It characterized by encasement of the small intestine in fibrous tissue. The diagnosis is often made at laparotomy for acute or chronic intestinal obstruction. The treatment of choice is sinple excision of the fibrous tissue. This condition was first described by Foo et al in 1978, and until September 1993 only 25 cases have been reported in English literature. This paper reports a case who was admitted and operated recently at the Srinagarind hospital , Faculty of Medicine, Khon Kaen University.Item Abdominal Migraine(Faculty of Medicine, Khon Kaen University, 2010-04-03) Somsak Tiamkaono abstractItem Abnormal Pap Smear in Amphure Kumphawapi(Faculty of Medicine, Khon Kaen University, 2010-05-28) Chaiyan AmnatvuddeeFrom September 1992 to October 1994, 13,234 women in Amphure Kumphawapi were taken Pap smear for screening and prevention of cervical cancer by the local cervical cancer prevention program. 33 women with abnormal Pap smear [class 3,4,5] were found, the estimated incidence was 0.249% , their average age were 44.76 years old, the yongest was 20 years old. Multiple sexual partner was the significant risk, OR = 3.34 [ p=0.00459]. The result indicated that to decrease the high incidence of cervical cancer there should be the effective nation cervical cancer prevention program in all over rural area of Thailand.Item Abnormally Thickened Endometrium in Postmenopausal Breast Cancer Patients(Faculty of Medicine, Khon Kaen University, 2010-03-06) Supat Sinawat; Taweesilp ChiyabutraBackground : Breast cancer is among the commonest malignancies found in women. Several studies indicated that there are some factors that might co-incidentally give rise to the development of both breast cancer and endometrial cancer. These factors include obesity, prolong reproductive phase and excess estrogen level. It is interesting, therefore, to investigate whether postmenopausal breast cancer patients treated in Srinagarind hospital have thickened endometrium or other abnormalities of reproductive organs.Objective : To determine the prevalence of endometrial thickening in postmenopausal breast cancer patients.Design : Cross-sectional descriptive study.Setting : Srinagarind hospital, Faculty of Medicine, Khon Kaen University.Subjects : Total of 66 postmenopausal breast cancer patients receiving treatment at Srinagarind hospital from 1 July 1999 to 31 August 2000 were included in the study. Patients who have been treated with hormones such as estrogen, progesterone or tamoxifen are excluded from the study.Measurements : Thorough history taking and physical examination as well as transvaginal ultrasonography were conducted in all patients.Results : Among the 66 patients included in this study, the mean age was 54.97 years. The mean body weight and mean body mass index was 57.95 kg and 24.48 kg/m2, respectively. The mean age at diagnosis of breast cancer was 51.62 years. The majority of patients (75.76%) had stage II disease. The mean + SD of endometrial thickness found in this study was 3.55 + 1.72 mm. The prevalence of thickened endometrium (defined as ET \> 5mm from TVS) was 10.60%. Other pelvic pathologies detected by ultrasonography were myoma uteri (4.55%) and ovarian mass (1.52%).Conclusion: The prevalence of thickened endometrium in postmenopausal breast cancer patients found in this study was interestingly high. It thus seems justified to propose that thorough pelvic examination as well as transvaginal ultrasound scan should be done in all postmenopausal patients diagnosed with breast cancer in order to detect endometrial and other pelvic pathologies that could arise in such cases.Item Academic Achievement Correlated to Stress, Depression, and Sleep Deprivation in Medical Students(Faculty of Medicine, Khon Kaen University, 2010-06-20) Kittipong KongsomboonBackground and Objective: Academic achievement is an important dimension for studying in university, so the effect on academic achievement is an important topic in medical training. The medical students have higher depression rates and stressful than students of other paculties. The medical training causes medical students to study hard at night and resulting sleep deprivation. Aims of this study were to determine the academic achievement and its association with stress, depression and sleep deprivation.Methods: All of the medical students from Srinakharinwirot University were selected. The design was cross-sectional study. The total subjects were 646; the response rate was 75%. Each participant was evaluated at the end of the second semester during February, 11th 2008 to March, 4th 2008. Each student was asked to answer the questionnaires including Health-Related Self-Reported (HRSR) Scale from Mental Health Department, Ministry of Public Health, Suanprung stress test. Categorical variables were analyzed using the Chi-square test or Fisher exact test. For continuous and binary response variables, we first identified significant explanatory variables through bivariate linear or logistic regressions, respectively. A p-value of less than 0.05 was considered significant.Results: Spearman’s rank correlation between academic achievement (grade) and duration of sleep, depression and level of stress showed correlation with duration of sleep and levels of stress (p \< 0.05) but not correlated to depression. Sleep 6 hours and more had odds ratio of academic achievement 1.8 times to sleep less than 6 hours (p \< 0.05) but depression and levels of stress were not significantly different.Conclusion: Poor academic achievement of medical students, correlated to sleep deprivation of less than 6 hours per day and correlated to increase levels of stress.Key words: academic achievement, stress, depression, sleep deprivation, medical studentsItem Acceptane First Trimester prenatal Screening for Thalassemia at Srinagarind Hospital(Faculty of Medicine, Khon Kaen University, 2010-05-05) Jamras Wongkham; Masinee Paibool; Thawalwong Ratanasiri; Witoon PrasertcharoensukBackground : Thalassemia is a hereditary anemia which is an important health problem of Thailand. About 30-40% of population are carriers while 1% of them are diseases. There were 1,000 thalassemia patients who received medical care at the out patient department of Srinagarind hospital per year. Screening pregnant women for thalassemia carriers or diseases and hemoglobinopathy is one method in controlling thalassemia.Objective : 1. To study the acceptance of screening for thalassemia in first trimester pregnant women. 2. To study the relation of demograpfic factors, history of preterm labor, family history of thalassemia, with screening acceptance and husbands’s acceptance of screenings and analytical.Design : Descriptive studySetting : Obstetric and Gynecology out patient department, Srinagarind Hospital.Subjects : The 299 first trimester pregnant women who had a first prenatal visit between 29 january and 31 July 1996 were chosen for the study.Measurements : Data was collected by questionnaires after the study group had received the information about thalassemia from brochures. The percentage, mean values and Chi-square or Fisher exact test were used for data analysis.Results : The first trimester pregnant women who accepted screening were 295 cases (98.7%). There were no factures that significantly affected acceptance. Of the 155 positive screening pregnant women, 124(80%) had their partner tested. The family income was the factor that affected partners’ acceptance.Conclusions : These results indicate that the acceptance of screening for thalassemia in the first trimester pregnant women including the acceptane of testing their partners when positive screening was high. This could be one of the effective methods in the thalassemia prevention program.Item Accessory abductor digiti minimi muscle(Faculty of Medicine, Khon Kaen University, 2010-03-13) Kowit Chaisiwamongkol; Wanchai Itarat; Weerachai KosuwanBackground: Accessory belly of the hypothenar muscles in hand have been involved in vascular and nerve compression. The most common variable muscle is abductor digiti minimi (AbDM). The purpose of this study is to investigate the anomaly of AbDM that is called AAbDM.Objective: To study quantity and describe the AAbDM in Thai people.Study design: A descriptive studySetting: The study was carried out at the Department of Anatomy, Faculty of Medicine, Khon Kaen University, Thailand.Subject: The study included six hundred hands from 300 formalin-embalmed cadavers, 25-82 years of age.Methods: 1. Study the number of hands which was found AAbDM. 2. Describe the origin, insertion and nerve supply of each muscle.Results: Only one hand from 600 hands was seen AAbDM. It was found in the right hand of Thai male cadaver aged 55 years. The origin of this muscle arises from pisiform bone, splitting from AbDM then forms aponeurosis and inserts into the base of the proximal phalanx of the fifth finger. The innervation is by the deep branch of ulnar nerve.Conclusion: Only one right hand from 600 hands was found AAbDM. So there are many kinds of alginates from several companies. Some are accepted by American Dental Association but others have not yet been accepted. This research studies the accuracy of casts produced from 8 kinds of alginates sold in the market. Only 5 of which are acceptable. The research measures the different dimension of casts produced from alginate impressions of the standard metal model. The first result is that the difference of dimension between casts and the standard metal model are significant (pItem Accuracy of Hadlock1 and Shepard Equation in Prediction of Intrauterine Fetal Weight(Faculty of Medicine, Khon Kaen University, 2010-01-02) Jaitip RujanawechBackground: Fetal weight prediction is very importance and will give benefit to both mother and fetus. Because both will get good support appropriately and timely. It will also give information to obstetrician to help making decision so to select the most accurate equation is very crucial.Objective: To develop the predictive intrauterine fetal weight equation by using real time ultrasound to measure fetal abdominal circumference and femur length. Methods: A prospective study was conducted during July 15, 2007 to December 20, 2007 in 133 pregnant women in the labour room of Nong Khai Hospital. Data analysis for frequency, percentage, mean, standard deviation, comparison of the accuracy between the two equations using odd ratio and regression analysis were used to bring up the new equation.Results: 136 fetuses were included in the study. Hadlock1 equation had more accuracy in weight prediction when compared with Shepard equation. However, the accuracy of predicted value within 10% of actual birth weight was only about 69.1%. So the new predictive intrauterine fetal weight equation was weight (gm) = weight calculate by Hadlock1 equation x 0.932 + 316.66 R = 0.891 and adjusted R2 = 0.792.Conclusion: The new predictive intrauterine fetal weight equation using real time ultrasound to measure fetal abdominal circumference and femur length had high accuracy for prediction of intrauterine fetal weight. It has more accuracy than Hadlock1 and Shepard equation. However, this new equation must be incorporated with each evaluation when determining this condition. To establish and develop personality in order to promote good leadership and remember the essential merit and that correlate to the social needs of students.2. To promote in Medical students and supervisors a better relationship which will lead to trust and consultationMethods: The students were asked to attend the camp and advisers encouraged to understand their role so that they had ability to extend group relations. After this the students attended a 2 day and 2 night camp with various activities. Each group had 1-2 supervisors for group relationship activities using games as a medium by which to present social situations and human behavioral analysis. There was a video show about development, community life, leadership and human relations. There were group planning conferences, project planning activities, self and family development group presentations and other activities. Assessment by collective observation and the use of questionnaires showed that the target objectives were achieved. As a result of the course students personalities were developed to produce good leaders as well as producing good team member of society. Good ethical beliefs were fostered as was the aim of being a worthy member of society. The right values of affection, good person to person relations a pride in the institute, adaptation to the school environment and to colleagues were achieved. A feeling of responsibility to society and to the Nation together with the promotion of relationships both to students and supervisors was established. The recommendation from students and participating supervisors is that it should be extended from 2 days and 2 nights to be 3 says and 2 nights by using a weekday and weekend or only the former which means doing the course after the students finish their exam. The location should be nor too far away from University. It should have a wide-enough conference room that suits various activities, a yard or sports ground that is suitable for physical or moral exercise of the students and nearby accommodation with an adequate restroom. Food should be arranged for everyone. This could be used by 3 rd year medical students so that they could prepare themselves for the clinical year. The assessment should be carries out regularly so as to improve weak points and become Faculty’s policy in the following academic year by training the 1st or 3rs year medical student and applying the formula that has been practiced by the 2 nd year medical student for the 2 st year medical student. Furthermore, the formula arranged for clinical year students to be demonstrates in the 3 rd year to medical students before they start their 4 th year. Subsequently it should be distributes so as to be part of the University plan.Item Achievement Evaluation of the 2004 Graduates in Technology in Biomedical Communications, Faculty of Medicine, Khon Kaen University.(Faculty of Medicine, Khon Kaen University, 2010-04-06) Chongchareon Metta; Piyathida Kuhirunyaratn; Dusadee Musikpodok; Songpol OopachitkulBackground: Achievement assessment is an essential task to the performance and characteristics of the graduates in Technology in Biomedical Communications . The result is useful information in regard to the graduates and quality assurance.Objectives : To evaluate the achievement of the 2004 Graduates in Technology in Biomedical Communications, Faculty of Medicine, Khon Kaen University.Methodology: Descriptive study design was used. The questionnaire with 1-5 Likert’s scale (5 = excellent, 4 = good, 3 = fair, 2 = poor, and 1 = very poor, need improvement) covering of 10 items in 4 aspects : (1) competency, (2) performance, (3) ethic and (4) total achievement were conducted, sending by mail to graduates and their employers. Data analyses were mean, standard deviation and chi-square.Results: There were 88.24% of response rate from the graduates and 100% from their employers, respectively. Total of achievement among the graduates was at high level. This study also found the achievement scoring by the graduates at 4.3 (SD 0.61) and achievement scoring by the employers was 4.06 (SD 0.96). There were no statistically significance between the graduate’s opinion and their employers (P\> 0.05).Conclusion: This study found achievement evaluation of the 2004 graduates and their employers at high level in 3 parts : competency, performance and total achievement. The ethics of the graduates were at the highest level and at high level from their employers. It has been found that to accomplish the target object. The students program needed to be supplemented \& develop in order to get better personality, leadership and member, good merit \& ethics in society. Appropriate values had to be emphasized such as unity, affection, relationships with one another, a price in the institute, adaptation to colleaques and the environment. These values also include the feeling of responsibility for society that effect the entire country in the future. The recommendation from students and lecturers who attended this program is that it should have there activities for the 2 nd year and the 3 rd year students and there should be a chane of locations for demonstration. The suggested place should be within a walking distance to the campus and provide proper accommodation and a conference room. Interesting \& effective media should be used. There main meals should be arranged and the light meal or snack could be reduced. Public relations procedure should be advertised throughroughly and lecturers should participate in the activities, The evaluation should be done regularly so as to adjust and improve the weak points and consequently become the faculty policy Further more. By collecting food, water and rectal swab specimens prescribed to examine the virility of the Severe Diarrhoea agent. The patient developed recurrent acute dacryocystitis 3 times in the past two years. Diagnosis was made by Gram staining and culturing pus from fistula. The patient was treated by combination of doxycycline and cotrimoxazole orally for 3 mouths. Dacryocystorhinostomy was performed to correct nasolacrimal duct obstruction. There was no recurrent infection in one year of follow up period.Item Acute Respiratory Infection in Children as a Cause of Admission to Srinagarind Hospital(Faculty of Medicine, Khon Kaen University, 2010-05-08) Srivieng Pairojkul; Jamaree Theeratakulpisan; Sureeporn HengA retrospective study of acute respiratory infection (ARI) as a cause of admission of children to Srinagarind Hospital was performed. From 1983 to 1987, ARI was a cause of admission in 14.7% of cases with pneumonia being the leading cause. Mortality rate from ARI comprised 12.3% of the total mortality of which 90% of the mortality rate from ARI was due to pneumonia. Of children who died from pneumonia 50% were under 1 year of age.Item Acute Rheumatic Fever in Adults : A Retrospective Clinical Analysis of 21 patients(Faculty of Medicine, Khon Kaen University, 2010-05-08) Ratanavadee NanagaraA retrospective clinical analysis of adult acute rheumatic fever was performed in 21 patients who were admitted in Srinagarind Hospital between 1984-1987. The mean age was 22 yr, sex ratio was F:M = 1:1.3 and most of them had previous rheumatic attcks. The most common major criteria was carditis (80%) and the second was arthritis (47%). Most of carditis patients had auscultatory murmurs (82%). Mitral regurgitation and mitral stenosis were equally found. The carditis could occur as the new carditis or on-top preexisting rheumatic heart diseases. All or the patients responded well to therapy and no one died in the acute phase of acute rheumatic attack. Other clinical manifestations, laboratory findings, antibiotic prophylaxis and relapse are discussed in details.Item Adnexal Torsion(Faculty of Medicine, Khon Kaen University, 2010-01-02) Kamonwan SaphanAbstract not availableItem Adolescent counselling service(Faculty of Medicine, Khon Kaen University, 2010-05-13) Chuanchom Sakondhavat; Yuthapong Werawatakul; Pannee Kukieattikool; Pattamavadee Pinitsoontorn; Soontorn Sakummai; Nikom ThamoseangA project on health education and heallh services regarding human reproduction and contraception among adolescents was carried out in 1987-1988. It was conducted by the family Planning Unit of the Department of Obstetrics and Gynecology, Faculty of Medicine, Khon Kaen University, with the support of the United Nations Population Fund. The project design was a longitudinal, prospective study. A total of 5,651 students (2,875 male and 2,776 female) from three vocational schools in Khon Kaen were recruited as the target population, ranging in age from 15 to 26 years. For the health services, special adolescent clinics were set up in schools, in Khon Kaen City and in Srinagarind (University) Hospital. A total of 761 students (13.5%) in the project used the services. Seventy-five percent of the students who came to the clinics came for counselling on sex-related health matters and 25% for general health. Males came twice as often as females. Forty-one percent of the students requested birth control, 16.1% came for sexually-transmitted disease treatment and 9% came for counselling on unwanted pregnancies. Fewer students living with their families (parents) came for health counselling than those living independently.
