    PERIOPERATIVE ANXIETY IN CHILDREN: NARRATIVE REVIEW

    Preoperative anxiety in children is common and can have negative effects on their overall surgical experience. Several factors, such as age, personality traits, and past medical experiences, can contribute to increased preoperative anxiety. Behavioral interventions, such as distraction techniques, parental presence at induction of anesthesia, and preoperative preparation programs, can be effective in reducing anxiety. Behavioral family preparation programs have been found to reduce the number of children requiring preoperative sedation, but they may not be practical for all centers. Anesthesiologists and child life specialists can play an essential role in reducing anxiety in children by interacting with them in an age-appropriate way, providing therapeutic play experiences, and offering developmentally appropriate language. Distraction techniques, such as non-procedural distracting talk, electronic devices, music, stories, and guided imagery, can also help children cope with anxiety. Pharmacological interventions, such as sedative premedication, should be used with caution, especially for children with neurodevelopmental disorders, due to potential drug interactions and increased risks of upper airway obstruction.

    A lot of research is available on the effectiveness of search as an advertising channel. Most of these studies tend to treat a click on a search ad as a binary event. All of them study the events leading to the click. This paper goes beyond this to study the post click actions taken by a user subsequent to clicking on a search ad, and refers to those actions as depth of interaction, and testing the variables that have an effect on the nal outcome. We use a prescriptive research design employing binary logistic regression analysis. Results indicate that the duration of time spent, device used, and recency of visit have a very high positive effect on the nal outcome.

    FIELD TRIAL OF CYANOBACTERIA BASED BIOFERTILIZER FOR CULTIVATION OF TRITICUM AESTIVUM PLANT

    The continuous growth of the global human population and the depletion of natural energy resources are posing a threat to the environment and the sustainable supply of food and energy. To address this issue, a solution known as "green technology" has been developed, focusing on the use of eco-friendly approaches. One of these approaches involves harnessing the power of cyanobacteria, which are ancient microorganisms that have existed on Earth for a long time. This approach was used to probe the diversity of phototrophic microorganisms in Wheat samples. The presence of nif genes allows cyanobacteria to x nitrogen which plays a vital role in nitrogen cycle and biological processes such as plant growth and soil fertility. Wheat production test in current study evaluates the yield and quality of wheat crops. It shows factors such as improvement in growth, heat resistance and environmental adaptability, improvement in production; provide insights for optimizing cultivation practices and ensuring successful high yield. Identication of Cyanobacteria for their combination of nif and molecular identity is prior for production of bio fertilizers. Cyanobacteria can be cultivated on a large scale to produce biomass that has multiple uses, including biofertilizer, secondary metabolites, cosmetics, and medicines. In the eld of agriculture, production of healthy crop cyanobacterial biofertilizer are being increasingly used in eco-friendly and sustainable practices. These biofertilizer are produced through mass cultivation of cyanobacteria and offer several benets.

    Medicinal plants are considered colossal producers of bioactive therapeutics agents. Fruit is an essential part of the human diet and is of great interest because of its richness in phytochemicals. The present study was chosen the objectives are assessment the antibacterial activity against the dominantly affected seven Urinary Tract Infected (UTI) pathogenic bacterial species plant leaf extracts and fruits such as Mentha piperita (L.), Flacourtia jangomas (Lour) Raeusch. Experimental UTI pathogenic organisms named as Alcaligens sp., Kliebsiella sp., E. coli, Proteus sp., Staphylococcus sp., Pseudomonas sp., Hence the current result has been clearly depicted that the methanol extract of Mentha piperita leaf expressed the potential signicant antibacterial activity against the ve UTI pathogenic organisms such as Alcaligens sp., and Klebsirlla sp. Proteus species and Pseudomonas sp., than other two experimental extracts of benzene and ethanol. From the present result clearly noticed three elucidated compounds are act as an antibacterial bio-compounds such Octadecanoic acid methyl ester, 1, 2, 4, 5-tetramethyl Piperone and α-Propyl tetradecanol present in methanol leaf.extract of M. piperita (L.) plant. Similarly F. jangomas (Lou.) fruit extract (methanol).possessed the antibacterial property biologically effective compounds named as Lavandulol, α-Humulene and Terpineol. Therefore, both experimental plant leaf and fruit extract possessed signicant antibacterial activity against the tested seven UTI clinical pathogenic organisms Furthermore nanoparticle study also denoted both experimental methanol extracts of M. piperita (L.), (leaf) F.jangomas (Lour) expressed the signicant nanoparticle production. It was noticed the range between 21.50 to 41.03nm and 60.52 to 19.22nm. for M. piperita (L.) F. jangomas fruit juice extract respectively. From the current research clearly showed that methanol extract of both experimental samples could be act as a potential traditional plant based medicines for urinary tract infected diseases.

    NEONATAL SEIZURES: NARRATIVE REVIEW

    Neonatal seizures are often complex and difcult to recognize, but can be identied through electroencephalogram (EEG) monitoring. The Brighton Collaboration has developed a scheme with ve levels of diagnostic certainty to guide treatment decisions when EEG is not available. Different seizure types are usually associated with specic underlying causes, which may require specic diagnostic and treatment approaches. Neonatal seizures require prompt management, including the stabilization of cardiovascular and respiratory function and the identication of the underlying cause. EEG monitoring is considered essential for the detection of seizures and should be performed until the neonate has been seizure-free for 12 to 24 hours. Treatment involves the use of antiseizure medication and may include pyridoxine challenge or other treatment options such as the ketogenic diet, intravenous immunoglobulin, or corticosteroids if seizures are refractory to conventional antiseizure medication. It is important to differentiate between seizures and nonepileptic motor phenomena, which can occur without obvious cause or as symptoms of drug withdrawal, electrolyte abnormalities, hypoglycemia, or infection. Neuroimaging is also considered essential for the detection of possible structural abnormalities in neonates with seizures.

    ANESTHESIA FOR OPHTHALMIC SURGERY: PERIOPERATIVE CONSIDERATIONS

    The use of anesthesia during ophthalmic surgery is crucial for ensuring patient comfort and safety. Different types of ophthalmic surgeries require different types of anesthesia techniques. Cataract surgery is the most performed ophthalmic surgery and is typically performed under local anesthesia with sedation, while glaucoma surgery can be performed using local or general anesthesia depending on the complexity of the procedure. The use of local anesthesia can reduce intraocular pressure (IOP) and is associated with fewer postoperative complications compared to general anesthesia. Three ophthalmic reexes, the oculocardiac, oculorespiratory, and oculoemetic reexes, should be recognized by the anesthesiologist. Appropriate prophylaxis and treatment of postoperative nausea and vomiting are essential for patients undergoing ophthalmic surgery.

    FETAL ECHOCARDIOGRAPHY AS A ROUTINE ANTENATAL SCREENING TOOL FOR DETECTION OF FETAL CONGENITAL CARDIAC STRUCTURAL ANOMALIES.

    Background: Fetal echocardiography plays a pivotal role in detecting congenital cardiac structural anomalies. Though foetal echocardiography is mostly reserved for high risk pregnant women, its role as a routine prenatal screening tool needs to be dened. To evaluate the role of fetal echocardiography as a routine Objectives: antenatal screening tool for detection of congenital cardiac structural anomalies.To compare the prevalence of congenital cardiac structural anomalies by fetal echocardiography in antenatal women with low risk and high risk factors for cardiac structural anomalies. The study was carried out in the department of radiodiagnosis, SNMC, Agra. Materials and methods: 500 fetal hearts between 18-26 weeks gestation were evaluated through fetal echocardiography.They were categorised into two groups -low risk (Group I) and high risk (Group II) and scanned through four chamber , LVOT , RVOT and three vessel view. Result: The prevalence of fetal congenital cardiac structural anomalies in this study is 14/1000.Fetal echocardiography had a sensitivity of 85% & specicity of 99%. PPV is 85% and NPV is 99%. The incidence of congenital cardiac structural anomalies in high risk and low risk group was 12 % & 14.8 % per 1,000 respectively. based on these ndings it is highly Conclusion: suggestive that every pregnant woman should be subjected to a detailed fetal echocardiography. Fetal echocardiography should be included as a part of routine antenatal screening irrespective of risk factors for congenital cardiac structural anomalies.

    NARRATIVE REVIEW OF PSORIASIS: PATHOGENESIS, CLINICAL FEATURES AND MANAGEMENT STRATEGIES

    Psoriasis is a chronic autoimmune disorder that affects approximately 2-3% of the global population. It is characterized by red, scaly, and itchy patches on the skin that can cause signicant discomfort and have a negative impact on patients' quality of life. Despite being a well-known condition, the pathogenesis of psoriasis remains poorly understood, and there is no cure for the disease. Therefore, the management of psoriasis primarily focuses on symptom relief and improving patients' quality of life. The epidemiology of psoriasis varies by geographic location, ethnicity, and age, with some studies suggesting a higher prevalence among certain populations. The most common subtype of psoriasis is plaque psoriasis, which is characterized by erythematous plaques covered with silvery scales. Other clinical subtypes include guttate psoriasis, pustular psoriasis, erythrodermic psoriasis, and inverse psoriasis. Each subtype has unique clinical features and requires specic management strategies. Epidemiological studies have identied several risk factors for psoriasis, including family history, smoking, obesity, and stress. The exact mechanisms by which these factors contribute to the development and progression of psoriasis are not fully understood. However, it is thought that genetic and environmental factors interact to disrupt the normal immune response and trigger the inammatory cascade that drives psoriasis pathogenesis

    STUDY ACCLIMATIZED SEED SAMPLE SAPOSHNIKOVIA DIVARICATA (TURCZ) SCHISCHK IN VIETNAM

    In this study, we evaluated several agronomic characteristics of S. divaricata when grown in various regions in Vietnam, including Northeast, Northwest, and Central Highlands. Among the three regions, S. divaricata grown in Northwestern Vietnam has the highest yield reaching 6.21 tons/ha, with a total active ingredient content of 0.655% (Prim-O-glucosylcimifugin 0.383% and 5-O-methylvisamminoside 0.272%). This is followed by the Central Highlands of which S. divaricata yielded 4.12 tons/ha, with a total active ingredient content is 0.543% (Prim-O-glucosylcimifugin 0.292% and 5-O-methylvisamminoside 0.251%). The lowest yield of S. divaricata was recorded in Northeast with 3.13 tons/ha, of which a total active ingredient content was 0.394% with 0.253% for Prim-O-glucosylcimifugin and 0.141% for 5-O- methylvisamminoside. With the applied analytical conditions, HPLC - DAD chromatograms are obtained with sharp peaks of prim-O-glucosylcimifugin (tR = 7.51 min) and 5-O-methylvisamminoside (tR = 20.23 min) , balanced, clear on the background of the medicinal plants Saposhnikovia divaricata (Turcz.) Schischk. In particular, the applicable analytical conditions allow for simultaneous qualitative and quantitative analysis of both prim-O-glucosylcimifugin and 5- O-methylvisaminoside. The results of building the standard curve prim-O-glucosylcimifugin Y = (16146)X – 4020.3, R2 = 0.9992, standard curve 5-O- methylvisamminoside Y = (20490)X – 6921.8, R2 = 0.9999. The quantitative results of prim-O-glucosylcimifugin and 5-O- methylvisamminoside in all regions were slightly higher than that of the pharmacopoeias of Vietnam, China and Hong Kong with the total active ingredient content not less than 0.24%.     ESTIMATION OF HETEROSIS FOR YIELD AND YIELD RELATED COMPONENTS USING DIVERSE TESTERS IN PEARL MILLET [PENNISETUM GLAUCUM (L.) R. BR.]

    Experiment comprised of twenty six female type parents and seven restorer parents of pearl millet and their 182 hybrids was conducted at two locations viz., ICRISAT, Patancheru (Latitude 17.52° N; Longitude 78.26° E and Altitude 522 m) and research farm of Super seeds Pvt Ltd., Hisar (Latitude 29.15° N; Longitude 75.72° E and Altitude 215 m) during rainy 2018, for studying the extent of hybrid vigour in F1 for grain yield and its components. For days to 50% owering, top cross hybrid MBL-5 X MOPT-31 (-13.37**) showed highest signicant and positive heterosis over check 86M86. The top cross hybrid MBL-17 X MOPT-33 (45.21**) highest, signicant standard heterosis in positive direction over check 86M86 for panicle length (cm). The single cross hybrid MBL-3 X MIT-29 (14.12*) exhibited signicant and positive standard heterosis over 86M86 for panicle girth (cm). for productive tillers, single cross hybrid MBL-8 X MIT-28 (98.34**) showed highest signicant positive standard heterosis over check 86M86 and the single cross hybrid MBL-24 X MIT-28 (18.42) showed positive standard heterosis over check 86M86

    POINT-OF-CARE ULTRASONOGRAPHY: NARRATIVE REVIEW

    Point-of-care ultrasonography (POCUS) is a growing eld in medicine that allows clinicians to use ultrasonography at a patient's bedside, and it has transformed how clinicians deliver care. POCUS is used across various medical specialties, and non-radiologists and non-cardiologists can also become competent in the performance of POCUS, leading to increased adoption of this technology in clinical practice. POCUS and consultative ultrasonography are complementary, and both can be used together to provide a more comprehensive assessment of a patient's condition. In addition, POCUS is an invaluable tool that clinicians can use to monitor clinical conditions that can progress rapidly, such as acute respiratory failure, intracranial hypertension, and hemodynamic failure. Advances in POCUS technology have led to its use as a screening tool for the identication of certain disorders such as abdominal aortic aneurysm and for diagnosis of various medical conditions. POCUS has been associated with changes in clinical decision making in medical practice and has shown promising results in facilitating diagnosis and procedural guidance.

    COMPARISON OF LUNG FUNCTION IN PATIENTS WITH BRONCHIECTASIS DURING ACUTE EXACERBATION AND IN CONVALESCENCE - A PROSPECTIVE STUDY

    Background- Bronchiectasis is a common chronic respiratory disease. A noticeable drop in lung function during exacerbations and recovery during convalescence has been revealed in patients with asthma or chronic obstructive pulmonary disease (COPD). Hence this study was planned with the objectives to compare lung functions during acute exacerbation and convalescence in patients with bronchiectasis. This was a prospective cohort study Methods- conducted in 50 patients over a duration of 18 months. Measurement of exacerbations and convalescence visits comprised of spirometry, sputum bacteriology, serum and sputum biomarker. Student's paired t test, Fischer's exact test or Chi square test was used to analyze the signicance of difference. P value <0.05 was considered as statistically signicant. In this study, Results- there was a signicant association of mMRC dyspnoea score in exacerbation and in convalescence. (p <0.0001). The analysis showed that mMRC dyspnoea score was shifted to lower scores in convalescence compared to that in exacerbation. FEV1, FVC and FEV1/FVC) and 6 MWT distance were signicantly lower in patients in exacerbation as compared in convalescence. Conclusion- There was marked improvement in lung functions in convalescence as compared to patients in acute exacerbation

    The COVID-19 pandemic has caused signicant changes in daily life, with many countries implementing quarantines and transitioning to online learning. As the vaccine is not expected to be available before 2021, online learning has become an inevitable option. Despite EFL/ESL teachers expressing favorable attitudes towards online instruction, there is a need to motivate them to return to universities for face-to-face instruction following the pandemic. This research aims to identify the factors that could encourage EFL/ESL teachers to resume traditional classroom instruction in Iranian universities. It is crucial to provide teachers and students with a safe learning environment equipped with the necessary tools and procedures, as their motivation is a key component of successful teaching and learning. The study calls for collaborative efforts from all concerned parties to raise awareness about the importance of boosting motivation to return to universities after the COVID-19 crisis
