Item Absorption and metabolism of paracetamol in rats drinking high concentration of arsenic.(2005-07-02) Islam, N; Siddique, M A; Misbahuddin, MThe effect of chronic intake of arsenic on the plasma concentration of paracetamol in rat was investigated. Rats received saline water with or without arsenic trioxide (10 mg/kg body weight/day) by gastric gavage on every alternate day for 29 days. A single dose of paracetamol (range 10 infinity 40 mg/kg body weight) was administered by gastric gavage to both arsenic-untreated and -treated rats on 30(th) day. Rats were sacrificed after 30 min and the amounts of free paracetamol and its metabolites in plasma were estimated using isocratic reverse-phase High Performance Liquid Chromatography (HPLC). Arsenic toxicity reduced the plasma concentration of paracetamol to 53 - 65% when compared with the rats received no added arsenic. There were maximum 67.4 and 76.9% inhibitions of sulfate and cysteine conjugations of paracetamol respectively. But arsenic had no effect on glucuronide and mercapturate conjugations. Both liver and small intestine showed increased accumulation of arsenic and decreased amount of glutathione in arsenic-treated rats. Successful emergency coronary arterial by-pass graft (CABG) and excision of the myxoma was done within 24 h of the onset.Item Assessment of Acute Kidney Injury in Patients Undergoing Elective Coronary Angiography and Percutaneous Coronary Intervention.(2012) Alam, A B M M; Moniruzzaman, M; Alam, M B; Islam, N; Khatoon, F; jahan, N; Ali, Z; Chowdhury, N UBackground: CIN has gained increased attention in the clinical setting, particularly during cardiac intervention but also in many other radiological procedures in which iodinated contrast media are used. There is at present good clinical evidence from well-controlled randomized studies that CIN is a common cause of acute renal dysfunction. Methodology: This was a prospective study conducted among the patients who underwent coronary angiography and percutaneous coronary intervention in the Department of Cardiology, Dhaka Medical College Hospital during January 2010 to December 2010. A total of 111 patients age range from 25 to 75 years were included in the study. Serum creatinine level at baseline and at the end of 48 hours was done in all these patients. Study population was divided into two groups according to development of acute kidney injury (AKI). Group-I = AKI, Group II = Not developed AKI. Results: AKI developed 11.7% of the study patient. DM and Preexisting renal insufficiency were significantly higher in group I patients. HTN was (61.5% Vs 44.9%) higher in group I but not significantly. History of ACE inhibitor/ARB, NSAID intake and LVEF <40% were significantly higher in group I patients. The mean±SD volume of CM (Contrast Media) were 156.9±44.8 ml and 115.4±30.0 ml in group I and group II respectively, which was significant. The mean±SD of serum creatinine after 48-72 hours of CAG/PCI was 1.4±0.37 mg/dl and 1.1±0.2 mg/dl in group I and group II respectively. The serum creatinine level increased significantly (p<0.05) after 48-72 hours of CAG/PCI in group I. In group II, S. creatinine level increased but not significant (p>0.05). Impaired renal function was found 76.9% and 2.0% in group I and group II respectively. DM, HTN, preexisting renal insufficiency, ACE inhibitor/ARB, NSAIDs, contrast volume (>150 ml), eGFR (<60 ml/min/ 1.73m2) and LVEF (<40%) are significantly (p0.05) associated for CIN development, Conclusion: CIN is an iatrogenic but preventable disorder results from the administration of contract media. Although rare in the general population, CIN occurs frequently in patients with underlying renal dysfunction and diabetes. In patients with pre angiographic normal renal function, the prevalence is low but in pre-existing renal impairment it may pose a serious threat. Thus risk factors are synergistic in their ability to predispose to the development of CIN. A careful risk-benefit analysis must always be performed prior to the administration of contrast media to patients at risk for CIN.Item Blood glucose and ischemic brain damage.(1998-01-15) Islam, N; Aftabuddin, M; Moriwaki, A; Koirala, T R; Hori, YThe effect of hyperglycemia on ischemic brain damage was studied in a rat model of incomplete ischemia. Incomplete ischemia was produced by permanent occlusion of one (either left or right) common carotid artery (CCA). Hyperglycemia was induced by intraperitoneal injection of 50% glucose, and same volume of physiological saline was injected in the controls 40 min before CCA ligation. Serum glucose level, at the time of vessel ligation, was 33.3 mMol/L. After CCA ligation, the rats were allowed to wake up and survive for upto 1 month. Perfusion-fixed brains were embedded in paraffin, subserially sectioned, and stained with haematoxylin-eosin/cresyl violet. Brain from sham-operated animals showed no damage neurons. Only mild neuronal damage was observed in saline pre-treated rats in CA1 area. Histological examination 24 h after CCA occlusion revealed ischemic neuronal cell damage to be more extensive in hyperglycemic rats. Neuronal damage was found in the major brain structures vulnerable to several insults. Some of those damaged neurons recovered well, but presence of some damaged neurons at 1 month of recovery suggesting delayed recovery. The results indicate that increased blood glucose level (hyperglycemia) during brain ischemia exaggerates structural alterations and leads to delay in recovery.Item Caesarean Myomectomy is No Longer a Dreaded Job: a Case Control Study in Multiple Specialized Centre.(2013-01) Zeba, D; Das, S R; Fatima, N; Islam, N; Uddin, J; Roy, R KRemoval of uterine myoma during caesarean section (CS) is not commonly done due to fear of severe haemorrhage which may lead to hysterectomy. Contradicting the previous belief many studies have been showing that myomectomy during caesarean section is a safe procedure without significant increase of risk. The objective of this study is to analyze the safety and clinical outcome of caesarean myomectomy. This is a prospective study carried out in different hospitals of Faridpur district, Bangladesh from 01.01.10 up to 31.12.12. Total 16 (study group) patients had undergone caesarean myomectomy. Operation time, peroperative blood loss, post operative complications and length of hospital staying was compared with that of 32 women (control group) with caesarean section alone. The result shows that peroperative blood loss was average 350±100 ml in study group which is 50 ml more than control group. Operation time was 20 minutes more and length of hospital staying was 1 day more than control group. There was no post partum haemorrhage, no hysterectomy was done at the time of CS and there was no maternal or perinatal mortality. So, caesarean myomectomy is safe and convenient to patient and cost effective without increasing any extra risk.Item Cardiac malignant lymphoma in severe combine immunodeficient mice.(1996-07-01) Aftabuddin, M; Islam, N; Alimuzzaman, M; Koiralla, T R; Hori, YExtranodal malignant lymphomas are known to occur with increased frequency in patients with human immunodeficiency virus infection. Although cardiac malignant lymphomas are rare, recently an increasing number of patients with acquired immune deficiency syndrome (AIDS) and cardiac lymphoma have been reported. To clarify the relationship between AIDS and cardiac lymphomas, we used 60 severe combine immunodeficient (SCID) mice as animal models in whom five different types of human lymphoma cell lines were injected. Primary lymphomas at the site of cell line injection developed within 14-30 days in 58 mice. Cardiac malignant lymphoma was detected in 10 cases (17%), among which tumour was macroscopically evident in 2. Tumour was present on the pericardial aspects of the heart extending into the myocardium in most cases, but no intracavitary lesion was observed. In one case, obstructive features were marked near the root of the great vessels by direct pressure of the growth. This study indicates that lymphomas have a predilection to invade the heart in immunocompromised conditions.Item Cardiac Troponin-I And CK-MB for Risk Stratification in Acute Myocardial Infarction (First Attack): A Comparative Study.(2011) Joarder, S; Hoque, M; Towhiduzzaman, M; Salehuddin, Af; Islam, N; Akter, M; Kamal, I MMyocardial infarction is associated with release of two important enzymes. The enzymatic diagnosis is mainly based on the measurement of CK-MB and troponin-I. Cardiac troponin- I(cTnI) is known to have higher specificity and analytic sensitivity than CK-MB for detection of myocardial injury & risk stratification. These are used both as diagnostic and prognostic marker. This prospective observational study included 60 patients of 40-65 years age range, diagnosed as acute myocardial infarction. The mean ages were 50± 8 years and 53±8 years respectively. Male and female patients included were 86.7% and 13.3%; BMI was 25.3±1.5. The two important cardiac markers troponin-I and CK-MB were studied in 60 patients, admitted in the hospital with acute MI. Blood samples to estimate these markers were collected from the patients after admission at 6-9 hours, 9-24 hours and after 24 hours and their mean values with ±SD were calculated, evaluated and compared between the two groups of patients with low and high risk MI. The patients with low risk MI were those who recovered early and the high risk patients improved later in comparison to low risk group. Out of 60 patients, 37 had troponin-I level>1.5 ng /ml. Among them 29 developed high risk MI and 8 recovered earlier than high risk group. 23 patients had troponin-I <1.5 ng /ml, out of whom 10 were high and 13 were low risk. The difference of troponin-I levels between high and low risk groups of patients was statistically significant (p<0.01). On the other hand CK-MB level was >7 ng /ml in 33 patients. Out of them 22 patients developed high and 11 patients were low risk but 18 patients out of 27 who had CK-MB <7 ng /ml became high and 9 patients were low risk. The difference of outcome in respect to higher and lower values of CK-MB between the two groups was not statistically significant (p>0.05). Both troponin-I and CK-MB were estimated in all 60 patients on three occasions. The mean troponin-I levels were statistically significant between the high and the low risk groups on all occasions .On the contrary, the values of CK-MB were not statistically significant on two occasions but was significant (p < 0.01) on one occasion when it was estimated at 9 - 24 hour. Serum cTnI is better and more characteristic biomarker than CK-MB for risk prediction and prognosis evaluation in AMI patients.Item Carotid artery occlusion causes reversible brain damage in rat.(1994-05-01) Aftabuddin, M; Islam, N; Moriwaki, A; Hori, YIn order to assess the density and distribution of brain damage after pure focal ischemia uncomplicated by hypotension or hypoxia, ischemia was produced by occlusion of one and/or both common carotid arteries (CCA) temporarily or permanently in the rat, followed by 3 months recovery. The brains were perfusion-fixed with formaldehyde, embedded in paraffin, subserially sectioned, and stained with hematoxylin-eosin/cresyl violet. Mild ipsilateral neuronal damage was observed after unilateral permanent occlusion, but confined only to the hippocampus. Damage of neocortical neurons began to appear after 1 day following bilateral permanent occlusion. Damage was worst over the superolateral convexity of both hemispheres, CA 4 and subiculur neurons; subsequently other sectors of hippocampus and other brain structures were affected. Some damaged neurons in the superficial cortex and CA 1 sector of hippocampus, and almost all in the other regions of brain reverted to normal profile by 3 months of recovery. All brain structures showed normal profiles, except a relative neuronal rarefication in neocortex and CA 1 sector of hippocampus after 3 months of recovery. These results suggests that only CCA occlusion causes partial/incomplete ischemia in the rat brain, where damaging effects are counterbalanced by metabolic and circulatory adjustments.Item Cirrhosis of liver in Bangladesh. (A preliminary report).(1975-04-01) Islam, N; Khan, MThe incidence of cirrhosis of liver in Bangladesh, its possible aetiological factors and clinical features have been studied. This being the initial report, embraces the population of only two hospitals in Dacca viz., Institute of Postgraduate Medicine and Research (P.G. Hospital) and Sir Salimullah Medical College Hospital situated three and a half kilometers apart. In a population of 5889, a total of 154 cases (2.6%) of cirrhosis of liver have been recorded. Nutritional deficiency has been considered to be the important aetiological factor whereas alcoholism does not appear to have any significant role. Almost all the cases (94.2%) were non-alcoholic. Pattern of clinical features have been compared with those of other authors and variations recorded.Further study on an elaborate scale has been suggested.Item Cirrhosis of liver.(1981-12-01) Islam, N; Khan, M; Ahmed, ZClinical analysis of 293 cases of cirrhosis from two moderate sized hospitals in the city of Dacca has been presented. Maximum number of cases were in the age group over 40 with 150 (51.2%) males and 19 (5.8%) females. Significant past history included viral hepatitis (21.5%), kala-azar (11.6%) and malaria (10.24%). History of alcoholism was present only in 16 (5.5%) cases. Weakness (84.3%), weight loss (72%) and anorexia (39.3%) constituted the most common symptoms. Ascites (45%), haematemesis (11.6%) and melaena (28.7%) were the next common symptoms. Hepatosplenomegaly was found in about one-third of the cases. Testicular atrophy was recorded in 41.63% cases whereas gynaecomastia was relatively less common (5.5%). Scanty body hair and white nails were present in almost equal number of cases (14.7% and 18%). The cases presented here are those with overt manifestation. Nevertheless, the clinical features are not materially different from those reported by other authors. In the absence of alcoholism, viral hepatitis is presumably the most important aetiological factor in our cases and the clinical features compare favourably with non-alcoholic cirrhosis of the western writers. Cryptogenic cirrhosis has been considered to be most common type constituting 43.7% of our cases.Item Cirrohosis of liver in Bangladesh, part--II.(1976-06-01) Islam, N; Choudhury, N A; Ali, S MItem Clinical profile: prognostic index in hepatocellular carcinoma.(1991-10-01) Khan, M; Zaki, K M; Ahmed, K U; Ali, S M; Islam, NA clinical study and follow-up of 77 patients (63 males and 14 females) with hepatocellular carcinoma with age range from 22 to 80 years were collected from the Institute of Post Graduate Medicine and Research and eight private hospitals from Dhaka City. Past history of transfusion was present in 16 (20.8%), Jaundice in 20 (26%) and 13 (16.9%) patients had associated cirrhosis. HBs Ag was positive in 17 (33.33%) out of 51 patients and liver ultrasound suggested hypoechogenic lesion in 44 (57.2%) patients. CT was performed in 7 (9.1%) and in one MRI was done. Eight (50%) out of 16 patients had alphafetoprotein ranging from 1000-12000 ng/ml. Space occupying lesion was detected in 25 (71.4%) out of 35 cases by isotope scan and needle biopsy was confirmatory in 25 (32.5%). Commonest presentations were abdominal lump (96.2%), weakness (79.3%), weight loss (74%), and loss of appetite (78%). Fifty six (72.2%) patients were followed weekly till death (2.9 +/- 2.4 months). The mean survival was higher under 30 years (5.9 +/- 3.7 months; P less than 0.05). Serum bilirubin above 5 mg/dl with HCC also had poor prognosis (1.6 +/- 0.8 months; P less than 0.01) Those who had prothrombin time higher than 16 seconds died earlier (1.6 +/- 0.7 months; P less than 0.01). Survival was poor in those who had the tumour size over 7 cm (2.5 +/- 0.9 months; P less than 0.01).Item Co-localization of c-fos protein and protein kinase C gamma in the rat brain following anodal polarization.(1995-07-01) Islam, N; Moriwaki, A; Hori, YThe expression of protein kinase c gamma (PKC gamma) and c-fos protein was examined by means of double labeling in the rat brain in relation to the molecular mechanism of central plastic changes associated with anodal polarization. Under normal, non-polarized condition, approximately 75% of all fos positive neurons in the neocortex were immunopositive for PKC gamma. Conversely, nearly all PKC gamma positive neurons were fos immunopositive. Although both pyramidal and non-pyramidal neurons express both types of protein, the pyramidal cell type represents the vast majority. An anodal direct current of 3.0 microA for 30 min to the surface of the left sensorimotor cortex resulted in a pronounced increase in the intensity of immunoreactivity for both PKC gamma and c-fos protein ipsilateral to the polarization. Approximately, 91% of fos positive neurons in the polarized neocortex was also intensely immunoreactive for PKY gamma. The high degree of codistribution of both transduction proteins in specific neurons following anodal polarization suggests the functional connection between PKY gamma activation and c-fos expression in polarization phenomenon.Item Comparative study of lung functions in women working in different fibre industries.(2008-07-16) Khanam, F; Islam, N; Hai, M AA cross sectional work has been done on Bangladeshi females, working in different fibre industries, to study the effect of exposure to fibre dust on pulmonary functions. The ventilatory capacities were measured by VMI ventilometer in 653 apparently healthy women (160, 162 and 167 were jute, textile and garment industry workers, respectively). For the controls 164 females were recruited who never worked in any fibre industry. The observed FVC, FEV1 and PEFR were lower in all groups of fibre industry workers than those of the control. Among the industry workers, the jute mill workers had the lowest ventilatory capacities and garment industry workers had the highest values. The jute and textile mill workers had also significantly lower FEV1 and PEFR than those of garment industry workers. The FEV1 and PEFR were significantly lower in jute mill workers than those of textile ill workers. The low ventilatory capacities were almost proportionate with the length of service of the workers. Thus, the present study indicates that the fibre dust, on regular exposure for longer duration, may limit the lung functions.Item Complications of cirrhosis of liver in some hospitalised population.(1985-06-01) Khan, M; Islam, NA total of 323 cases of cirrhosis of liver with various complications in some hospitalised population were studied. Microcytic anaemia was found to be the commonest (46.43 percent) complication. Next common (21.05) complication was protosystemic encephalopathy (PSE). Gastrointestinal haemorrhage and primary carcinoma of liver constituted 12.37 percent and 9.28 percent respectively. In view of the nature of complications of liver, it was felt necessary that these type of patients should be exposed to facilities for the specialised management whenever possible. Development of primary carcinoma of liver emphasises the need for the prevention of hepatitis B virus infection for which preventive vaccine is now available.Item Drug policy in Bangladesh.(1987-01-01) Hasan, M; Islam, NItem Echinococcosis in cattle and goats.(1976-12-01) Islam, N; Rashid, H; Winter, PIn the 500 cattles examined, 90 (18%) had hydatid cysts in one or more organs whereas only 12(2.4%) goats, out of 500 had the disease. This is considered to be due to shorter period of longevity and age of slaughter of goats compared to catte. Since dog is an important definitive host responsible for the spread of echinococcosis, a survey on the incidence of hydatid cyst in dogs is necessary from public health point of view.Item Effect of Ocimum Sanctum Linn (Tulsi) on body weight and some biochemical parameters in restraint stressed albino rats.(2008-12) Siraji, D; Islam, N; Begum, N; Ferdousi, SBackground: With the increased trend of urbanization of our society, the incidence of various stress related diseases are increasing day by day. People under chronic stress often tends to seek relief through drugs, which may have side effects. But some natural supplements called adaptogen, can be more beneficial in reducing symptoms of stress. Objective: The present animal model experimental study was designed to observe the effects of restraint stress on body weight, serum ALT, AST, Glucose, Cholesterol and their modifications by Ocimum sanctum Linn (tulsi) pretreatment which is an Ayurvedic adaptogen and has a long history of therapeutic use. Study design: For this purpose, 30 albino rats aged 90 to 120 days were included in this study. Twenty rats of experimental group were further subdivided into two groups. One group consisted of 10 rats exposed to one hour restraint stress daily for 7 days and 10 rats of the second group were pretreated with tulsi for 7 days before exposure to stress in the same way. 10 non stressed , non pretreated rats were taken into control group.This study was undertaken in the laboratory of the Department of Physiology,BSMMU during the period from July 2003 to June 2004. Methods: The body weight of all control rats were recorded daily in the morning. Body weight of the experimental rats were recorded just before exposure to stress. After completing the experiment , the animals were sacrificed and blood was collected . Serum glucose, cholesterol and ALT,ASTwere determined by standard laboratory technique. Data were compared among the groups and the results were statistically analyzed using unpaired student t’ test. Results : The body weight in untreated stressed group was significantly lower (p<0.001) than those of the control group and tulsi pretreated group. Serum levels of glucose, cholesterol, aminotrasferases (ALT and AST) were significantly higher (p<0.001) in stressed group than those of control . Again in Tulsi treated group all these biochemical parameters were significantly lower (p<0.001) than those of stressed group. This restraint stress-induced changes in body weight and biochemical parameters may be due to hypophagia, altered secretion of various metabolic hormones and neurotransmitters, changes in membrane permeability & hypovolaemia resulting from stress induced secretion of corticosterone and epinephrine through hypothalamo-pituitary adrenal axis activation. Conclusion: Prevention of stress induced changes in biochemical parameter by tulsi pre treatment indicates its anti stressor effect.Item Effects of anodal polarization on protein kinase Cgamma (PKCgamma) in the rat brain.(1997-07-01) Islam, N; Aftabuddin, M; Moriwaki, A; Hori, YAn anodal direct current of 0.3 microA or 30.0 microA was unilaterally applied for 30 min or 3 hr to the surface of the sensorimotor cortex of rats, and the effects of anodal polarization on protein kinase C (PKCgamma) activity were examined. The brains were processed by means of immunocytochemistry using the monoclonal antibody 36G9 raised against purified PKCgamma. In sham-operated animals, PKCgamma-like-immunoreactivity (PKCgamma LI) was noted in neuronal cytoplasm, as well as in processes in the cerebral cortex and in the hippocampus. Anodal polarization with 3.0 miroA for 30 min resulted in a pronounced increase in the number of PKCgamma-like-positive neurons in accordance with the intensity of immunostaining in the cerebral cortex, and an increase in the polarized hemispheres was highlighted by repeated applications of the currents. Polarization with 0.3 microA for 3 hr also increased the PKCgamma LI, but 0.3 microA for 30 min or 30.0 microA for any duration had no effects. The effect of polarization on PKCgamma activity, as evaluated by the intensity of immunostaining and the number of neurons, began to increase 1 h after polarization, peaked at 3 hr and thereafter decreased to the control levels by 72 hr. These results indicated the involvement of the gamma-isoform of PKC in the neurochemical mechanism of long-standing central and behavioral changes induced by anodal polarization.
