Item Adult onset celiac disease in north India.(2002-07-16) Sachdev, A; Srinivasan, V; Maheswary, S; Mohan, H; Ashish, B; Singh, L SINTRODUCTION: Informations on celiac disease among Indian adults is scarce. With the availability of improved and more accessible diagnostic tools for celiac disease, the disease is being more frequently recognized among the adults. Therefore, a retrospective analysis of duodenal biopsies were performed to identify adult celiac disease among Indian patients. MATERIAL AND METHODS: A retrospective analysis of the patients, who had villous atrophy on duodenal biopsy between February, 1997 to June 2001, was performed. The clinical presentation, laboratory parameters, treatment and follow up details of patients diagnosed as adult onset celiac disease were analysed. Diagnosis of celiac disease was established in these patients as per ESPGAN criteria. RESULTS: There were 68 duodenal biopsies during the study period. Thirteen (10 were under 15 years of age and 3 had followup biopsy) biopsies were excluded. Eleven (20%) out of 55 patients with villous atrophy in their duodenal biopsy satisfied the ESPGAN criteria for the diagnosis of celiac disease. The age at the time of diagnosis ranged from 15-56 years (mean 36.8 years). Male to female ratio was 5:6. Chronic diarrhea (99%) was the most common presentation followed by weight loss (88%) and anemia (66%). Only one patient had refractory iron deficiency anemia (11%). Histopathological examination showed, subtotal villous atrophy in 6 patients and partial villous atrophy in 5. Nine out of 11 patients had raised concentration of IgA antigliadin antibody. Two patients also had raised concentration of antiendomysial antibody. All of them showed favorable clinical response to Gluten free diet. CONCLUSION: Coeliac disease is considered rare in the tropics. The incidence of sperm agglutinating and sperm immobilising antibodies was found to be 14 per cent and 10 per cent respectively in infertile couples. Agglutinating antibodies were detected in 18 per cent males and 16 per cent of females while cytotoxic antibodies were found in 14 per cent males and 10 per cent of females. Only 4 per cent of the normal fertile couples taken as a control group showed positive ASA, hence the difference in incidence of positive ASA between the infertile and fertile couples was found to be statistically significant. Of these 65, 12 (18%) had a negative CRP test at the time of diagnosis and rest all had significantly elevated CRP and m-ESR compared to matched controls at the time of diagnosis. A persistently negative CRP test indicated bad prognosis. With treatment a declining trend of CRP was seen in survivors, but in deteriorating/expired babies the levels kept on increasing. However, m-ESR had no prognostic significance.Item Epithelioid haemangioendothelioma of the rib--a case report.(2001-07-25) Mohan, H; Palta, A; Nada, R; Punia, R S; Jogai, S; Gupta, REpithelioid haemangioendothelioma is a rare primary malignant tumour of the bone that accounts for less than 1% of all primary bone malignancies. The case discussed here is of a 35 years old male who presented with gradually increasing left infrascapular mass attached to 10th rib. X-ray showed an expansile lytic lesion in 10th rib. On FNA the diagnOsis of fibrous dysplasia or fibrous-histiocytic lesions was suggested. The lesion was excised along with adjacent rib. Histopathological examination showed features of epithelioid haemangioendothelioma. Immunohistochemistry revealed focal factor VIII related antigen positivity.Item Evaluation of sepsis screen for diagnosis of neonatal septicemia.(1993-07-01) Sharma, A; Kutty, C V; Sabharwal, U; Rathee, S; Mohan, HFifty clinically suspected cases of neonatal septicemia were studied for evaluating the role of sepsis screen. Sensitivity and specificity of C-reactive protein test, micro-ESR, gastric aspirate cytology for polymorphs and toxic granules in neutrophils were studied singly and in combinations of two and three tests. Positive blood culture was obtained in only 20% cases, thereby underlying the need for a sepsis screen in the diagnosis of neonatal septicemia, especially in areas where adequate micro-biological facilities are lacking.Item Images in pathology. Zinc is necessary for growth, sexual maturation and reproduction. Semen analysis is helpful in ascertaining the evidence of spermatogenesis, sperm transport, sperm quantity and quality and also provides clue to the functioning of the accessory glands i.e. seminal vesicles and prostate. The present study was conducted in 50 oligospermic infertile males and control group consisting of 25 normospermic known fertile males. Semen and Serum samples were obtained from each case in study as well as control group. Semen and serum were processed by wet ash method. The seminal and serum zinc levels of each case in study and control group were estimated. Statistically, serum zinc and semen zinc levels were significantly lower in infertile patients than fertile males. One year following post-operative radiotherapy, he had to undergo surgery for intestinal perforation. On laparotomy the recurrent tumor had involved pelvic organs and sigmoid colon. It now showed predominantly osteogenic sarcoma on histopathologic examination.
