    Fascia Lata Pedunculated Flap For Upper Third Reconstruction Of The Lower Extremity: A Clinical Case Presentation

    Objectives: To describe the importance and versatility of the fascia latae ap for coverage of wide defects of the upper third and lower extremity. A retrospective study was carried out in a male Method: patient diagnosed with sarcoma of the left inguinal region, who underwent surgical treatment (wide excision with safety margins) + reconstruction and coverage of the bloody area with a local myofasciocutaneous fascia ap of fascia lata. Results: A 64-year-old man reported nodular presence in the left inguinal region of 1 year of evolution, the beginning of the size of a pea, which has been increasing in size and becoming painful on palpation, becoming visible on inspection, on palpation the presence of a hard, xed, painful tumor lesion. Computed Tomography (CT) scan revealed ndings regarding probable myxoid liposarcoma and embryonic biopsy of malignant mesenchymal neoplasia in favor of sarcoma. Extensive excision was performed with margin of injury safety + reconstruction and coverage of the bloody area with myofasciocutaneous local pedunculated fascia lata ap. Sarcoma is a rare and aggressive soft tissue tumor that can appear an Conclusion: ywhere in the human body. There are more than 50 types. Treatment for curative purposes will always be a complete excision with safety margins in both extension and depth, leaving great defects in coverage and functionality in the affected region. It is here where the knowledge of aps is put into practice for better coverage and preserve the functionality of the area involved. In the upper third of the thigh, the pedunculated fascia latae ap is considered to be of choice due to its versatility (cutaneous, fascius, muscular), extension, rotation and very low risk of complications.

    Hepatoblastoma: 5-Year Retrospective Study. Pediatric Oncological Surgery Service "GENERAL Oncology Hospital.Solon Espinosa Ayala" Solca Quito- Ecuador

    Objectives:Describe the demographic variables, types of liver tumor, surgeries performed and survival children diagnosed with liver tumor undergoing surgical treatment.Method: A retrospective analysis was performed in patients with pediatric liver tumors undergoing surgical treatment, from January 2010 to December 2015 at the "General Oncology Hospital. Solon Espinoza Ayala”.Results:Data from our study reported a diagnosis of hepatoblastoma in 51.85% of all pediatric liver tumors; 50% are routine controls without evidence of disease, 14.28% have been completed clinical treatment, 21.42% died from a second primary diagnosis with metastasis, and another 14.28% (only surgery) who were not followed up because they were transfers from another health system; with respect to global survival it was 64%. The ages ranged from 0 to 15 years old with an average of 5.5.Conclusion: It is very important a timely detection and adequate treatment by a specialized center and trained professionals, liver surgery is a very important chapter for the treatment of liver tumors. The surgical approach with tumor-free resection along with multidisciplinary treatment is the goal for healing.

    Relevance And Importance Of Breast Reduction As An Adjuvant In The Treatment Of Dorso-Lumbar Pain In Women With Gigantomastia: Clinical Case Presentation

    Objectives: To describe complications of gigantomastia in women and adjuvant treatment, breast reduction analysis as treatment. A retrospective study was carried out in a young female patient Method: diagnosed with gigantomastia and dorso-lumbar pain, who underwent surgical breast reduction treatment. A 15-year- Results: old female with gigantomastia and onset of dorso-lumbar pain for 10 months, with no evidence of correction of spinal curvature by imaging examinations and evident recurrent cervical, dorsal and lumbar paravertebral muscle contracture despite antiinammatory treatment and physical therapy. Bilateral breast reduction + mastopexy was performed, obtaining excellent therapeutic results for pain, aesthetic and psychological. The exaggerated growth of mammary glands Conclusion: in women (gigantomastia) can bring health problems in the short and long term; mainly back pain that does not completely subside to analgesic treatment and physiotherapy and in some cases can increase and even involve bone damage of the spine and nally be associated with psychological disorders due to dissatisfaction with their body. This is how the importance of a treatment for symptom relief, preventive by reducing the risk of bone alterations in the spine and of course improving the psychological part of the patient arises.
