Item The deep circumflex iliac artery flap for central arch mandibular reconstruction in a patient of peripheral vascular disease: a case report(Medip Academy, 2025-03) Naskar, S; Ghosh, S; Guha, R; Adak, A; Tripathy, ACarcinoma of oral cavity requires resection followed by reconstruction. For the carcinoma of alveolus, lower gingivo-buccal sulcus or gingivo-lingual sulcus of middle 3rd bony reconstruction is required. Generally free fibula flap is preferred. But free fibula cannot be harvested if there is peripheral vascular disease. Here we present a case of carcinoma alveolus of mandible middle 3rd in a case of peripheral vascular disease. Deep circumflex illiac artery used instead of free fibula flap. Patient had a very good outcome, facial cosmesis was good, no hernia in the donor site.Item The Impact of the COVID-19 Pandemic on Health-Care Delivery System for Other Diseases and Antimicrobial Resistance Threat in India(Wolters Kluwer – Medknow, 2022-06) Chakraborty, D; Majumdar, A; Debnath, F; Naskar, S; Dutta, S.India’s health‑care delivery is challenged with different inequalities and the dual burden of communicable and noncommunicable diseases. Lockdown posed negative effects on the growth and economy of the country; simultaneously, some positive effects, like increased health consciousness and adoption of hygienic practices, were also there. Health‑care delivery system faced tremendous challenges in diagnostics, therapeutics, infrastructure for inpatient care, and protection of health‑care manpower. During this period, people chose to self medicate which in turn increased the threat of emergence of antimicrobial resistance. Due to shifting priority to COVID from other diseases, resources were shifted to COVID, affecting the management of other acute and chronic diseases. The launching of COVID‑19 vaccination campaign showed some hope. However, despite the vaccination drive, strengthening infrastructure, and surveillance system, the devastating second wave could not be avoided due to the conglomeration of the crowd for pilgrimage, election campaign, and tourism in an unrestricted manner. It may be concluded that the fourth wave may be short lasting due to increased herd immunity.Item Nutritional Status and its Associated Factors Among Under-Five Children Attending the Well Baby Clinic of a Tertiary Care Institute of Kolkata, India(Dr.Viral R.Dave, 2025-03) Naskar, S; Pattanayak, S; Nath, S; Gupta, K; Sarkar, NH; Sardar, SS.Introduction: A Well-Baby Clinic (WBC) is a primary healthcare facility, where children are monitored for growth and development, receive routine immunization and their caregivers are given appropriate nutritional counselling. Objective: To assess the nutritional status (Underweight and anaemia) of under-5 children attending WBC of a Tertiary Care Institute of Kolkata and to identify any associated sociodemographic characteristics. Method: A cross-sectional study was conducted for a period of 3 months (May 2023 to July 2023) among 106 under- 5 children, attending WBC of a Tertiary care Institute of Kolkata, selected using consecutive sampling. Face-to-face interviews were conducted among caregivers of children using an interview schedule, and anthropometric measurements and haemoglobin levels of the children were obtained using appropriate tools. Nutritional status was defined as Normal, Underweight, and Severely Underweight based on weight for age. Anaemia levels were categorized as mild, moderate, and severe based on haemoglobin levels. Association of sociodemographic characteristics with nutritional status was assessed using Binary Logistic Regression. Results: Among the children, 72.6% belonged within 12-24 months. Almost 19% and 17% of children were Moderately Underweight and Severely Underweight, respectively. Nearly 75% had a history of exclusive breastfeeding till 6 completed months of life, while 34% received formula feed. Regression analyses revealed statistically significant associations between the nutritional status of children with their gender and mothers' education. Conclusion: Nearly 36% of the under-5 children who attended the Well-Baby Clinic of a Tertiary Care Institute in Kolkata were categorised as Moderately or Severely Underweight, indicating that a sizable fraction of these infants had nutritional issues. Furthermore, the nutritional health of these children was found to be statistically significantly correlated with maternal education and gender, highlighting the significance of addressing sociodemographic inequities.Item Prominent early adulthood hyperferritinemia in scrub typhus(Medip Academy, 2024-06) Naskar, S; Chaudhuri, BN; Basak, A; Guchhait, P; Das, S.Background: Scrub typhus is an acute infectious disease, caused by Orientia tsutsugamushi. It is endemic to a geographically distinct region of Japan, Taiwan, China, and South Korea, known as the tsutsugamushi triangle, as well as in Nepal, Pakistan, and Papua New Guinea. In India, the incidence of this disease varies from 4-46% with high morbidity and mortality. Methods: In this cross-sectional study at a tertiary care hospital in Kolkata, India we analysed ferritin blood levels along with different demographic and laboratory test parameters of 60 suspected scrub typhus patients who were also scrub typhus IgM antibody positive during a one-year study period. Results: It was found that serum ferritin levels were unusually high in 21-40 years age group patients along with mildly increased urea and creatinine levels. Other important findings were below 40 years of age males were mostly affected, but above 40 years of age, females were generally affected. Haemoglobin and platelet levels were normal in all age groups. Although WBC count and neutrophils were normal in the 21-40 years age group, in other age groups increased levels were observed. CRP, SGPT, and SGOT levels were uniformly high above 20 years of age. Electrolytes were mildly decreased. Conclusions: It appears that oxidative stress-induced acute kidney injury may be an important reason for the rising ferritin levels in the 21-40 years age group. These important findings may be helpful to clinicians in management of these patients.Item Study of fine needle aspiration cytology evaluation of peripheral lymph nodes(Siddharth Health Research and Social Welfare Society, 2019-08) Gayen, P; Ghosh, S; Naskar, SIntroduction: Lymph nodes are an integral component of the immune system and their enlargement is commonly noted in clinical practice in a wide spectrum of diseases, including infections like tuberculosis and malignancy. FNAC is an important diagnostic tool for rapid evaluation of mainly superficial lesions, especially of lymph nodes. It is cost effective, relatively less traumatic, and enables the pathologist to provide the clinician with a diagnosis in a very short time, and hence is ideal especially for OPD patients. Objectives: 1. To study the age and sex distribution of the patients of FNAC of peripheral lymph node. 2. To study the spectrum of diseases diagnosed on FNAC of peripheral lymph nodes. Methods: Cross-sectional hospital based Observational study. Total 50 patients who had superficial lymphadenopathy were included in this study. Male patients were 21 (42%) and Female patients were 29 (58%). FNAC was performed on this 50 patients. Diagnosis was made by light Microscopy. Result was tabulated and statistical analysis was done. Results: Male patients were 21 (42%) and Female patients were 29 (58%). 50 % patients were in the age group of 21 to 40 years. Reactive hyperplasia was 46% and Granulomatous lymphadenitis was 18%. Cervical lymph nodes were most commonly involved. Conclusion: FNAC is a simple, quick, low cost, minimally invasive and easy diagnostic procedure which is very much helpful in the diagnosis of diseases causing superficial lymphadenopathy in all age groups. Reactive hyperplasia of lymph node was the most common cytological diagnosis followed by Granulomatous lymphadenitis.
