while splenomegaly in 31.4% cases. Liver function test showed elevated globulin level in 15.8% and alkaline phosphatase in 51.3%, alanine amino transferase (ALT) in 11.8% and aspartate amino transferase (AST) in 27.6% of cases. Evidence of portal hypertension was found in 33.3% patients. Liver biopsy reports of 45 patients showed non-cirrhotic portal fibrosis in 41, cirrhosis in 2 and normal histology in 2 cases. There was no correlation between the quantity of arsenic taken through water and the level of arsenic in hair, nail, liver tissues and the degree of fibrosis. There were 5 deaths of which one had skin cancer. The various non-cancer manifestations which were observed in these patients were much severe than those reported in similar cases in other parts of the world.Item Clinical features of HIV infection in drug users of Manipur.(1994-11-01) Panda, S; Kamei, G; Pamei, M; Sarkar, S; Sarkar, K; Singh, N D; Deb, B CBACKGROUND. The human immunodeficiency virus was first detected in young intravenous drug users in Manipur in 1989 and it quickly reached a high prevalence in this group. Diagnostic facilities are scarce and it is thus important to suspect the presence of the infection by its clinical features. METHODS. We did a cross-sectional survey for 13 months among residents of different detoxification centres of Imphal, Manipur, to study the sensitivity, specificity and positive predictive values of different signs and symptoms occurring at the early phase of the infection. RESULTS. Most of the young injectors in this survey were found to be in the early phases (stage I 43%; stage II 32%; stage III 15% and stage IV 9.9%) of the World Health Organization clinical staging of human immunodeficiency virus infection and disease. Herpes zoster, oral candidiasis, pruritic papular eruptions, jaundice and lymphadenopathy had positive predictive values of 100%, 100%, 93%, 93% and 88% respectively. Cryptosporidial diarrhoea and tuberculosis (pulmonary and extrapulmonary) were also encountered. CONCLUSION. Intravenous drug users in Manipur who have human immunodeficiency virus infection suffer from different opportunistic infections which give rise to clinical features that are easily recognizable. It is important to be aware of these in areas which lack diagnostic facilities for confirming the infection.Item Clinical picture of craniopharyngioma in childhood.(2007-07-19) Hamid, R; Sarkar, S; Hossain, M A; Mazumder, U; Akanda, N I; Parvin, RThis cross sectional and observational study was carried out among the admitted patients of the department of Neurosurgery, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka during the period of 1st March 2004 to 31st March 2005. Among the all intracranial tumours, 2.5-04% are Craniopharyngiomas. Although there is a bi-modal age distribution-1st peak at 5-10 years and the 2nd peak at 55-65 years, the common patients are children (9% of childhood tumour). A typical child with Craniopharyngioma (CP) is short, obese and half blind and has a poor school record. The study has been undertaken to know in details, how child patients with Craniopharyngiomas in Bangladesh present their disease at the hospital. Earlier diagnosis and management may improve the quality of life and longivity. The average age of the patients was 13 years ranging from 07 to 17 years. The vast majority patients were admitted with visual problems as their presenting complaints. Among the patients 33% had total blindness of which 03 had primary optic atrophy and 1 has secondary optic atrophy. Among the remaining 08 patients, 75% were found to have field defect. All patients showed fundal changes ranging from early papilloedema to optic atrophy. We found major endocrinological deficiency in child patients with Craniopharyngioma in 17% cases. Raised Prolactin level may not be significant, because it could be due to stalk effect. Although 25% patients were of short stature, their hormonal profile was within normal range according to age and sex.Item Clinical profile of myasthenia gravis.(1998-11-07) Saha, S P; Mukherjee, S; Das, S K; Ganguly, P K; Roy, T N; Maiti, B; Bhattacharya, S; Sarkar, SSeventy three patients with myasthenia gravis were studied over 9 years period (1987-1995) in departments of neuromedicine, respiratory care unit, cardiothoracic unit of Institute of Postgraduate Medical Education and Research and Bangur Institute of Neurology, Calcutta, with reference to their clinical presentations, laboratory findings and various modes of treatment. Commonest age of presentation was 5th decade in men and 3rd decade in women. Fifty five percent of patients belong to type 2A myasthenia gravis (Osserman classification). Presentation was insidious (67.2%) and course was slowly progressive (65.7%) in majority of cases. Fatigability and ptosis were commonest clinical presentation and diurnal variation was noticed in 60% of cases. Edrophonium test was positive in 90.4% of cases and repetitive nerve stimulation showed 93.5% positivity in 30 cases. 27 patients (36.9%) underwent thymectomy and out of these, 89% of patients showed hyperplastic change and thymoma in 11% of cases. Mortality rate including both operated and nonoperated patients was recorded to be 9.6%. We observed earlier onset of myasthenia in male, higher incidence of oculo-bulbar involvement and lower incidence of respiratory problem and thymoma.Item Clinico-pathological study in large posterior fossa midline tumors.(2007-01-09) Sarkar, S; Hossain, M A; Mazumder, U; Rahman, K M; Akanda, N IThis cross sectional analytic study was carried out among the admitted patients of the department of Neurosurgery, Bangabandhu Sheikh Mujib Medical University, Dhaka during the period of 1st July 2002 to 31st December 2004. The age ranged from 2.5 years to 70 years. The size of all posterior fossa tumors at presentation were more than 3 cm and the mean size of posterior fossa tumor was 4.38 cm. 62% of posterior fossa mid-line tumors were mixed density in NECT (non-contrast CT).Regarding enhancement characteristics, mild-moderate enhancement and marked heterogeneous enhancement was equally distributed 46% followed by marked homogeneous enhancement only 08%.. The calcification was present only in 07 (14%) patients and most of them were ependymoma. Histopathologically, medulloblastoma was the common variety (32%). The CT scan diagnostic modality sensitivity, accuracy and positive predictive value were 100%, 84.78% and 84.78% respectively but in MRI diagnostic modality 100%, 91.30% and 91.30% respectively.Item Comparative evaluation of somatic & excretory-secretory antigens of Entamoeba histolytica in serodiagnosis of human amoebiasis by ELISA.(1996-07-01) Pal, S; Sengupta, K; Manna, B; Sarkar, S; Bhattacharya, S; Das, PThe excretory-secretory antigens collected from the axenic culture medium and conventional somatic antigen prepared from the whole trophozoites of E. histolytica were compared for their efficacy in serodiagnosis of amoebiasis. A total of 280 sera collected from different clinically proven cases of amoebiasis and healthy subjects were analysed against both the antigens in ELISA. Both antigens showed a 100 per cent correlation in amoebic liver abscess cases, patients infected with enteropathogens other than amoeba and healthy subjects. However, excretory-secretory antigens showed slightly higher detection rate in patients suffering from acute amoebic dysentery and asymptomatic cyst passers groups. These results clearly suggested the use of excretory-secretory antigens by ELISA for serodiagnosis of amoebiasis due to its better or equal sensitivity, specificity and easier preparation compare to conventional antigen. The use of excretory-secretory antigen in serodiagnosis will not only help in performing more tests utilizing the same chemicals, but also save the cost, time and troubles for importing the foreign chemicals required for cultivation of E. histolytica.Item Comparative studies on different small intestinal oligosaccharidase activities in some vertebrates and invertebrates like molluscs.(1976-07-01) Palit, M M; Dasgupta, S R; Dutta, D; Sharma, A K; Sarkar, SOligosaccharidase activities of the small intestinal mucosal homogenates were measured in vertebrates viz fish, toad, garden-lizard (calotes), pigeon, rat and some invertebrates viz, molluscsa. Maximum activities of the enzymes Lactase, Sucrase and Maltase were found in the mammalian species rat, whereas much less activities were found in the non-mammalian vertebrates among which toad shows the highest values and garden lizard the lowest. Among the invertebrates Pila globosa shows higher values of all the enzymes than Achatina fulica. The results obtained have been discussed in the lights of phylogeny and diet habits.Item A comparative study between Antioxidant vitamin and Kellgren–Lawrence grading scale in knee osteoarthritis patients.(2014-01) Dawn, I; Sarkar, S; De, COsteoarthritis (OA) is a strongly age-related joint disorder that is defined as a gradual loss of articular cartilage. Current concepts of the pathogenic mechanisms of OA suggest that there isa shift in the homeostatic balance between the destruction and synthesis of bone and cartilage,with a net progressive destruction of these tissues. Recent studies of the biology of chondrocytes show that these cells actively produce reactive oxygen species (ROS). ROS are capable of inducing apoptotic cell death in chondrocytes, but more important, they can result in the degradation of aggrecan and collagen. Micronutrient antioxidants such as alpha- Tocopherol (Vitamin E) and Vitamin C also act to reduce the effects of ROS. A hospital based case control study was conducted in Burdwan Medical College, West Bengal, India on 76 postmenopausal women aged 45–70 years suffering from Osteoarthritis (OA) of the knee joint, which was diagnosed from symptoms, clinical examinations and radiographic findings. The 151 controls were selected from the healthy persons .plasma vitamin C and serum vitamin E ware measured.. Knee joint radiographs were evaluated with the Kellgren–Lawrence grading scale .Furthermore both were correlated with Kellgren–Lawrence grading scale to find out possible association between the antioxidant vitamins and the disease progression. Results showed that there was a significant decrease in antioxidant vitamin levels in patients with knee joint osteoarthritis as compared to the controls suggests that treatment with antioxidants in the initial stages of the disease may be useful as secondary therapy. Both antioxidant vitamins showed negative correlation with Kellgren–Lawrence grading. Both vitamins decreased with increasing Kellgren–Lawrence grade.
