Fluorescein angiography revealed delayed arterial filling in the right eye and absent arterial filling in the left eye, suggesting bilateral central retinal artery occlusion. Systemic evaluation revealed a history of chronic low-grade fever and generalized lymphadenopathy. HIV (ELISA) was positive, and other systemic comorbidities were ruled out. Cervical lymph node biopsy stained positive for acid-fast bacilli, with large areas of necrosis, palisaded by epithelioid cell granulomas, macrophages, and multinucleated giant cells, suggesting lymph node tuberculosis. When all axillary nodes were free of tumour, the mean mast cell count was 35.75/sq mm. In the metastatic node the mast cell count was inversely proportional to the amount of metastatic tumour. The mast cell number decreased (25.64/sq mm) as the metastasis developed in any of the enlarged axillary nodes. Their count diminished further (23.55/sq mm) as the amount of metastatic tumour increased in individual nodes and when tumour involved all enlarged nodes. Mean mast cell count was lowest (10.50/sq mm) in metaplastic carcinoma. The mast cells in metastatic nodes were found mainly at the edges of tumour deposits.Item The Double Trouble: Caudal Duplication Syndrome-A rare case series with a review of literature(Siddharth Health Research and Social Welfare Society, 2024-04) Datta, D; Saha, S; Dakshit, D; Maity, A; Bansal, R; Basu, NCaudal duplication syndrome (CDS) is a very rare disease entity with prevalence of less than 1 per 100,000 at birth.It includes a broad range of abnormalities and clinical symptoms , from single or partial duplication of the gastrointestinal, genitourinary and distal neural tube system organs to total duplication.There have been several ideas proposed to explain the complicated yet symmetrical abnormalities and the wide range of clinical manifestations of caudal duplication syndrome.In this case series, we present two confirmed cases of Caudal Duplication Syndrome in our institute.The management for this syndrome is individualized and may include surgical intervention to fuse or excise the duplicated organs.Item Effectiveness of competency-based teaching and comparison to traditional teaching of Phase I MBBS students(Mrs Dipika Charan of MedScience (India) Publications, 2022-06) Thind, A; Jain, A; Kumar, A; Kaur, P; Bansal, R; Singh, R.Background: During MBBS course, the students’ study about cardiovascular system (CVS) in physiology, pathology, pharmacology, preventive medicine, pediatrics, and medicine for the management and diagnosis of cardiovascular disorders. Blood pressure measurement is a common and a beneficial tool for the diagnosis of cardiovascular disorders (hypertension, hypotension, etc.) along with their treatment and management. Thus, knowing the proper technique of measuring blood pressure becomes an essential part of learning the course and management of cardiovascular disease. This study is designed to teach and train the first phase undergraduate students to measure blood pressure in a proper way so that they do not stumble on the first step of examination of CVS. Competency-based training is an outcome-based approach that involves identifying the skills acquired by Indian medical graduate. Competency-based teaching involves teaching of undergraduate students to support their achievement and assessment of the communication and technical skills and to develop their ability to interpret the results of blood pressure measurement for apt patient management. Aim and Objectives: This study aims to assess and compare the effect of competency-based teaching and traditional teaching in learning the skill of blood pressure measurement. Materials and Methods: Two hundred students were taken from Phase I undergraduate medical students which were randomly divided in two Groups A and B. Competency-based teaching of blood pressure measurement was done in Group A in by teaching method of demonstrate, observe, assist, and perform and Group B was taught by traditional demonstration. Pre-test questionnaire was given to both the groups before beginning of the practical session. The students learning of skill to measure blood pressure was assessed by directly observed procedural skills (DOPS) checklist after which student feedback was provided along with filling up of the post-test questionnaire. Furthermore, feedback from both student and teacher was collected. Results: Highly significant difference was found between pre-test and post-test of both Group A and Group B. Significant difference was found between Group A and Group B post-test scores. It was found that Group A in which competency-based teaching was done scored higher post-test scores then Group B. Furthermore, highly significant difference was seen among the scores of DOPS checklist between Group A and Group B. Conclusion: In the present research study, it was found that the students who underwent competency-based training of the skill of blood pressure measurement showed significantly higher scores in learning and performing the skill. Furthermore, on feedback provided by the students, 96% of the students strongly agree that the competency-based teaching and learning is better than traditional teaching.Item The effectiveness of nurse-led visual rehabilitation on visual outcome among post-operative patients of sellar/suprasellar tumours-protocol of a randomized controlled trial(Medip Academy, 2025-03) Bharti; Dhandapani, M; Salunke, P; Bansal, R; Sukhija, J; Dhandapani, S.Background: Tumour or mass of the suprasellar region are common pathologies and are also important due to their adjacency to the vital anterior visual apparatus. They may cause serious ocular signs and symptoms in addition to neurological complications resulting from increased intracranial pressure, cranial nerve impairment, or brain compression. Increased ICP may lead to headache, double vision and loss of peripheral vision, can lead to sinus pain or ear pain, drooping eyelid and seizures. After surgery, patients may continue to experience neuropsychological symptoms, physical symptoms and develop complications. Methods: Randomized controlled trial. A computer random table with allocation concealment will be used to recruit and assign patients with visual complaints resulting from sellar/suprasellar tumors operated on at PGIMER to the experimental and control groups. Enrolment will be done on 5th to 7th day of surgery and intervention will be provided on the same day until discharge and booklet with nurse-led visual rehabilitation will be provided for continuous practice at home. The post-op follow-up will be done at 1 month and 3rd month of surgery. Conclusions: This aim of the study is to determine the effectiveness of “nurse-led visual rehabilitation” on visual outcome among post-op patient with sellar/suprasellar tumours at 1 month and 3 months. Conservative treatment, percutaneous spine interventions and surgery have all been used as treatments; and the treatment that is chosen depends on the severity of the clinical and neurologic presentation. Many medical professionals currently use epidural steroid injections for the treatment of lumbosacral radiculopathy. Methods: A prospective study was conducted over one year at Dr. B.R. Ambedkar Medical College and Hospital, involving 70 patients of either gender with lumbar radiculopathy. Pre-procedure evaluation was done and patients were made fit for the procedure Leeds Assessment of Neuropathic Symptoms and Signs (LANSS) score was assessed followed by assessment using the Oswestry Disability Index (ODI). Half of the sample patients were administered epidural steroid injections, remaining half were chosen for selective nerve root block procedure Follow-up of the patients was done at 1, 3 and 6 months, post-procedure and assessed using the Oswestry Disability Index (ODI) questionnaire. Results: Out of the 70 patients, 41 (58.5%) were male and 29 (41.5%) were female. The distribution of age of the patients was 30– 70 years. Duration of symptoms at presentation ranges from 1 to 10 months with an average of 4.10 (±3.13) months. L4L5 (72.8%) level was the most common level involved followed by L5S1 (18.5%), both L4L5/L5S1 together (7.1%) and L3L4 (1.4%). Conclusions: Epidural steroid injection is an easy and safe method with better pain relief and improvement of functional disability than nerve root block. The effect of nerve root block is short-acting in most of the patients and recurrence is repeated in some.
