    Awareness, attitudes, and beliefs of the general public towards HIV/AIDS in Hyderabad, a capital city from South India.

    CONTEXT: Information is the first step in human immunodeficiency virus (HIV) prevention. Ignorance about the disease and how the virus is transmitted can generate fear and prejudice towards those who are infected. AIM: To assess the awareness, attitudes, and beliefs of the general public toward HIV/AIDS in Hyderabad, the capital city of Andhra Pradesh. SETTING AND DESIGN: A cross-sectional study conducted for a period of 2 months in 2004 on 800 individuals living in Hyderabad. MATERIALS AND METHODS: A survey was conducted with a questionnaire titled 'General Population Behavioral Surveillance Survey.' The survey used a 5-part, 65-item questionnaire eliciting information about the knowledge of HIV (10 items), modes of transmission (20 items), ways of prevention (10 items), society's attitude toward HIV (15 items), and finally evaluation of Government's measures (10 items). RESULTS: Approximately 80.63% (645/800) of the study population were sketchily aware of HIV/AIDS, but had incorrect perceptions about the mode of transmission or prevention. Despite the vigorous outreach programmes, which the government and other organizations had carried out in the city, many people had several misconceptions about HIV or about people living with HIV/AIDS. CONCLUSION: As it was observed that awareness was more among the literates, the Government and Health educators should provide tailor-made education programmes for those at the lower education levels.

    Bioactivity of marine organisms: Part IX--screening of some marine flora from the Indian coasts.

    Alcoholic extracts of 48 identified species of marine flora were screened for a wide range of biological activities. Of these, 3 extracts showed diuretic activity while 2 extracts showed hypotensive effect.

    Methanolic and chloroform-methanol extracts of 25 identified species of marine fauna have been screened for a wide range of biological activities. Of these, 2 extracts exhibited diuretic activity, while antibacterial, antiviral, oxytocic and spasmolytic activities, were observed in 1 extract each.

    Brain abscess due to Entamoeba histolytica.

    A patient of cerebral amoebiasis due to Entamoeba histolytica with no evidence of disease elsewhere is described. He made a complete recovery after surgical excision of the abscess along with metronidazole therapy.

    A bacteriologically proven case of brain abscess, due to Streptococcus oralis is being reported in a 12‑year‑old girl who is a known case of congenital heart disease. The patient presented with fever, headache and vomiting. Pus cultures yielded S. oralis.

    Cerebral phaeohyphomycosis due to Cladophialophora bantiana.

    Cladophialophora bantiana, a dematiaceous fungus and a member of the family Phaeohyphomycetes, is primarily a neurotropic fungus causing central nervous system (CNS) infection. We report a case of a well preserved, young adult male presenting with a capsuloganglionic abscess caused by C. bantiana, a rare entity. Diagnosis was made based on the mycology and histopathology findings of the aspirate from the abscess through a burr hole. The patient responded clinically to amphotericin B.

    Changing trends in the antibiograms of Salmonella isolates at a tertiary care hospital in Hyderabad.

    The present study was undertaken to compare the changing trends of antibiograms of Salmonella enterica serovar Typhi and Salmonella enterica serovar Paratyphi A isolates. A total of 80 isolates of salmonella obtained from blood cultures between 2001-2004 were included in the study. Identification and antibiotic sensitivities of the isolates were performed by using mini API (bio Merieux, France). Sixty isolates were identified as Salmonella enterica serovar Typhi and 20 were identified as Salmonella enterica serovar Paratyphi A. More than 67% of S.typhi and 80% of S.paratyphi A isolates were sensitive to chloramphenicol. Sensitivity of S.typhi isolates to cephalosporins was found to have increased from 2001-2004 while that of S.paratyphi A showed a decline. With increasing resistance to ciprofloxacin and the possibility of re-emergence of sensitivity to chloramphenicol, the policy of empirical treatment of enteric fever needs to be rationalized.

    Clinicomycological study of candidiasis.

    Opportunistic pathogens like candida are responsible for a major cause of morbidity and mortality in the chronically debilitated and the immunocompromised patient. The clinical data of 100 patients from whom candida species were repeatedly isolated were analysed. Most of the patients (40%) were above 51 years. Postoperative patients (34%) and patients with diabetes mellitus (30%) had a high incidence of candidiasis. All these patients had either long standing indwelling catheters or were on prolonged antibiotics, steroids or cytotoxic drugs as a part of the management for the underlying cause. Candida albicans (38.9%) and Candida stellatoidea (37.1%) were the most common species isolated. Maximum isolation was from urine (140) mostly obtained from patients with indwelling urinary catheters for more than a week or so. None received antifungal therapy except four, two of them died in spite of appropriate antifungal therapy.

    Culture of body fluids using the bact/alert system.

    The BacT/Alert system was evaluated for its utility to recover clinically significant bacteria from several sterile body fluids, other than blood. Of the1500 specimens processed, 530 (35.33%) cultures were recorded positive by the BacT/Alert bottles while only 379 (25%) cultures were recovered on the plate cultures. The mean time to detection was < 12 hours for Gram-negative organisms and 12 - 16 hours for the Gram-positive organisms. The rapid detection of important pathogens like Streptococcus pneumoniae, Streptococcus pyogenes and Salmonella typhi, within 12 hours of inoculation, is of particular interest especially in serious infections like deep-seated abscesses. This enabled the treating physician to start the specific and appropriate antibiotics early and facilitated a better prognosis in the patient. The system is also very efficient in the early detection of anaerobes from the clinical specimens.

    Elective stenting in superior vena cava syndrome caused by idiopathic fibrosing mediastinitis: use of self-expandable wallstent.

    We present a case of superior vena cava obstruction caused by idiopathic fibrosing mediastinitis treated with a self-expandable Wallstent. A Gortex jump graft had been used previously, which was totally occluded. This procedure relieved symptoms and alleviated the need for re-operation.

    Emergency department based HIV screening: an opportunity for early diagnosis in high prevalent areas.

    The Emergency Medicine Department (EMD) is an ideal place for public health interventions and provides ready access to the health care system, offering a great opportunity for HIV testing and counselling. Between 2003 and 2005, rapid test was requested for 59.39% of 10,752 cases from EMD, where as ELISA was requested for 40.61%. Of the 317 HIV reactive cases, available medical records of 249 were reviewed for epidemiological and clinical information. Nearly 42% of total reactive cases detected in our Institute were from EMD. Three percent (317/10,752) were diagnosed as HIV reactive, 1.52% of the total samples were reactive by rapid test and the other 1.43% by ELISA. Two and half percent (163/6386) of those who had rapid testing and 3.53% (154/4366) who had ELISA testing, were identified as HIV reactive. All these cases were diagnosed within a mean EMD stay of 2.5 days. Eighty-five percent of HIV reactive individuals were unaware of their reactive status. Additional 53 cases of asymptomatic spouses were diagnosed as HIV reactive, thus making it possible to seek early treatment for HIV infection. The study emphasizes the importance of offering HIV testing to all patients who present to emergency department.

    Estimation of levels of carbohydrates in mealey bug (Maconellicoccus hirsutus) infected stem of hibiscus rosa sinensis.

    Mealey bug (Maconellicoccus hirsutus) causes dreadful infection in Hibiscus rosa sinensis.Infections are highly virulent and alters biochemical componenets of the species. Therefore an attempt has been made to evaluate the alteration of biochemical components in Hibiscus stem at various stages of infection. The results reveal that partial infected stem had shown 14.2% increase in its total soluble sugars whereas highly infected had shown 40% decrease.The results also show that total soluble sugars had increased significantly(14%) in highly infected stem and noticeable increase (3%) in partial infected.

    Evaluation of commercially available third-generation anti-hepatitis C virus enzyme-linked immunosorbent assay in patients on haemodialysis.

    Restricted antibody reactivity to hepatitis C virus (HCV) synthetic peptides has been observed in HCV-infected patients on haemodialysis (HD). The aim of this study was to evaluate third-generation anti-HCV enzyme-linked immunosorbent assay (ELISA) test systems containing either synthetic peptide HCV antigens or recombinant HCV antigens or a combination of synthetic and recombinant antigens in screening of 69 chronic renal failure patients on HD for HCV infection. Seven patients were detected to have antibodies to HCV by the 'recombinant HCV antigens'-containing kits, of which the recombinant immunoblot assay for HCV confirmed four cases. The recombinant kits had a sensitivity of 100% and a specificity of 66%. However, the ELISA kits with only synthetic HCV antigens failed to detect antibodies in any of the cases (zero sensitivity). Hence a recombinant protein containing ELISA test system is ideal for screening of HCV infection in patients on hemodialysis.

    Hepatitis B surface antigen--3 years experience.

    Epidemiological surveillance of Hepatitis B surface antigen in the city of Hyderabad at our Institute shows that the incidence of HBsAg is considerably high among the general population. In a comparative evaluation of the two techniques used, RPHA and ELISA, the latter emerged as a reliable one with high specificity and sensitivity. Screening of all blood samples by ELISA is imperative for uniform reporting of the incidence. Non risk groups like voluntary blood donors, cardiac patients serve as potential reservoirs of HBV infection.
