Item 99m Tc DTPA scan as a diagnostic marker of acute rejection in renal transplantation.(1994-04-01) Kohli, H S; Gupta, A; Arora, P; Mishra, V K; Kher, V; Bhandari, M; Ghambir, S; Kumar, A; Sharma, R K; Das, B KForty renal transplant recipients were subjected to 99m Technicium Diethylene triamine pentacetic acid (DTPA)scans at regular intervals & whenever there was suspection of rejection. Serial scans of a group of 15 recipients from immediate post transplant period till withdrawal of cyclosporine were analysed separately & the results compared to with single scan analysis. The sensitivity & specificity of DTPA scan in the absence of acute tubular necrosis (ATN) was 94.1% & 87.5%, while the positive & negative predictive values were 88.8 & 93.3% respectively. Sensitivity & specificity of serial scan analysis (88.8% and 75%) in early post transplant period was higher than that of interpretation of single scan (75% & 66%). Serial scan changes predated clinical rejection during cyclosporine withdrawal period. We conclude that DTPA scan is both a sensitive & specific non-invasive diagnostic marker of acute rejection in absence of ATN & serial scans during early post transplant period & at the time of cyclosporine withdrawal are helpful in detecting the rejection accurately & at the earliest.Item Abdominal tuberculosis with special reference to pathological aspects.(1975-12-01) Vasantha, V C; Habibullah, C M; Kumar, AItem Abnormal erythrocyte membrane phospholipid organisation in chronic myeloid leukaemia.(1987-03) Kumar, A; Daniel, S; Agarwal, S S; Gupta, C MThe membrane phospholipid organisation in the red cells of humans suffering from chronic myeloid leukaemia has been analysed using the amino-group labelling reagent trinitrobenzenesulphonic acid and the fluid-sensing fluorophore, Merocyanine 540. Unlike the normal human erythrocytes, trinitrobenzenesulphonic acid in intact chronic myeloid leukaemia erythrocytes modified about 30% phosphatidylserine, under controlled conditions. Also, the chronic myeloid laukaemia red cells, but not the normal cells, were found to bind the fluorescent dye Merocyanine 540. These results demonstrate that loss of the transmembrane phospholipid asymmetry in chronic myeloid leukaemia erythrocytes is accompanied by an enhancement in the outer surface fluidity and, therefore, suggest that the red cells membrane phase-state asymmetry originates probably from the asymmetric arrangements of phospholipids across the membrane bilayer.Item Abrogation of dengue virus-induced alterations of Fc-receptor functions of macrophages by splenectomy or drug treatment.(1985-06-01) Nagar, R; Chaturvedi, U C; Kumar, A; Mathur, AItem Accuracy of self reporting malaria in Orissa--a case study.(1994-12-01) Kumar, A; Sharma, R CFor rapid assessment of malaria in difficult forested areas in Orissa a study on the accuracy of self-reporting of malaria showed 72 per cent accuracy along with the sensitivity and specificity values of 46 per cent and 76 per cent respectively. The study clearly indicates its utility in the ongoing national malaria eradication programme and can be effectively used for rapid assessment of the disease prevalence. The information so gathered can be used for rapid assessment by the planners and programme managers in devising the strategies for the containment of malaria in the forested areas of Orissa.Item Acquired double depressor palsy following acute haemorrhagic conjunctivitis.(1988-01-01) Prakash, P; Menon, V M; Gupta, A K; Kumar, AItem Acquired immunodeficiency syndrome (AIDS) in multitransfused children with thalassemia.(1993-04-01) Sen, S; Mishra, N M; Giri, T; Pande, I; Khare, S D; Kumar, A; Choudhry, V P; Chattopadhya, D; Kumari, S; Malaviya, A NTwo hundred and three multi-transfused children with thalassemia attending the Thalassemia Clinic of the Charak Palika Hospital, New Delhi were screened for human immunodeficiency virus (HIV) antibodies by ELISA and all positive cases were confirmed by Western Blot. Of the 203 children screened, 18 (8.9%) were HIV positive, and in these children a detailed immunological work up was done and compared to 18 age-matched HIV negative thalassemics as controls. The tests included absolute lymphocyte counts (ALC), absolute and percentages of CD4+ and CD8+ cells and their ratios (CD4/CD8), immunoglobulin levels (IgG, IgM and IgA) and delayed cutaneous hypersensitivity (DCH) test by Multitest CMI in all the cases and the controls. Of the 18 HIV positive children, 6 were diagnosed to have clinical AIDS as per the WHO criteria. After immunological testing, the children were further classified according to the CDC criteria. By these criteria, 11 children were classified as P1 A (asymptomatic infection, normal immune function), 1 child as P1 B (asymptomatic infection, abnormal immune function), 2 children as P2 A (symptomatic infection with non-specific findings), 1 child as P2 C (lymphocytic interstitial pneumonitis), 1 child as P2 D1 (Pneumocystis carinii pneumonia) and 2 children as P2 D2 (symptomatic infection with infections). In this paper, the clinical features of the children with AIDS is described, and the immunologic functions of these children are compared with the HIV positive asymptomatic children and with controls. A male preponderance was found and 83.3% of cases were below 5 years of age. The interval between onset of measles rash and that of encephalopathy ranged from 0 to 20 days. Fever (96.6%), convulsions (70%), and coma (90%) were found in a high percentage of cases, while focal neurological deficits and abnormal movements were found in a small number of patients (3.3% and 16.6% respectively). Signs of cerebellar or medullar involvement were not seen. Only 1 (3.3%) case had pleocytosis in the cerebrospinal fluid. Eleven (36.6%) patients died in hospital. The patient had many unusual features like coronary ectasia and atherosclerosis within the myocardial bridge segment. Eight patients with ANLL whose blast coexpressed the T cell associated CD7 antibody were identified among 462 consecutive ANLL cases. Seven had FAB defined AML according to morphocytochemical criteria, whereas one patient was classified as MO on the basis of ultrastructural studies. The incidence of CD7 positivity was particularly significant in the less differentiated sub-types MO and M1 compared to other FAB sub-groups. Detailed long term studies will be required to realize their biological and clinical significance.Item Acute renal failure due to rifampicin.(1990-04-01) Agnihotri, M S; Bansal, S; Kumar, AA 23-year-old male patient with bacteriologically proven pulmonary tuberculosis was treated with the various regimens of antituberculosis drugs for nearly 15 months. Rifampicin was administered thrice as one of the 3-4 drug regimen and each time he developed untoward side effects like nausea, vomiting and fever with chills and rigors. The last such episode was of acute renal failure at which stage the patient was seen by the authors of this report.
