Item Adrenal medullary hyperplasia with coexistent cerebral angiomas(Indian Association of Pathologists and Microbiologists, 2018-12) Gupta, L; Bhatt, AS; Mallya, V; Khurana, N; Lal, PSporadic adrenal medullary hyperplasia (AMH) is a rare entity and mimics pheochromocytoma clinically as well as pharmacologically. It is characterized by increase in adrenal medullary cells with the expansion of cells into areas normally not seen. A 59-year-old male presented with chronic hypertension and raised 24-h urinary normetanephrine levels. Radiological and clinical possibility of pheochromocytoma led to left transperitoneal laparoscopic adrenalectomy. Histopathology, however, showed increase in adrenal medullary to cortical ratio, further confirmed by immunohistochemistry. The absence of any well-defined lesion led to the diagnosis of AMH. Furthermore, on routine imaging, two asymptomatic cavernous hemangiomas were seen. We present this case to reiterate that AMH is an entity which should be considered as a differential for pheochromocytoma. Furthermore, the presence of asymptomatic cavernous hemangiomas in the cerebrum, in this case, makes it rarer since this sporadic association is seldom seen.Item Anticonvulsant activity of benziminazoles.(1973-05-01) Gupta, L; Arora, R B; Singh, MItem Clinical evaluation of supplementing tongue brushing to most advocated regime of tooth brushing.(1998-03-30) Gulati, M S; Gupta, LTongue, because of its surface texture contributes significantly in plaque formation and accumulation, has remained a neglected part in the oral cavity. The study conducted on twenty students aged 18 to 23 years, was undertaken to clinically evaluate the role of supplementing tongue brushing to most advocated regime of tooth brushing. In both the phases of the study, plaque scoring was done daily and oral debris was collected on the eighth day. It was observed that tongue brushing when supplemented to the most advocated regime of tooth brushing that is, immediately after breakfast in the morning and immediately after dinner at night significantly reduced the initial rate of plaque formation and total plaque accumulation.Item Disseminated primary cutaneous histoplasmosis successfully treated with itraconazole.(2003-11-24) Singhi, M K; Gupta, L; Kacchawa, D; Gupta, DA 60-year-old immunocompetent lady with disseminated primary cutaneous histoplasmosis is reported. Histology showed a granulomatous skin infiltrate with numerous intracellular PAS positive rounded yeast cells within macrophages. Culture on Sabouraud's dextrose agar yielded a typical cottony white colony characteristic of Histoplasma capsulatum. The efflux and initial efflux rate (K) of noradrenaline was determined in 36 young relatives (mean age 29.2 years) and in 20 young controls with no family history of hypertension (mean age 26.5 years). From the groups of relatives all those with definite hypertension had been excluded. Efflux and efflux rate K was significantly higher in young relatives (50.82 +/- 3.84 and 28.2 +/- 6.3) than in the controls (39.2 +/- 0.71 and 15.2 +/- 4.8). Majority of patients were male (58.27%). The mostcommon reason for ICU referral was respiratory problem (31.75%) followed by cardiovascular (26%). Lung carcinoma wasthe most common among solid malignancy, whereas lymphoma was the most common hematological malignancy. Patientswith hematological malignancy were relatively younger (55 years vs. 63 years). Patients who required inotropes/vasopressorsor mechanical ventilation had poor prognosis than the others. The mortality rate was higher in patient with hematologicalmalignancy than solid malignancy patients.Conclusion: The demand for intensive care for critically ill cancer is increasing, and considering the improvement in prognosiscancer patients should not be denied ICU care merely on the basis of a patient suffering from cancer.
