Item Coagulation abnormalities in systemic lupus erythematosus.(1989-10-01) Saraya, A K; Rai, R; Saxena, R; Singh, R R; Padmakumar, K; Malaviya, A NCoagulation profile was studied in 55 patients of systemic lupus erythematosus (SLE). Abnormal kaolin clotting time (KCT) was observed in fewer patients (12.9%) as compared to abnormal Russel's viper venom time (RVVT, 20.4%) or activated partial thromboplastin time (APTT, 32.7%). Prolonged prothrombin time (PT), observed in 7.3 per cent patients was not found to be a sensitive test for lupus anticoagulant (LAC). The correction of RVVT and KCT on addition of inosithin suggested a deficiency of platelet lipid factor in these patients. The initial value of uncorrected KCT in patient's plasma did not correlate with the amount of inosithin required for neutralisation. Occurrence of thromboembolic events was significantly associated with prolonged KCT. No other clinical feature showed significant association with any coagulation abnormality.Item Community based leprosy survey in U.T. As a secondary outcome measure, we compared ultrasound and magnetic resonance imaging (MRI) for the diagnosis of adherent placenta. Methods We retrospectively analysed the records of 5 years of women with adherent placenta. Twenty-five women with an antenatal diagnosis of placenta increta and percreta operated by transfundal uterine incision were included. Blood loss, transfusion requirements, operative injuries, and maternal and neonatal intensive care unit (ICU) stay were compared among three different types of adherent placenta. Surgical and other outcome measures were also analysed. Results On antenatal screening with ultrasound, an accurate diagnosis could be achieved in all cases of increta and two-thirds of percreta. Antenatal diagnosis by MRI detected 93.3% of increta and all percreta cases. The mean (SD) gestation at delivery was 34 (4.9) weeks in accreta, 34.9 (2.7) weeks in increta and 31 (4.8) weeks in percreta patients. The mean blood loss encountered intraoperatively was 1012.5 (193.1) ml, 1566.67 (566.52) ml and 1591.67 (629.61) ml in accreta, increta and percreta patients, respectively. Inadvertent bladder injury occurred in 3 women who had placenta percreta invading the bladder. There was no long-term morbidity and no mortality. Conclusion Transfundal incision for delivery of baby is associated with the advantage of avoiding the placenta thereby minimizing blood loss.Item Hemoglobin A2 in iron deficiency and megaloblastic anemia: relation with severity and etiology of anemia.(1987-04-01) Rai, R; Pati, H; Sehgal, A K; Sundaram, K R; Saraya, A KItem Installation of solarium--2000 for phototherapy.(2003-01-24) Tiwari, A; Srinivas, C R; Musthafa, M; Rai, R; Surendran, PSunlight is the cheapest and most easily available source of ultraviolet light for phototherapy. The disadvantage of using sunlight as the source of ultraviolet light includes its variation with time , place and season, lack of privacy, and atmospheric conditions. To ensure correct dose of UV light in spite of variation in irradiation from the sunlight,we used a PMA2100 meter [Figure:2] and 2110 UVA detector ( Solar light Co, Philadelphia). Privacy for the patients was achieved by erecting a solarium and incidental UVB radiation was cut off by using window glass as the roof of the solarium. We recommend the solarium as a less expensive alternative to artificial phototherapy units, especially in places where there is plenty of sunlight.Item Medical arthroscopy--emerging era of rheumatological intervention.(2004-04-08) Chaturvedi, V; Chopra, G S; Dutta, V; Singal, V K; Chawla, M L; Rai, R; Bharadwaj, J R; Lahiri, A K; Shahi, B NOBJECTIVE: To evaluate the safety and diagnostic value of arthroscopy performed by a rheumatologist. METHODS: Decisions for performing arthroscopy were taken when detailed clinical history-and relevant rheumatological investigations failed to arrive at a definite diagnosis. Arthroscopies were performed under local anesthesia as a daycare procedure. Synovial biopsies taken during procedures were subjected to histopathological examination (HPE). RESULTS: Of the 50 patients enrolled, 39 were males while 11 were females with mean age of 35.5 years. In lower limb oligoarthritis group of patients, three had macroscopic picture of crystal arthropathy, rest of the 29 patients revealed gross picture indicative of non-specific synovitis. While in polyarticular group of eight patients, three had macroscopic picture suggestive of crystal arthropathy (probably polyarticlar gout) while five were indicative of rheumatoid arthritis. In monoarticular disease pattern (n= 10) macroscopic picture findings were as follows- crystal arthropathy-two, tubercular-three, synovial chondromatosis-one and non-specific synovitis-two. HPE of synovium did not correlate in many cases. CONCLUSION: Arthroscopy using a 4 mm scope under local anesthesia in the hands of rheumatologists is a safe daycare procedure. In few cases arthroscopy helped in arriving at a final diagnosis but many patients remained undiagnosed. Both the rheumatologists and the pathologists require further experience in this field.Item Multidetector computed tomographic findings in jejunogastric intussusception.(2009-11) Mahmood, N S; Rai, R; Suresh, H B; D’Souza, SItem Naxos disease: a rare occurrence of cardiomyopathy with woolly hair and palmoplantar keratoderma.(2008-01-12) Rai, R; Ramachandran, B; Sundaram, V S; Rajendren, G; Srinivas, C RNaxos disease is a rare genodermatosis with woolly hair, keratoderma of palms and soles and cardiomyopathy. A seven-year-old boy presented with woolly hair and hyperkeratotic lesions on the palms and soles since birth. His cardiac status was evaluated and echocardiography revealed early cardiomyopathy. Scalp biopsy revealed hair shaft in an angulated outline suggestive of woolly hair. So the diagnosis of Naxos disease was made. Since he was asymptomatic no treatment was offered but a regular follow-up of the patient and treatment of emergent symptoms should prevent sudden death.Item Nutritional, Antioxidant and Antimicrobial Properties of Citrus Peels: A Sustainable Valorization Approach(Ms. M. Mondal, Ph.D., 2024-12) Sengupta, S; Khatun, R; Kumari, A; Rai, R; Dutta, S; Mondal, T; Chattaraj, B; Bhattacharya, M.Background: Citrus peels are often discarded as waste despite being rich in bioactive compounds with potential health benefits. Traditional extraction methods for these compounds are not always sustainable and can lead to environmental harm. Using a sustainable vaporization approach, the bioactive compounds in citrus peels can be efficiently extracted and retained for various applications. Valorization strategies have been used for various waste streams, including agro-industrial by-products and olive mill wastewater. Aims: Using a sustainable vaporization method, this study seeks to examine the antibacterial, antioxidant, and nutritional qualities of citrus peels. Methods: waste valorization remains a promising approach for sustainable waste management and resource recovery. Quantitative studies and antimicrobial activities were carried out using peels of C. aurantifolia (Key Lime), C. maxima (Pomelo), C. sinensis (orange), Citrus limetta (Sweet Lemon), and C. reticulata. Citrus aurantiifolia is believed to be a hybrid of Citrus medica, Citrus grandis, and a Micro-citrus species, with significant antibacterial activity. Result: Citrus limetta Mosambi is gaining popularity due to its delicious taste and high vitamin-C content. Citrus sinensis is a small, spherical fruit rich in vitamin C and vitamin A. Pummelo, or Citrus maxima, is known for its large, sweet fruits with abortifacient and menstrual stimulant properties. Peels of C. maxima exhibit the highest amount of antioxidant activity equivalent to ascorbic acid and display significant antimicrobial activity against Klebsiella species.
